Qualifications
EXPERIENCE: Working knowledge of nursing process and customer service principles. National certification in specialty preferred. Acute care hospital experience preferred. IV skill preferred. Secondary language skills preferred.
LICENSURE/CERTIFICATION: Medical Surgical units and PCU: Current BLS certification. Currently certified in ACLS or completion within 3 month of employment. PCU training required after 6 months of hire for new graduates; and 3 months of hire for experienced RN's, or successful completion of BKAT. NNR required for OB.
QUALIFICATIONS: Must possess current CT RN licensure or in the process of obtaining the same and be a graduate of an accredited school of nursing.

Reporting to the Director of Clinical Services, this role ensures the accuracy, compliance, and quality of Outcome and Assessment Information Set (OASIS) assessments by reviewing for regulatory and policy adherence. The individual in this role provides clinical staff with guidance on documentation, utilization, and patient care decisions to support optimal clinical and operational outcomes. They review visit utilization for care appropriateness and collaborate with clinicians on necessary modifications, reporting any potential over or underutilization to leadership. The role requires timely completion of workflows in Homecare Homebase (HCHB) and flexibility to meet agency needs.
Collaboration with teams is key to identifying trends and developing action plans to address them. Participation in quality and performance improvement initiatives is expected, along with supporting the agency’s compliance with data submission requirements for regulatory and payor standards.
JOB RESPONSIBILITIES
Reviews OASIS assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability.
Guides staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost-effective corrections that support clinical and operational goals.
Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager. Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs.
Collaborates to identify documentation trends, supports improvement planning, participates in quality initiatives, and helps ensure compliance and accurate data submissions across all settings.
Qualifications
Degree from an accredited educational program leading to licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP). Required
Bachelor's degree in nursing, master's degree in PT, OT, SLP preferred
2 years in certified home health clinical required. 3 years in verified home health preferred. OASIS Certificate and/or Quality/Compliance Certificate preferred. Homecare Homebase experience preferred.

JOB SUMMARY

The Ridge Recovery Center is a new state-of-the-art substance use disorder treatment facility in Windham, CT. The Ridge Recovery Center offers personalized paths to recovery in a serene and tranquil setting. The Registered Nurse will work with and assist other professional, clinical, and medical staff in providing day-to-day direct care to individuals with substance use disorders. Some of the duties and responsibilities for this position include: assessing client’s conditions, administering medications, managing withdrawal symptoms, providing direct nursing care and education to clients in regard to their health, wellness and recovery. 1. Admits/readmits patients and provides intermittent nursing service to patients and their families in compliance with agency policies and State and Federal Regulations, incorporating the following:
- A complete physical and psychosocial assessment.
- Develops and implements an appropriate care plan and therapeutic goals with the patient/family.
- Evaluation of patient safety on a 24-hour basis.
- Exercises appropriate clinical judgment in coordination of patient care by all services involved.
- Ongoing evaluation of significant changes in the condition of the patient and of progress toward the therapeutic goals.
- Use of Agency and community consultants/resources.
- Instruction of patient/family in procedures or responsibilities they are assuming and optimum wellness practices.
- Planning for the patient’s discharge from service.
2. Establishes and maintains case mix weight, utilization, patient satisfaction that aligns with industry standards and agency operational objectives for quality care.
- OASIS documentation correlates with the Plan of Care.
- Uses recommended Oasis assessment techniques 100% of the time.
- OASIS, documentation, and plan of care correlate to 95%.
3. Documents all services provided in the clinical record in accordance with State and Federal Regulations and Agency policy:
- Documentation, including activities and mileage is exported within 36 hours of visit or prior to next scheduled visit.
- Obtains physician verbal/written orders and complies with all reporting requirements.
- Participates in coordination of care with physicians and with other disciplines as appropriate.
4. Manages work assignment in effective and efficient manner:

- Visits all new therapeutic clients within 24 hours of referral unless otherwise specified
- by the physician.
- Reports any cases to the Supervisor of Clinical Services where goals can not be met
- or at-risk situations exist and responds appropriately.
- Remains flexible in daily planning, allowing time for emergencies or unexpected
- visits.
- Maintains productivity as established by the Agency.
- Utilizes payer sources correctly, reflecting changing levels of care.
- Delegates patient care responsibilities to Home Care Nurse (LPN) as appropriate.
- Demonstrates an understanding of the principles of case management.

5. Establishes and evaluates the Plan of Care to be delegated to the Home Health Aide, orients and supervises the Home Health Aide in accordance with State and Federal Regulations and Agency policy:

- Develops written plan of care for the Home Health Aide and updates this plan as patient’s condition warrants; plan of care must be reviewed every 60 days.
- Obtains input from Home Health Aide regarding patient progress.
- Prepares written performance evaluations of home care aides under his/her supervision.
- Provides supervision of home care aides at a minimum of every 14 days or as required by payer source.
6. Develops and maintains sufficient professional skills and credentials to meet the needs of patient care:
- Attends a minimum of 12 hours of in-service per year to include mandated annual in-services.
- Participates actively in self-appraisal and the formation of a plan of self-development.
- Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
- Seeks appropriate clinical input to ensure appropriate patient care.
- Projects professional image through attitude and compliance with dress code.
- Capable of precepting new nurses and students.
- Functions as a team member with consideration to all staff.
- Provides Agency with current copy of professional license.
- Maintains CPR certification.
Ancillary Duties

POSITION SUMMARY/PURPOSE
Practitioners are responsible for providing physical exams, psychiatric evaluations and addiction treatment including: diagnosis; prescribing; coordination of care, and patient education. Primary responsibilities will be in the inpatient withdrawal management and residential program, but may include Partial Hospital, Outpatient and community settings as determined by the Medical Director. The practitioners will work closely with the Residential Medical Director, staff psychiatrists, and clinicians in a patient centered multidisciplinary treatment team approach. The practitioner will hold licensure through the State of CT Department of Public Health and be credentialed through the medical staff of Rushford.
ESSENTIAL FUNCTIONS
1. Evaluate patients through detailed physical exam, a full review of psychosocial comprehensive assessments, and development of a detailed medical history.
2. Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with primary care physicians and medical specialists.
3. In collaboration with clinical and nursing staff, develops and implements a treatment plan that involves and includes the patient in active participation in treatment. And revises the plan as indicated.
4. Writes orders for medications, treatments and other matters pertaining to patients care.
5. Writes progress notes on patient’s charts indicating patient status and treatment recommended.
6. Provides education to individuals or groups as appropriate and requested by the Medical Director.
7. Assumes care for physical and psychological needs of patients during examinations and provides emotional support to family members when appropriate.
8. Charts observations, test results and all patient communication in the medical record as directed by Rushford policy and procedure.
9. Ensures accurate maintenance of patient’s medical records and that patient records are treated as confidential information.
10. Advises patients on use of various medication including dosage, side effects and composition.
11. Reviews and interprets results of diagnostic studies and tests, consults with supervisory physician when appropriate, incorporates information into plan of care.
12. Determines and discusses diagnosis with patient, documents information on charts including billing codes.
13. Recommends and develops new services and programs.
14. Participates in peer review, chart reviews, staff education, clinical guidelines development, and other continuing education and quality assurance activities to demonstrate compliance with standards.

Advanced Practitioner Requirements
· Graduate of an accredited Nurse Practitioner Master’s program. Licensed, or eligible to be licensed, as a Advanced Practice Registered Nurse (APRN) in the state of Connecticut

Qualifications - ExternalHIPAA and Privacy: Properly direct any complaints and/or grievances as indicated per VNHSC policy. Comply with HIPAA regulations, specifically the Privacy and Security Rules. Protect personal health information (PHI), electronic personal health information (EPHI) and financial information by limiting the use and disclosure of health information and financial information to the minimum amount necessary with only those who must know how to get the job done. Follow the department's specific policies and procedures to protect health information. Help one another to ensure confidentiality at all times.
General Summary of Duties: The Home Care Nurse (LPN) is responsible to provide nursing care consistent with plan of care established by the RN Case Manager.
Supervision Received: Reports directly to a Team Leader. Performs nursing care under the direction of the RN Case Manager
Supervision Exercised: None.
Qualifications
Typical Physical Demands: Requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. Requires standing, walking, and the ability to ascend and descend stairs. Occasionally lifts and carries items weighing up to 30 pounds. Requires corrected vision and hearing to normal range. Requires working under stressful conditions. Requires some exposure to communicable diseases and/or bodily fluids.
Typical Work Conditions: Work is performed in patients' homes, community settings, clinics and in an office environment. Some exposure to communicable diseases, toxic substances, and medicinal preparations.
Essential Duties: (This list may not include all of the duties assigned.)
The Home Care Nurse (LPN) functions under the direction of the RN Case Manager.
1. Provides intermittent skilled nursing service to patients and their families in compliance with state and Medicare regulations, incorporating the following:
a) Assesses the physical and psychosocial status of the patient.
b) Follow-through of an appropriate care plan and therapeutic goals with the patient/family and RN Case Manager.
c) Referral to appropriate therapies with approval of the RN Case Manager.
d) Periodic review of progress toward the therapeutic goals in conference with the RN Case Manager.
e) Use of Agency and community consultants/resources.
f) Instruction of patient/family in procedures or responsibilities they are assuming.
2. Documents all services provided in the clinical record in accordance with state and federal regulations and Agency policy:
a) All visits are documented within 24 hours.
b) Documents all physicians' telephone orders, informing nurse/therapist of all changes to the plan of care before implementation and obtains their signature.
c) Complies with all reporting requirements.
3. Visit responsibilities:
a) Confers regularly with the RN Case Manager assigned.
b) Remains flexible in daily planning, allowing time for emergencies or unexpected visits.
c) Reports any cases to the supervisor where goals can not be met or unsafe home situations exist.
d) Maintains required productivity as established by the Agency.
4. Develops and maintains sufficient professional skills to meet the needs of patient care:
a) Attends a minimum of 12 hours of in-service per year.
b) Participates actively in self-appraisal and the formation of a plan for self-development.
c) Knowledgeable of Agency policies and procedures.
d) Seeks appropriate supervision.
e) Projects professional image through attitude and compliance with dress code.
f) Functions as a team member with consideration to all staff.
g) Maintains CPR certification.
h) Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement.
Ancillary Duties
1. Performs other duties as necessary to support Agency programs as assigned by the clinical supervisor:
a) Agency committees as requested.
b) Flu/special clinics as needed.
c) Adult Consultation Hours as assigned.
d) Special Agency or community programs during and/or after regular working hours.
2. Performs other duties as necessary to maintain current Agency personnel policies:
a) Time worked is recorded regularly and accurately.
b) Personnel records are complete with but not limited to current professional license, auto and malpractice insurance.
c) Abides by Agency weekend/holiday rotation of nurses.
Knowledge, Skills, and Requirements: Knowledge of professional nursing theory and practice and the skills to provide competent patient care. Knowledge of Agency policies, regulations and procedures to administer patient care. Knowledge of medical equipment currently used to provide patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods and techniques of professional nursing to provide ongoing patient care. Skill in identifying patient-related problems and recommending solutions. Skill in documentation in clinical records. Skill in establishing and maintaining effective working relationships with patients, families, physicians, co-workers, and the community. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly. Possess adequate transportation to carry out job requirements. Must show proof of satisfactory automobile insurance with minimum coverage to be $100,000/300,000 determined by agency policy.
Education: Graduate of NLN approved school of licensed practical nursing.
Experience: One (1) year of recent medical/surgical nursing experience, additional experience in community health nursing preferred.
Certificate/License: Possession of a current license to practice as a licensed practical nurse in the State of Connecticut.
