Providers will work onsite as part of a dynamic, collaborative Patient Aligned Care Team (PACT) dedicated to comprehensive, veteran-centered care.
Key Responsibilities:
- Deliver comprehensive primary care services including chronic disease management (diabetes, hypertension, COPD, heart failure), preventive care (immunizations, screenings, lifestyle counseling), and acute care for new/urgent symptoms.
- Lead care coordination as part of an interdisciplinary team (PACT model), collaborating with specialists, mental health providers, social workers, pharmacists, and community care partners.
- Integrate geriatric, palliative, and women’s health into patient care.
- Conduct telehealth visits and remote patient monitoring as required.
- Participate in patient hand-offs, consult/referral procedures, staff meetings, and QA/QI activities.
- Maintain thorough and timely clinical documentation, ensuring compliance with VA standards (EMR, privacy, infection control).
Required Qualifications:

The Child, Adolescent, and Young Adult Connections (CAYAC) program is currently seeking a Psychologist to join our team. The CAYAC Team is a unique model of community mental health which aims to support families through the process of screening, assessment, diagnosis, and treatment of mental disorders and/or substance use disorders in children, adolescents and young adults (up to age 18 / K-12).
