The Registered Professional Nurse is responsible for the delivery and coordination of patient care provided by the caregiving team. S/he assures that competent, compassionate patient care is uniformly provided to customers including referring facilities, hospital staff and patients in the inpatient, ambulatory, and community settings.

Clinical Duties:

  • ο»Ώο»ΏAssists the attending Transplant surgeons, house staff, and Transplant staff with the direct care of hospitalized patients and outpatients on a rotating basis with the other clinical care coordinators, including: facilitation of patient discharges and transplant teaching
  • ο»Ώο»ΏAttends once a week outpatient Transplant clinics: RN orders labs, performs injection teaching, coordination of care

Schedule appropriate non-invasive laboratory tests and participates in the interpretation of the results and the institution of appropriate therapy based off physician guidances

  • ο»Ώο»ΏSchedules appropriate surgery and tests for patients
  • ο»Ώο»ΏDischarge Planning
    Reviews discharge instructions to hospitalized patients
    Works with hospital case manager and social worker regarding patient discharge needs
  • ο»Ώο»ΏAnswers telephone inquiries from patients and/or visiting nurses after hospital discharge
  • ο»Ώο»ΏEducation of health care workers and the public regarding organ transplantation
  • ο»Ώο»ΏParticipates in teaching programs of the Abdominal Organ Transplant Program
  • ο»Ώο»ΏAdditional responsibilities as needed.

Patient Care and Monitoring:

  • ο»Ώο»ΏMonitor Transplant recipients after surgery for complications or rejection and report to Transplant

Medication Management:

Provide Education and support to lifestyle changes, infection prevent, medication schedules, emotiona support, provide resources, and warning signs of complications inpatient and outpatient.

This Physician Assistant position is a key member of the Acute Care Surgery & Critical Care (ACS&CC) team, providing clinical support in the emergency department (ED) during peak hours for trauma and surgical patients. The PA will collaborate closely with attending surgeons, surgical residents, of the ACS &CC care team to deliver high-quality care to surgical patients. Primary Responsibilities:

* Represent the ACS&CC team in the Emergency Room as a provider
* Provide back up for inpatient medical-surgical floor ACS APP providers
* Perform ED consultations with comprehensive patient assessments, histories, and physical examinations for the ACS&CC service line
* Perform procedures and wound care as required in the ED
* Deliver care to patients of the ACS&CC division that are "boarded" in the ED
* Respond to Level 3 traumas and provide trauma consultations.
* Coordinate patient discharges and ensure follow-up care is arranged
* Respond to Emergency Department ACS&CC consults and provide timely evaluations
* Document all clinical encounters and procedures accurately in the electronic medical record (EMR)
* Communicate effectively with patients, families, and team members
* Effectively partner with other members of the ACS & CC team (surgeons, APP, residents, medical students)
* Weekday or Weekend Shifts
* Conduct one quality improvement project annually

Qualifications:
