The Greater Manchester Region has approximately 2,500 employees. It includes Manchester Memorial Hospital, a 249-bed community hospital, Rockville General, a campus of Manchester Memorial Hospital, a 102-bed facility, a large multispecialty provider group and visiting nurse services. The Greater Manchester Region serves a region of 300,000 people in 19 towns.General Summary of Duties: The provision of intermittent nursing service to patients in their homes under a plan of care established in collaboration with the patientβs physician and according to agency policies and procedures.
Supervision Received: Reports directly to a Clinical Manager.
Supervision Exercised: Home Care Aides.
Education: Graduate of an NLN-approved school of nursing, associate or baccalaureate degree program.
Experience: One (1) year of recent medical/surgical nursing experience, additional experience in community health nursing preferred.
Certificate/License: Possession of a current license to practice as a Registered Nurse in the State of Connecticut.Β

Essential Duties: (This list may not include all the duties assigned.)
1.Β Admits/readmits patients and provides intermittent nursing service to patients and their families in compliance with agency policies and State and Federal Regulations, incorporating the following:
- A complete physical and psychosocial assessment.
- Develops and implements an appropriate care plan and therapeutic goals with the patient/family.
- Evaluation of patient safety on a 24-hour basis.
- Exercises appropriate clinical judgment in coordination of patient care by all services involved.
- Ongoing evaluation of significant changes in the condition of the patient and of progress toward the therapeutic goals.
- Use of Agency and community consultants/resources.
- Β Instruction of patient/family in procedures or responsibilities they are assuming and optimum wellness practices.
- Planning for the patientβs discharge from service.
2.Β Β Establishes and maintains case mix weight, utilization, patient satisfaction that aligns with industry standards and agency operational objectives for quality care.
- OASIS documentation correlates with the Plan of Care.
- Uses recommended Oasis assessment techniques 100% of the time.
- OASIS, documentation, and plan of care correlate to 95%.
3.Β Β Documents all services provided in the clinical record in accordance with State and Federal Regulations and Agency policy:
- Documentation, including activities and mileage is exported within 36 hours of visit or prior to next scheduled visit.
- Obtains physician verbal/written orders and complies with all reporting requirements.
- Participates in coordination of care with physicians and with other disciplines as appropriate.
4.Β Β Manages work assignment in effective and efficient manner:

- Visits all new therapeutic clients within 24 hours of referral unless otherwise specified
- by the physician.
- Reports any cases to the Supervisor of Clinical Services where goals can not be met
- or at-risk situations exist and responds appropriately.
- Remains flexible in daily planning, allowing time for emergencies or unexpected
- visits.
- Maintains productivity as established by the Agency.
- Utilizes payer sources correctly, reflecting changing levels of care.
- Delegates patient care responsibilities to Home Care Nurse (LPN) as appropriate.
- Demonstrates an understanding of the principles of case management.
- Demonstrates ability to manage cost and quality
5.Β Β Establishes and evaluates the Plan of Care to be delegated to the Home Health Aide, orients and supervises the Home Health Aide in accordance with State and Federal Regulations and Agency policy:

- Develops written plan of care for the Home Health Aide and updates this plan as patientβs condition warrants; plan of care must be reviewed every 60 days.
- Obtains input from Home Health Aide regarding patient progress.
- Prepares written performance evaluations of home care aides under his/her supervision.
- Provides supervision of home care aides at a minimum of every 14 days or as required by payer source.
6.Β Β Develops and maintains sufficient professional skills and credentials to meet the needs of patient care:
- Attends a minimum of 12 hours of in-service per year to include mandated annual in-services.
- Participates actively in self-appraisal and the formation of a plan of self-development.
- Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
- Seeks appropriate clinical input to ensure appropriate patient care.
- Projects professional image through attitude and compliance with dress code.
- Capable of precepting new nurses and students.
- Functions as a team member with consideration to all staff.
- Provides Agency with current copy of professional license.
- Maintains CPR certification.
Ancillary Duties
1.Β Β Performs other duties as necessary to support Agency programs as assigned by the Supervisor of Clinical Services.

b)Β Β Follow-through of an appropriate care plan and therapeutic goals with the patient/family and RN Case Manager.
c)Β Β Referral to appropriate therapies with approval of the RN Case Manager.
d)Β Β Periodic review of progress toward the therapeutic goals in conference with the RN Case Manager.
e)Β Β Use of Agency and community consultants/resources.
f)Β Β Β Instruction of patient/family in procedures or responsibilities they are assuming.
2.Β Documents all services provided in the clinical record in accordance with state and federal regulations and Agency policy:
a)Β Β All visits are documented within 24 hours.
b)Β Β Documents all physicians' telephone orders, informing nurse/therapist of all changes to the plan of care before implementation and obtains their signature.
c)Β Β Complies with all reporting requirements.
3.Β Visit responsibilities:
a)Β Β Confers regularly with the RN Case Manager assigned.
b)Β Β Remains flexible in daily planning, allowing time for emergencies or unexpected visits.

Reporting to the Director of Clinical Services, this role ensures the accuracy, compliance, and quality of Outcome and Assessment Information Set (OASIS) assessments by reviewing for regulatory and policy adherence. The individual in this role provides clinical staff with guidance on documentation, utilization, and patient care decisions to support optimal clinical and operational outcomes. They review visit utilization for care appropriateness and collaborate with clinicians on necessary modifications, reporting any potential over or underutilization to leadership. The role requires timely completion of workflows in Homecare Homebase (HCHB) and flexibility to meet agency needs.
Collaboration with teams is key to identifying trends and developing action plans to address them. Participation in quality and performance improvement initiatives is expected, along with supporting the agencyβs compliance with data submission requirements for regulatory and payor standards.

Reviews OASIS assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability.
Guides staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost-effective corrections that support clinical and operational goals.
Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager. Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs.

Qualifications

Follows trends in specialty area of wounds for home care industry; makes recommendations of treatment options for incorporation into the program.
Participates in training, coaching of staff or onboarding to the Wound program

We are a home care agency working with senior care clients in Contra Costa, Alameda, Sonoma, and Marin counties and weβre looking for a dynamic Home Care Community Partner to join our sales team in maintaining, growing, and developing new community partner relationships for sales growth.
The Home Care Community Partner will work with existing community partners to generate referrals for Hillendale, as well as develop new partnerships with local healthcare and senior care communities as well as elderly care industry professionals, organizations and establishments (ex. Geriatric Care Managers, independent/assisted/skilled living facilities, hospitals, concierge doctors, etc).
What Youβll Be Doing:
- Responsible for business development within the senior care community including establishing partners and networks for direct client referrals and new service starts.
- Responsible for maintaining existing community partnerships and developing new partnerships and services within the region.
- Partner with Sales and Client Care leadership to align pricing plans to meet the demands of the market with a focus on gross margin.
- Review, update, and maintain current product and service offerings for clients and families, ensuring that all internal partners are appropriately trained on products and services offered.
- Responsible for working with community partners and clients to find the right care solutions and building relationships for business retention.
- Generate regular streams of new business by providing exemplary client service, regular referral marketing to the community, and leading and participating in community outreach events to spread brand awareness.
- Call on and develop new referral accounts in the local healthcare and senior care community with elder care industry professionals, organizations and establishments (ex. Geriatric Care Managers, independent/assisted/skilled living facilities, hospitals, concierge doctors, etc).
- Communicate with family members and Client Care team to refine and improve clientβs lives, ensuring a high client satisfaction based upon key performance metrics.
- Work with the Client Care team regarding new clients, renewal of current clients, and any new products or services added as part of the care plan.
- Represent Hillendale at community events, upholding company values and high standards of care.
- Maintain accurate records on all prospective and active clients and referral sources.
- Present sales and other metrics to senior leadership including dashboard reporting.

Requirements:
Experience, Skills, and Education:
- Bachelor's Degree in business management, marketing management or similar field.
- 4+ years prior experience in a business development, account management, sales, and/or marketing role within the home care industry.
- Innovative business development skills with proven track record of new client acquisition and retention of existing clients through community partnerships and new business prospects.
- Excellent communication skills and the ability to anticipate the needs of customers
- Strong sense of urgency and drive to grow the business
- Demonstrated strong analytical skills.
- Possess strong problem solving skills and the ability to make sound judgment calls.
- Strong organizational and time management skills.
- Experience working in customer service programs and databases (CRM)
- Advanced skills in Excel; solid experience working in Google Suite (Docs, Sheets, Slides, Gmail).
- Must have clear driving record.

As an Addus Registered Nurse, you will:
- Responsible for the supervision and oversight of all patient care services including the development of personnel qualifications and the assignment of personnel.
