Additional Information

Base salary range for this role is 128,700 - 196,000 USD annually. This role may also be eligible for annual bonus OR commission, as well as benefits and other incentives.

We are seeking an Administrative Specialist (Client File Management) to join our team.



Hourly Range: $16.50 - $18.50



This role is responsible for uploading documents, managing/auditing and entering data into the electronic management systems associated with Cornerstones of Care and the Kansas Department for Children and Families (DCF). This role requires strict standards of confidentiality, excellent communication, and customer service skills to manage a wide range of sensitive records and documents from diverse sources and stakeholders. As a member of the KS Foster Care team, you will work with other team members and report to our Administrative Manager.



WHAT YOU WILL DO:




  • Completes data entry and uploading of designated items into Department of Children & Families (DCF) and Cornerstones of Care electronic client management systems (i.e., Care Match & Avatar).
  • Organizes and maintains youth files including client records, in accordance with agency/program standards, Department of Children & Families requirements, and other governing directives.
  • Duties could include maintaining court calendar & court documents, tracking the acquisition of various records, and ensuring client files correctly contain all necessary documentation.
  • Perform periodic client file audits according to both external and internal expectations laid out by Cornerstones of Care and the Department of Children and Families.


WHAT YOU WILL BRING:



Our ideal candidate will have 2 years of administrative, clerical, and/or quality assurance experience, in addition to familiarity with electronic medical record maintenance, and the following:




  • Bachelor's or Associate's degree in related field, preferred.
  • Familiarity with Foster Care Case Management, preferred


REQUIREMENTS:




  • High school diploma or equivalent with 2 years of relevant experience.
  • At least 21 years of age and pass background check, physical, and drug screening.

We have an exciting opportunity for a full-time, Social Worker to join Ramapo Ridge Behavioral Health Hospital. Ramapo Ridge is a 58-bed inpatient behavioral health hospital licensed by the State of New Jersey and accredited by The Joint Commission. The hospital consists of two divisions: a general adult psychiatric program and a geriatric psychiatric program, which specializes in the diagnosis and treatment of illnesses. The Social Worker provides services to psychiatric patients and their families including individual, group and family supportive and educational counseling, care coordination and post-acute care planning. The individual is competent to care for patients ages 18 and over including older adults and those with dementia. Demonstrates an understanding of, and embraces, the mission statement of the Christian Health Care Center.

The opportunity for professional growth working at The Haven at Ramapo Ridge, New Jersey’s first and only inpatient trauma- informed care program and which has received the Clinical Excellence in Behavioral Health award from the New Jersey Hospital Association (NJHA).

Clinical supervision provided on site to those seeking hours toward LCSW licensure.

Competencies:

  • Conducts individual, group and family psychotherapeutic counseling and may function as primary therapist in identified cases.
  • Completes psychosocial assessments including obtaining information for diagnosis and treatment as required by regulatory and accrediting agency requirements.
  • Serves as liaison to patients and families and available community partners and resources.
  • Maintains accurate, timely documentation as a basis for review by regulatory bodies, accrediting agencies and the interdisciplinary treatment team.
  • Provides support, education and assistance to families and their support system regarding community resources, course of treatment and hospitalization, and discharge planning.
  • Coordinates and facilitates post-acute care plan for assigned patients.
  • Attends and actively participates in Interdisciplinary Treatment Team Meetings, prepared to offer psychosocial assessment information to guide treatment. Participate in the development of wellness and recovery plans.
  • Facilitates communication among the patient/family and treatment team and holds family meetings.
  • Provides coverage for department colleagues.
  • Provides coverage for Utilization Review team as needed.
  • Assists in developing favorable attitudes in the community toward recognition and treatment of mental illness and the aging process through participation in community activities concerned with mental health and/or aging.
  • Performs other related duties as required.

Schedule: Monday - Friday 8:00am - 4:00pm or 9:00am-5:00pm, weekend coverage if needed.

Education: Master's degree in Social Work required.

Licensure: Current valid New Jersey Licensed Social Work (LSW) required.

Responsibilities

The Clinical Social Worker is an integral member of the care team and contributes to the care coordination for patients with multiple social stressors and/or behavioral health concerns.  The Clinical Social Worker utilizes screening criteria for the coordination of quality health care services, reduction of service fragmentation, enhancement of quality of life, and the appropriate use of healthcare and community resources.

 

Essential Duties and Responsibilities

  • Manage a caseload of high-risk socially complex patients 
  • Conduct face to face and telephonic psychosocial assessments with the patient and/or the caregiver 
  • Investigate psychological and social determinant barriers and design appropriate interventions to assist in closing gaps in care and needs 
  • Provide short-term counseling services as needed
  • Coordinate services to ensure the patient/family understands a treatment plan
  • Work as part of a collaborative care team to achieve optimal quality and patient experience outcomes, including being an active member in High-Risk Huddles
  • Provide outreach to community-based services to support the patient’s ability to age at home
  • Develop and implement shared goals of care with the patient and family
  • Work with Community based agencies to develop relationships and provide appropriate resources to assist in maintaining patient independence

Qualifications

Required Knowledge, Skills and Experience 

  • Master’s degree in social work
  • Must be licensed as an LCSW or LISW (depending upon state)
