Role Overview

We are hiring a Medical Practice Floor Liaison to run patient flow across a high-volume, procedure-heavy multispecialty practice.

This role owns real-time coordination of MSK evaluations, procedures, and multi-visit treatment plans. You will connect providers, clinical staff, and scheduling to ensure patients move through care smoothly and efficiently.

What You Will Own

MSK and Procedural Flow

• Oversee daily MSK consults, follow-ups, and procedures
• Coordinate injections, interventional pain procedures, and advanced MSK treatments
• Align chiropractic and acupuncture visits within larger care plans
• Ensure correct sequencing of services to reduce delays
• Manage room readiness, staffing alignment, and provider timing

Multispecialty Coordination

• Serve as the operational bridge between MSK, pain, primary care, chiropractic, and acupuncture
• Support patient handoffs and shared treatment plans
• Maintain continuity across multi-visit and multi-provider journeys

Patient Flow Leadership

• Monitor clinic flow in real time
• Identify bottlenecks before they impact care
• Adjust schedules and resources as conditions shift
• Protect provider throughput and patient experience

Patient Journey Oversight

• Help patients understand where they are in their care plan
• Support complex, procedure-driven treatment pathways
• Escalate concerns quickly to preserve trust

Technology and Systems

• Use AI tools such as ChatGPT and internal systems as daily co-pilots

Job Description:

As a Primary Care Physician at a growing Senior Primary Care Outpatient Clinic, you will handle a variety of health-related problems in an outpatient setting, focusing on continuity of care, health maintenance, and disease prevention.

Key Responsibilities:

• Working closely with interdisciplinary healthcare teams, including nurses, specialists, and care coordinators, to ensure coordinated care and effective communication regarding patient needs and treatment plans.
• Contribute to ongoing projects and objectives within the clinic.
• Participate in Senior Tuberculosis (TB) screening program.
• Documentation and Compliance: Maintaining accurate and up-to-date electronic health records (EHR) or medical documentation in compliance with regulatory standards and company protocols
• Refer patients to specialists when necessary

Required Qualifications:

• 5+ years of technical experience in a medical setting.
• Valid NC medical license.
• Active DEA License
• Graduation from an accredited MD or DO program.
• Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine (Board Certification preferred but not required).

Preferred Qualifications:

• Specialty in Internal Medicine.
• Demonstrated passion for improving patient care and consumer experiences.

SUMMARY:

1. Acts as the Medicare Dual Eligible Special Needs Plan (D-SNP) clinical subject matter expert

2. Provides specialized clinical D-SNP leadership within a variety of Health Services functional areas 3. Develops and improves relationships with internal and external stakeholders, including the professional medical community, and maintains and enhances communications with similar Health Plan organizations

RESPONSIBILITIES:

• Acting as the clinical D-SNP subject matter expert across the organization

• Collaborating with the Chief Medical Officer, Medical Directors, and department directors to ensure compliance with Centers for Medicare & Medicaid Services (CMS) contractual and regulatory requirements

• Participating in the development of the Medicare D-SNP adjudication process and adjudicating D-SNP benefit appeals and denials

• Coordinating and consulting with department directors regarding the clinical direction of programs, studies, activities, and processes

• Participating in the development, interpretation, and presentation of reports and analytics needed for improvement

• Developing annual work plans, program descriptions, year-end reports, annual evaluations, and reviewing and updating policies, in conjunction with department directors and leadership

• Acting as the Alliance’s physician liaison for the D-SNP provider network

• Leading or participating in the Alliance’s internal committees and workgroups

• Educating internal stakeholders about clinical aspects of the D-SNP program

EDUCATION AND EXPERIENCE:

• Doctor of Medicine, current license to practice medicine issued by the State of California, Board certification in a specialty recognized by the American Board of Medical Specialties

• Minimum of three years of experience as an attending physician with the Medicare population or three years of experience as an attending physician
