JOB OVERVIEW

The Resident Care Director (RCD) serves as the nursing clinical leader for the community and is responsible to lead and manage the health and wellness along with coordination of care and services to residents within the community. Responsible for oversight and implementation of all Care programs, regulatory compliance, recruiting, and performance management.

RESPONSIBILITIES & QUALIFICATIONS

Managing Health and Wellness
  • Provide strategic leadership for resident care in the community by managing, directing, and monitoring the nursing team as applicable by State/Provincial law, to promote the health and wellness of the resident population.
  • Provide clinical care through the direct application of the nursing process; perform and document resident assessments and progress notes, evaluate changes in care needs, complete Individualized Service Plans (ISP), provide or delegate hands-on clinical care as indicated by the plan of care, and evaluate resident outcomes.
  • Collaborate with physicians, pharmacists, and other clinical providers to coordinate care and services for the residents.
  • Serve as the CLIA Director as applicable for the community and according to the federal and provincial requirements.
  • Partner with Neighborhood Coordinators to promote an integrated and collaborative approach to wellness operations and resident care.
  • Collaborate with Sales partners to determine community capability and assess potential residents' appropriateness for move-in.
Medication Management
  • Provide strategic leadership for resident care in the community by managing, directing, and monitoring the medication care managers, as applicable by State/Provincial law, to promote the health and wellness of the resident population.
  • Provide oversight of the community medication management program to promote resident safety in the medication use process including onboarding, training, and performance reviews.
  • Provide clinical training and education, as needed, to nurses, medication care managers, care managers, and others who provide resident care. Perform skill evaluation to assure clinical capability of care team members.
Quality Assurance and Regulatory Compliance
  • Track, trend, and report clinical quality data to identify risk.
  • Participate actively as a member of the community Quality Assurance and Performance Improvement committee.
  • Lead clinical quality and process improvement initiatives within the community to mitigate risk and improve resident care outcomes.
  • Recruit, hire, and train clinical team members and is responsible for performance management, evaluations, and engagement.
  • Complete direct report team member staffing and scheduling according to operational and budgetary guidelines.
  • Partner with the community leadership team to promote resident safety and compliance with Risk Management national/provincial regulations pertaining to occupational health and safety requirements.
  • Serve as the ICC and CLIA Director as applicable for the community.
  • Responsible for infection control programming, including delegation of infection control preventionist, as per provincial requirements.
  • Assure compliance with all Federal, State/Provincial, board of nursing, and other applicable regulations.
Financial Management
  • Manage the department budget to include labor/labour and other expenses and understand its impact on the community's bottom line.
  • Processes and submit monthly expenses and budget data in a timely manner, per Sunrise policies and internal business controls.
  • Understand the internal costs associated with all Sunrise resident care programs.
Training, Leadership and Team Member Development
  • Partner with the Regional Director of Resident Care and/or Executive Director in the delivery and participation in Sunrise University training and self-study programs.
  • On-board new RCD leaders and other department coordinators as needed.
  • Develop a working knowledge of state/provincial regulations and ensure compliance through supervising and coaching team members.
  • Complete clinical team member staffing and scheduling according to operational and budgetary guidelines when assigned to a community.
  • Hold clinical team accountable, correct actions when necessary, and document.
  • Attend regular meetings; Stand Up, Cross Over, Department Head, Town Hall, QAPI, and others as directed by the Executive Director.
  • Keep abreast of professional developments in the field by reading and attending conferences and training sessions.
  • Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
  • Perform other duties as assigned.

Experience and Qualifications
  • Graduate of an accredited college or school of nursing
  • Current state/provincial license as a professional Registered Nurse (RN)
  • Minimum two (2) years RN experience with at least one (1) year experience in home health, assisted living, or long-term care environment preferred
  • Minimum of one (1) year nursing management experience, including hiring, coaching, performance management, scheduling, and daily operations supervision
  • Certified in CPR and First Aid
  • Demonstrated knowledge of applied nursing practices, techniques, and methods in accordance with federal, state/provincial and board of nursing requirements.
  • Knowledge of infection control practices and prevention of disease transmission
  • Experience in tracking, trending, and analysis of clinical performance data preferred
  • Experience in quality and clinical process improvement and risk assessment preferred
  • Experience in staff development, training, and/or clinical education preferred
  • Computer proficiency with electronic medical records, the Microsoft Office suite, and the ability to learn new applications
  • Ability to work weekends, evenings and flexible hours as needed for resident care/services, including 24/7 on call responsibility

JOB SUMMARY

- Assist the Administrator with the process of admissions to community to include interviewing, assessing, verifying income, and completion of all pertinent paperwork. Must set up all new residents in Point Click Care initially and on-going as care plans and needs of residents change. Must complete all daily, weekly, monthly, quarterly, and annual assessments as required and set up in Point Click Care.
- Assess, develop, and implement a care plan for each resident initially and on an ongoing basis as determined by the needs of the resident.
- Report to and obtain orders from physicians regarding each resident. May delegate this to Med Techs/LPN as appropriate and with supervision. Must ensure that all orders are being followed and accurate documentation and entry is performed in Point Click Care.
- Must be knowledgeable of all Liberty Senior Living Policies and Procedures as well as State Rules and Regulations for Assisted Living Communities.
- Obtain medication for each resident from Long-Term pharmacy. May delegate this to Med Techs/LPN as appropriate and with supervision
- Chart on each resident and/or delegation to medication technicians with supervision according to Facility Policies and Procedures.
- Orient, teach, and train staff on all resident care policies and procedures including Point Click Care.
- Coordinate resident services with other professionals as ordered by the physician or as assessment and care plan dictate.
- Implement an infection control program, train staff, ensure compliance and conduct annual reviews.
- Order and maintain medical supplies as necessary with proper accounting program for charging residents for personal items.
- Weekly and monthly assessments of resident as well as taking vital signs, and administering all IM injections. May delegate this to Med Techs/LPN as appropriate with supervision.
- Coordinate with families and update families on a weekly and/or monthly basis on the status of resident as well as contacting immediately in the event of fall, sickness, etc.
- Maintain and supervise medication administration, medication ordering, medication count of controlled substances, and charting for each resident.
- Supervise and maintain records for Supervisors, Resident Aides, and Housekeeping, etc. employees, including TB screening, and Hepatitis B vaccine for all employees.
- Develop, implement, and train resident care staff on emergency procedures to be followed regarding fire, disaster, and resident incident and care issues.
- Conduct or arrange for pertinent training for Resident Rights, Infection Control, OSHA Safety, Back Safety, Needle Stick Safety, Dementia, etc.
- Perform scheduling for all staff using Workforce Management and assume and arrange for on-call responsibilities and respond to calls of an emergency nature from resident and/or supervisor on duty as needed even if reporting to work is necessary to assess the situation.
- Follow all policies and procedures, model relationship and leadership abilities, and work cooperatively and supportively with the residents, family members and friends, professionals,

Chaplain Resident work the supervision of a CPE supervisor, associate supervisor or certified candidate, the chaplain resident participates in the Clinical Pastoral Education (CPE) program in accordance with ACPE Standards and the policies and guidelines of the Department of Pastoral Care and Education. S/he is responsible for providing spiritual care, support and education for patients, their loved ones, and staff. S/he has the responsibility to ensure that religious and spiritual needs of the patients and their loved ones are met. This may be in the assigned units, while chaplain on duty, or whenever encountering persons throughout the medical center. S/he is capable of program planning and implementation. S/he identifies opportunities and makes recommendations for continuous improvement of processes and services on an ongoing basis. As a Chaplain Resident with us:

  • Communicating effectively with staff, shareholders, the Board and Management

Qualifications

  • BA/BS degree or similar certifications in a related field with strong academic credentials is preferred
  • At least 5 years of experience in a similar position
  • Demonstrated leadership and management skills
  • Ability to work well with all levels of staff, residents, the Board and the Managing Agent.
  • Experience in budgeting, invoice approval and expense management
  • Computer literate with an orientation towards applying relevant technologies to the building
  • Strong written and verbal communication skills
  • Ability to work flexible hours, including occasional weekend coverage
  • Knowledge of relevant Federal, State and Local Safety, Security, and other law/regulations


Additional information

The RM will report to the DEPM Managing Agent, is employed on a salary exempt status and is accountable to the Board of Directors of the Cooperative. The RM will be expected to provide reports at monthly Board meetings and periodic reports to Board committees verbally or in writing. The RM should be thoroughly familiar with the House Rules, Union Collective Bargaining Agreement and policies of the Cooperative.


Compensation

The RM position provides an attractive package of compensation based on experience and education including salary and a performance-based bonus, as well as a full suite of benefits and a two bedroom apartment in the building.
