The Director of Quality & Regulatory Affairs (DQRA) is responsible for assisting the Chief Quality Officer (CQO) with the execution of the SFDN Quality Management System including continual quality improvement throughout the organization. ESSENTIAL FUNCTIONS

β’ Directs, manages, and provides oversight for all activities related to the Quality Management System (QMS) and performance improvement.
o Develops and maintains a quality plan to ensure the best service possible. Oversees the implementation of the SFDNβs QMS. At a minimum, activities will include and are not limited to Licensing, Performance Improvement, Customer Complaint, Planned Deviation, Corrective Action/Preventive Action, Auditing, Document Control, Chart QA, Validation, and Death Record Review Systems.
o Promotes quality assurance with the expectation of accurate donor medical records and data elements, and validation of all performance data.

o Responsible for the administration of the electronic quality management system platform.

Job Summary:
Under the direction and supervision of the Center Manager, the Personal Care Attendant (PCA) provides home care services to Jordan Valley Health Senior Care participants. The Personal Care Attendant (PCA) will also participate in other activities and tasks as assigned to assist the home care, therapy, and adult day health staff.
Key Responsibilities:
- Provides personal care and escort assistance in accordance with individual treatment plans β including peri-care, grooming, dressing, showering, bathing, toileting, feeding, ambulation.
- Reports observed changes in participantsβ conditions to Center Manager and/or other center-based clinical staff.
- Proactively communicates participant health and safety concerns to Center Manager and/or other program staff.
- Demonstrates safe practices utilizing proper body mechanics and complying with universal precautions.
- Interacts in friendly, courteous, and professional manner with participants and their families/caregivers.
- Participates in social and recreation activities with participants as appropriate.
- Implements interventions, as appropriate to training, as directed by rehabilitation staff, including use of mechanical assistance for participant transfers and mobility.
- Occasionally accompanies participants to outside clinical appointments, when necessary.
- Assist with food service delivery and monitoring during mealtimes.
