What you will be doing as Home Health Aides (HHA)

  • Participates in coordination of care with physicians and with other disciplines as appropriate.

4.   Manages work assignment in effective and efficient manner:

    • Visits all new therapeutic clients within 24 hours of referral unless otherwise specified
    • by the physician.
    • Reports any cases to the Supervisor of Clinical Services where goals can not be met
    • or at-risk situations exist and responds appropriately.
    • Remains flexible in daily planning, allowing time for emergencies or unexpected
    • visits.
    • Maintains productivity as established by the Agency.

5.   Establishes and evaluates the Plan of Care to be delegated to the Home Health Aide, orients and supervises the Home Health Aide in accordance with State and Federal Regulations and Agency policy:

    • Develops written plan of care for the Home Health Aide and updates this plan as patient’s condition warrants; plan of care must be reviewed every 60 days.
    • Obtains input from Home Health Aide regarding patient progress.
    • Prepares written performance evaluations of home care aides under his/her supervision.
    • Provides supervision of home care aides at a minimum of every 14 days or as required by payer source.

6.   Develops and maintains sufficient professional skills and credentials to meet the needs of patient care:

  • Attends a minimum of 12 hours of in-service per year to include mandated annual in-services.
  • Participates actively in self-appraisal and the formation of a plan of self-development.
  • Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
  • Seeks appropriate clinical input to ensure appropriate patient care.

Position Overview

Responsibilities of the Nursing Supervisor   

  • Internal case management, evaluation, education and development of nursing talent   
  • Participating in the transition of our patients from the facility to their home through “Start of Care Assessments” and “Re-evaluations”   
  • Review physicians’ orders and update the plans of care and documentation, as necessary   
  • Assist with selecting, training and validation of new clinical staff   
  • Promote and enhance the level of clinical expertise of staff to provide safe and quality nursing care for our patients   
  • Collaborating with the operations and business development team   
  • Ensure adherence to State, Federal, Local regulations and Aveanna policies and procedures   

Qualifications of the Nursing Supervisor:   

  • Must have good standing license (RN) in the state in which the clinician will practice   
  • Must have at least 3 years of hands-on nursing experience in a hospital or home care setting
  •  Must be able to travel to patients’ homes in designated territory   
  • Must have reliable transportation, valid driver’s license, and pass MVR check   
  • Current CPR certification from AHA or ARC 
  • Must be willing to supervise caregivers providing care to infants, children, adolescents and/or adults

Job Description

Qualifications

·                  Registered Nurse with Valid CT RN license

·                  One year of RN experience; Homecare or Hospice experience preferred.

·                  Valid driver’s license and reliable transportation required.

·                  Computer proficiency required.

- Provides in-service education, health care professional education, and public education in addition to supervising peers and supporting personnel and students in training as needed by the company.
- Conducts research and assists with orientation of new employees as needed.

Job Requirements:

- Minimum bachelor’s degree in physical therapy from an accredited school of physical therapy and current licensure from the state where therapy will be provided.
- Prefer a minimum of one year’s therapeutic experience in a non-home health care environment.
- Strong knowledge of theory and concepts relating to the practice of physical therapy and the ability to evaluate and treat patients for whom a physician’s order exists.
- Minimum of twelve hours of continuing education annually.
- Accept travel and working conditions as assigned.

Position Overview:

- Coordinate the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers.
