Are you passionate about helping people, great at multitasking, and energized by meaningful work? Utah Community Action is looking for a Heat Intake Assistant to serve as the primary point of contact for our utility assistance programs-and a vital advocate for our clients.
This is more than a front-desk role. It's an opportunity to support individuals and families through challenging moments with compassion, professionalism, and purpose.
A Day in the Life:
- Answering incoming calls, voicemails, and emails related to utility assistance
- Welcoming and assisting walk-in clients with care and respect
- Educating clients about Utah Community Action utility assistance programs and eligibility
- Conducting initial screenings and processing applications
- Gathering documentation and scheduling appointments
- Referring clients to additional community resources to meet their needs
- Participating in outreach efforts (occasionally outside regular business hours)
All work is done using a trauma-informed, client-centered approach, because people come first, always.

What You Will Do:
The Chronic and Complex management program (CCM) Proactively identify patients who have multiple or complex medical and/or psychosocial needs or who are at risk of developing complex needs during an acute episode of illness. Provide early intervention and prevent recurrent crisis or unnecessary hospitalizations. Support and reinforce provider and patient interactions with recommended treatments and therapies. Facilitate communication among the patient, their families and caregivers, healthcare providers, the community, and the health plan.
- Support rising risk patient population and as needed support the MSW or RN with the higher risk patients
- Engages and enrolls patient in CCM
- Manages tasks in portal for patients that require outreach and support with coordination of care
- Outreaches Chronic Care Management patients as assigned, minimum 20 minutes/month
- Refers clinical issues to RN
- Support with connecting patients to resources

What You Will Need:
- Education: Preferred current certification from a national Medical Assistant certification body or obtain within 3 months of hire or transfer.
- Certification: BLS/CPR Certification for the Healthcare Provider American Heart Association (AHA) required

We are searching for the next Patient Access Specialist champion. This role is responsible for performing admitting duties for all patients admitted for services at the hospital. They are responsible for performing these functions while meeting the mission and goals of the organization and all regulatory compliance requirements. The Representative will work within the policies and processes as they are being performed across the entire organization.
Job responsibilities:
- Patient Access staff are responsible for assigning accurate MRNs, completing medical necessity / compliance checks, providing proper patient instructions, collecting insurance information, receiving, and processing physician orders, and utilizing an overlay tool while providing excellent customer service as measured by Press Ganey.
- Operates the telephone switchboard to relay incoming, outgoing, and inter-office calls as applicable.
- They are to adhere to policies and provide excellent customer service in these interactions with the appropriate level of compassion.
- Patient Access staff will be held accountable for point of service goals as assigned.
- Patient Access staff are responsible for the utilization of quality auditing and reporting systems to ensure accounts are corrected. These activities may include accounts for other employees, departments, and facilities. Conducts audits of accounts and assures that all forms are completed accurate, timely to meet audit standards and provides statistical data to Patient Access leadership.
- Patient Access Staff are responsible for the pre-registration of patient accounts prior to patient visits. This may include inbound and outbound calling to obtain demographic, insurance, and other patient information including the patient financial liabilities including collecting point of service collections as well as past due balances including payment plan options.
- The Patient Access Staff explains general consent for treatment forms to the patient/guarantor/legal guardian, obtains necessary signatures and witness's name. Explains and distributes patient education documents, such as Important Message from Medicare, Important Message from Tricare, Observation Forms, MOON form, Consent forms, and all forms implemented for future services.
- Reviews eligibility responses in insurance verification system and appropriately selects the applicable insurance plan code, enters benefit data into system to support POS (Point of Service Collections) and billing processes to assist with a clean claim rate.
- Responsible for accurately screening of medical necessity using the Advanced Beneficiary Notice (ABN) software to inform Medicare patients of possible non-payment of test by Medicare and distribution of the ABN as appropriate. Responsible for distribution and documentation of other designated forms and pamphlets.

Job SummaryThe Janitorial Manager is responsible for the supervision, inspection, and control of all custodial and related services associated within assigned contract. Properly allocate work responsibilities among subordinates. Job duties/responsibilities:
- Performs in depth, systematic assessment of all assigned patients.
- Formulates collaborative and individualized plan of care and identifies expected patient outcomes, using the nursing process and prioritizes patient care consistently utilizing available resources cooperatively with ancillary, nursing and other patient-team personnel to maintain standards for professional nursing practice.
- Demonstrates ability to function successfully in scrub role as identified in CST job description and as it relates to scrubbing, gowning, gloving, preparing instruments, & suture, implementing surgical asepsis, safety precautions, counts, and cost-effective safe care.
- Functions as a professional role model and resource person providing guidance to co-workers. Demonstrates/models the Network's Service Excellence Standards of Performance in interactions with all customers (internal and external).
- Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information.
- Complies with Network and departmental policies regarding attendance and dress code.
Training and experience:
Minimum of one-year operating room nursing experience required. Individualized orientation program for all areas. Maintain current BLS certification. Successful completion of Basic Cardiac Life Support Court. AORN Certification preferred (CNOR).

Education:
Registered Nurse with current license to practice in the State of Pennsylvania or New Jersey (Warren Campus).
Physical and sensory requirements:

The Section of Abdominal Imaging includes 19 ABR-certified Abdominal Imagers. Our team works closely with medical and surgical specialties across the organization. Our clinical service lines encompass all aspects of GI and GU imaging, from fluoroscopic exams to advanced body MRI exams. Our department is equipped with state-of-the-art US, CT, and MRI technology, including Photon counting and Dual-energy CT scanners, a large fleet of 1.5 and 3T magnets and a PET/MRI.
