The Hospice Registered Nurse is responsible for pain management, symptom control in the delivery of care to hospice patients, as established by the plan of care, the interdisciplinary team, attending physician and Hospice Medical Director.

Essential Job Functions:
- Ensures the timely and adequate delivery of hospice services to the terminally ill patient and their family, operating within the plan of care as established by the hospice team and attending physician.
• Assesses the total needs of the patient/family during regularly scheduled and after hour home visits; Documents: assessment, identified problems, nursing interventions, goals, and outcomes of interventions.
• Coordinates total patient/family hospice care under the supervision of the Director and with the interdisciplinary team, the attending physician, and other providers; documents such coordination.
• Collaborates with the interdisciplinary team in the development, review and revision of the clinical component of patient/family plan of care, including exchange of information, review of problems, assessing effectiveness of interventions and documenting outcomes.
• Ensures continuity of care between patient/family, team members, ancillary providers, long term and inpatient care facilities, and the attending physician.
• Communicates patient/family/caregiver needs, ongoing nursing assessment, interventions, goals and outcomes through the interdisciplinary process.
• Provides and documents continuing education of the hospice concept of care to patients and their families/caregivers.
• Supervises the delivery of patient care provided by Hospice Aides, Licensed Practical Nurses; ensuring compliance with the established plan of care and completing required documentation of supervision.
• Facilitates the delivery of hospice services to patients residing in long term care facilities, documenting joint coordination of care with facility professional staff, and attending facility care plan meetings as appropriate.
• Participates in on-call rotation for delivery of care after office hours, on weekends, and holidays.
• Provides ongoing monitoring of patient appropriateness for hospice services and completes required documentation for certification and recertification.

Palliative Nurse Practitioner Responsibilities:
- Participate in the development and updating of plans for patient care that are medically appropriate, meet LHPC standards and guidelines, and are consistent with accepted hospice and palliative care standards.
- Provide clinical care in collaboration with the IDG, including, but not limited to;Clinical Assessment/Prescribing and adjustment of cost-effective medications (as per license and DEA registration) including consulting the PA Prescription Drug Monitoring Program profile as required.
- Appropriate ordering of DME
- Appropriate ordering and interpretation of radiological and laboratory tests
- Participation in rotating on-call availability
- Education of patients and families as needed in collaboration with the IDG
- Complete accurate and timely electronic documentation in accord with organizational policies to support sound hospice practices and facilitate appropriate reimbursement of services.

- Potential signs and symptoms of disease progression

Palliative Nurse Practitioner Minimum Qualifications:
- NP with graduation from an accredited training program and current licensure to practice in Pennsylvania.
- State drivers’ license in good standing.
- Automobile in good working order and insured in accordance with the applicable state laws (proof of insurance to be provided annually).
- Minimum two years-experience; experience in the area of geriatrics, home care, or hospice preferred.

Provides primary case management for an assigned group of clients including ongoing assessment, direct clinical treatment, rehabilitation and support services, and medication delivery.
Provides case management for all program participants as needed and directed by supervisory staff.
Delivers medications daily, twice per week, and weekly to identified clients according to established protocol.
Administers injections to clients as directed by the PACT Prescriber and PACT Lead RN.
Educates all clients as needed regarding medications, symptoms, coping strategies, personal growth and development, etc.
Provides side-by-side support as needed to promote client independence.
Acts as a liaison between clients and community agencies, resources, families, and natural supports to facilitate treatment.

Adheres to defined productivity standards regarding client contact.
Clients on assigned caseload shall be met with a minimum of twice per week, unless this is deemed clinically inappropriate by supervisory staff.
Achieves a minimum of 150 contacts per month.
Failed attempts to engage clients for contact shall be documented.
Compensation: Starting salary for this position is approximately $32.76/hr based on relevant experience and education.
Schedule: Schedule is variable and includes a mix of 8am
- 5pm and 11am
- 8pm shifts.
Shifts include a rotation of both weekends and holidays.
Staff provide on call coverage that may include overnight contact with clients.
This position includes some flexibility to allow for coverage during staffing shortages.
Travel : Mostly limited to Knox County with the rare potential for travel to surrounding counties.
This position does require the transportation of clients in a personal vehicle.
Equipment/Technology: This position requires the use of basic technology including a cell phone and computer.
Equipment/Technical Competency : Must possess basic computer skills along with the ability to learn how to successfully navigate the electronic medical record.

- PACT RN Case Manager Experience / Knowledge: At least one year of experience working with the SPMI population preferred.

Education / License : Must have either a Bachelor's or Associate's degree in nursing.
Must have licensure as a registered nurse in the state of Tennessee.

Job Description:
I. Position Summary:The Hebrew SeniorLife Hospice is designed to provide a Jewish Hospice service to terminally ill patients and their families in the greater Boston area. The philosophy of the Hospice is rooted in Hessed (loving kindness), and Kevod HaBeriyot (honoring life), but Hebrew SeniorLife Hospice Care is open to anyone regardless of religious affiliation; our mission is informed by Jewish values that are shared across many faith traditions and spiritual belief systems.
Hebrew SeniorLife Hospice works collaboratively with patients and families in the final months, weeks, and days of life, with the goal of achieving optimum healing of body and soul when curing is no longer an option. This means helping the patient and family with: physical comfort, emotional and spiritual support, and companionship. The Hospice goal is to help every patient at the end attain a peaceful and pain free death and support the healing and bereavement of the family.
As a member of the interdisciplinary group (IDG), the Hospice Registered Nurse provides professional nursing care to the Hospice's patients. Identify patient/family needs and provide supportive care in accordance with the attending physician's orders, the IDG plan of care and the Hospice's policies and procedures. The Hospice Registered Nurse provides care under the direction of the attending physician, the Hospice Medical Director, the Clinical Director, and in compliance with the state's Registered Nurse Practice Act.
II. Position Responsibilities:
- Assumes responsibility for a patient visit or on-call schedule which includes the assessing, planning, implementing and evaluating phases of the nursing process.
- Provides Hospice nursing care based on systematic assessment focusing on pain control and symptom management.
- Initiates communication with attending physicians, other Hospice staff members and other agencies as needed to coordinate care and use of resources for the patient/family.
- Assesses the patient and develops an individualized and comprehensive plan of care in partnership with the Interdisciplinary group and in accordance with Hospice regulations/guidelines. Routinely assesses patients response to plans of care and performs comprehensive assessments, hospice aide supervision, and coordination/oversight of hospice services provided in assisted living, LTC, SNF and hospital settings.
- Maintains regular communication with the Clinical Director to review assignments and with the attending physician/medical director concerning patient/family needs.
- Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporates that data into the plan of care.
- Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur in accordance with agency policy and Medicare regulations.
- Accurately documents assessment findings, observations, interventions and evaluations pertaining to patient care management and services provided in accordance with Hospice policy and procedures on the day services are rendered. Submits documentation in a timely manner.
- Teaches, supervises and counsels patients/families regarding physical care and comfort, and other problems related to the patient's terminal condition.
- Provides guidance to patients/families to assist them in preparing for and coping with anticipated physical and psychological events throughout the dying process. Provides appropriate support at time of death and period of bereavement.
- Meets regularly with Hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.
- Participates in IDG conference as needed and facilitates discussion of issues from caseload for full staff discussion, consultation and evaluation.
- Supervises the Hospice Aide and the aide plan of care at least every fourteen (14) days and documents such supervision. If concerns are noted, observes the aide onsite providing care. If concerns are ongoing, reports to the Clinical Director of the need to repeat a competency evaluation with aide.
- Informs the Clinical Director of unusual or potentially problematic patient/family issues.
- Shares in providing 24-hour, seven-day a week coverage (evening and weekend on-call) to patients/families.
- Demonstrates working clinical knowledge and skills relative to hospice and end-of-life care, and a commitment to clinical excellence and ongoing training in the latest pain and symptom management interventions.
III. Qualifications:
- Graduate from an accredited School of Nursing, BSN preferred.
- Currently licensed in good standing to practice as a Registered Nurse in the State of Massachusetts.
- Must have no less than six months experience in acute care setting as an RN prior to home care assignment, or experience as required by state regulations.
- Minimum of one year’s experience as an RN required.
- Experience in Hospice is strongly preferred.
- Demonstrated ability to assess and respond to the needs of patients and families in varied settings and to cope with emotional stress.
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with patients and their families.
