Northwest Family Medicine - St. Maries Clinic is seeking a compassionate, patient-focused RN to join the team!

Under the supervision of the Practice Supervisor, the Registered Nurse directs and/or provides professional nursing care for an assigned group of patients across the age continuum. Utilizes the nursing process of assessment, planning, implementation, and evaluation to direct or provide nursing care. Responsible for delivering patient care services in a Family Medicine setting. This position will focus on assessing, identifying, and solving nursing care and pre-operative problems during the pre-operative phase. This position will assist with emergency procedures, providing medications, and initiating intravenous fluid replacement. Demonstrates knowledge of equipment, instruments, best nursing practices, standards, and protocols. The Registered Nurse will need to project a professional demeanor and appearance while maintaining the confidentiality of the Hospital, patients, and employees as appropriate. • Assists the attending Transplant surgeons, house staff, and Transplant staff with the direct care of hospitalized patients and outpatients on a rotating basis with the other clinical care coordinators, including: facilitation of patient discharges and transplant teaching
• Attends once a week outpatient Transplant clinics: RN orders labs, performs injection teaching, coordination of care

Schedule appropriate non-invasive laboratory tests and participates in the interpretation of the results and the institution of appropriate therapy based off physician guidances

• Schedules appropriate surgery and tests for patients
• Discharge Planning
  Reviews discharge instructions to hospitalized patients
  Works with hospital case manager and social worker regarding patient discharge needs
• Answers telephone inquiries from patients and/or visiting nurses after hospital discharge
• Education of health care workers and the public regarding organ transplantation
• Participates in teaching programs of the Abdominal Organ Transplant Program
• Additional responsibilities as needed.

Patient Care and Monitoring:

• Monitor Transplant recipients after surgery for complications or rejection and report to Transplant
  Physician
• Review lab results and biopsy reports
• Assess patient symptoms and clinical status
• Coordinate follow-up visits and routine post-transplant evaluations

Medication Management:

• Monitor drug levels
• Adjust medications based on provider orders
• Educate patient of medication changes and adherence

Provide Education and support to lifestyle changes, infection prevent, medication schedules, emotiona support, provide resources, and warning signs of complications inpatient and outpatient.

    The Rehabilitation teams at Samaritan evaluate and treat patients with injuries or conditions that have caused either a decline in self-care, physical mobility or functional independence. We are a collaborative multi-disciplinary team of therapists offering cutting-edge, evidence based practice and compassionate individualized treatment. We provide specialty care from pediatrics to geriatrics, from neurological to women's health, from lymphedema care to wheelchair fitting and more. Our acute services are centered out of Samaritan's Regional Medical system, a 160 bed, level 2 trauma center. Our hub status affords us a complex patient census that is among the state's top 5 in acuity. Our outpatient clinic features cutting edge equipment, a therapy pool with an underwater treadmill and separate adult and pediatric gyms. Our Medical Renal unit specializes in caring for patients with kidney-related conditions while also supporting a broad general medical population. Nurses on this team gain valuable experience managing complex renal diagnoses alongside a variety of acute medical conditions in a collaborative, fast-paced environment.

Our patient population includes individuals with renal failure, dialysis needs, acute kidney injury, urinary tract infections, and other kidney-related diagnoses, as well as patients with general medical conditions requiring inpatient care and monitoring.

• Education:
  - Graduate of an accredited school of nursing required.
  - 4 Year / Bachelors Degree, preferred.
  - If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. What you will do in this role:

• Assess the patient's condition during admission and each scheduled shift, promptly identifying and reporting any changes in patient status.
• Perform procedures, monitoring, or other functions as ordered by the medical provider(s), and ensure thorough and timely documentation of care administration in the patient's medical record. 
• Administer prescribed medications, monitor the patient for therapeutic response, and take appropriate action in the event of an unintended response to the medication.
• Provide exceptional care by responding promptly to patient requests, proactively anticipating patient needs, and resolving them.
• Educate patients, families, and caregivers about the patient's medical condition, treatment plan, medications, possible side effects, and follow-up measures, ensuring complete understanding by translating complex medical terminology.

• Required Education: Bachelor of Science in Nursing or enroll in a BSN or advanced degree program and obtain a BSN within 5 years of hire date into RN position. Grandfathering will be accepted between departments and Mission Health entities. Exemptions, or extensions, may be approved by the facility CNO and reported out to the CNO Council members. Each facility will ensure tracking of exemptions should this information be needed. RNs with 10 years or more of experience are exempt
• Preferred Education: Masters in Nursing. 
• Required License/Certifications: Must have and maintain current licensure as registered nurse with the North Carolina Board of Nursing. eNLC (Enhanced Nurse License Compact) license may apply, licensee should confirm with NCBON Certificate: BLS Healthcare Provider 
• Preferred License/Certifications: National Certification in area of specialty 
• Required Experience: One year nursing experience
