Key Responsibilities:

Required Qualifications:

The Ambulatory Care clinical pharmacist provides direct patient care within an ambulatory setting,
practicing under approved Collaborative Practice Agreements (CPAs). This role focuses on
comprehensive medication management, chronic disease state management, medication access
optimization, and population health initiatives. The pharmacist collaborates closely with primary care
providers and the interdisciplinary care team to initiate, modify, or discontinue medication therapy in

Summary
JOB SUMMARY/PURPOSE
Provides clinical pharmacy services to ambulatory patients. Provides comprehensive medication management in multiple areas of practice, including but not limited to anticoagulation, perioperative management of anticoagulants and antiplatelets, anemia management with erythropoiesis stimulating agents, diabetes, hypertension, hyperlipidemia, heart failure, COPD, asthma and opioid use disorder. Ensures departmental efficiencies and compliance with regulatory bodies. Instructs other pharmacists, pharmacy residents and pharmacy students.
DEPARTMENT DESCRIPTION
The Medication Management Clinic provides comprehensive pharmaceutical care and care coordination to ambulatory patients who are on high-risk medications or who require disease state management.
EXPERIENCE/EDUCATION/QUALIFICATIONS
Current unencumbered Oregon Pharmacist and Pharmacist Preceptor licensure required.
Healthcare Provider Level BLS required within 90 days of hire.
Previous ambulatory care clinic experience preferred. Alternatives may include completion of a PGY 1 residency with rotations in ambulatory care and anticoagulation rotation, completion of a PGY 2 residency in ambulatory care, or board certification in ambulatory care.

The Speech Language Therapist or Clinical Fellow will be responsible for the administration and direct application of Speech-Language pathology treatment modalities as prescribed by a referring physician. If a Clinical Fellow, they will be supervised by an onsite Speech Language Pathologist as per protocol until they obtain their SLP license following the completion of the Clinical Fellowship. Duties include evaluating, planning, directing, and treating all patients referred to Speech-Language Pathology. Rehab Department Hours: Hours of operation are Monday-Thursday, 7:00am-6:30pm, and Friday, 7:00am-5:00pm. Saturday and Sunday are on an on-call, as needed basis. The Rehab Department utilizes Epic.
Primary Responsibilities:
- Patient evaluations - Perform comprehensive assessments and review medical histories and physician referrals to determine therapy needs
- Establish goals to enhance independence with daily living - Develop personalized care plans based on patient goals, diagnosis, and progress and adjust as needed
- Therapeutic Interventions - Administer therapeutic exercises, manual therapy, and modalities such as ultrasound or electrical stimulation, gait training, posture correction, and pain management strategies
- Communicate and counsel patients and families on lifestyle modifications and injury prevention while coordinating care with physicians, occupational therapists, and other healthcare professionals

Required Qualifications:
- Graduate of an accredited College or School of Physical Therapy
- Bachelor's degree
- Current Ohio Physical Therapist license in good standing
- Driver's License and access to reliable transportation
Preferred Qualifications:
- Masters' degree
- 1+ years of work experience especially with women's health, Parkinson's, and vestibular rehab
- Experience in a hospital setting

The hourly pay for this role will range from $34.23 to $61.15 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
