Overview:

The ideal MDS Candidate is very detailed orientated, organized, self-motivated, while still being bedside for resident assessment and meeting with families. Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.

How you Will make a Difference:
  • Collect information to complete the MDS using the medical record, bedside assessment, and staff, resident and/or family interviews.
  • Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met. Review Plan of Care at least quarterly and with each Comprehensive Assessment to assure changes during the quarter are included and updated. 
  • Provide ongoing education of the RAI process to all interdisciplinary team members, direct and indirect caregivers, business office, etc. with all changes and as appropriate.
What you Need to make a Difference:
  • Registered Nurse with required current state licensure. 
  • Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred. 
  • Minimum one (1) year of MDS experience.

Parker Personal Care Homes (PPCH) is looking for dedicated, honest, and responsible individuals to provide compassionate care and support to adults with Intellectual or Developmental Disabilities in your home and as a part of your family.

We look for folks who can offer services and supports that might include personal care, problem solving, safety in the community, learning new skills, increasing levels of independence, managing challenging behaviors and/or medical needs.

You must be willing to provide a home that is safe and supportive while assisting with activities of daily living, recreation and socialization, health and safety concerns and medical appointments. We provide all of the necessary training for our providers to be successful, including training in the areas of developmental disabilities, CPR, First Aid, medication administration, behavioral safety training, members' rights, confidentiality, and more.

What is a Host Home setting like?

PPCH has numerous host homes located throughout the metro area. Each host home is unique in its area of specialty, general layout and physical location.

What are the responsibilities of a Host Home Provider?

The Host Home Provider's main focus is to provide a safe and supportive environment for the member(s) in services in their home. Each Host Home Provider is responsible for ensuring the health and safety of the individual living in their home, while ensuring they will have plenty of opportunities for socialization and access to community resources.

What type of training does a Host Home Provider receive?

PPCH provides specialized trainings to all providers. These trainings include CPR/First Aid, Medication Administration, Customer Rights & many other areas. All host homes are monitored closely by PPCH to ensure that the member in services is receiving the highest level of care.

Qualifications

Must be able to provide services and support for 1 member in your own home on an on-going basis
Must successfully complete PPCH’s required training courses
Must be able to chart individual daily member activities and medications in a web-based system (training is provided)
Have professionalism and compassion
Able to focus on individualized care and support
Ability to teach others
Have good written and verbal communication skills
Have a valid driver's license and reliable transportation
Must clear background screening and reference checks

The PCPP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.

ESSENTIAL JOB DUTIES/RESPONSIBILITIES:

  • The PCPP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
  • The PCPP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
  • It is expected that the PCPP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
  • The PCPP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
  • The PCPP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
  • For patients that are unable to come to the office—in hospital, SNF, LTC or homebound, PCPPs will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
  • PCPP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the market.
  • Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with, an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
  • Performs other duties as assigned and modified at manager’s discretion.

Provides compassionate, prescribed administration of hyperbaric oxygen therapy and/or related testing to HBO patients, safe, effective operation and maintenance of the hyperbaric facility, data entry, and other functions as prescribed by Certified Hyperbaric Oxygen & Safety Technician and/or Clinical Coordinator.
Responsibilities:

* Utilize required skills and education to deliver effective, compassionate patient care under guidance of Clinical Coordinator, CHBO & Safety Technician and Medical Director.
* Performs HBO Chamber operations and maintenance as required.
* Communicates maturely with all for the betterment of the Center.
* Maintains complete, accurate necessary HBOT patient care data for effective HBOT patient care.
* Performs other functions within Center as reasonably assigned by Clinical Coordinator and CHBO & Safety Technician.

Requirements:
Education:

* High school diploma or general education degree. Must have approved Hyperbaric Medicine introductory course. Medical training as exemplified in licensure/certifications itemized below.

Experience:

* Hours required to become certified.
* Must have obvious concern for patient welfare
* Basic/operable computer skills
* Ability to read, understand and implement professional journals as well as regulatory documents
* Ability to perform basic/operable mathematics
* Possess ability to communicate effectively
* Be particularly attentive to detail
* Ability to speak and read Spanish a plus.

Certifications, Registrations, or Licenses:

* Current certification as Certified Hyperbaric Technologist.
