• Assists the attending Transplant surgeons, house staff, and Transplant staff with the direct care of hospitalized patients and outpatients on a rotating basis with the other clinical care coordinators, including: facilitation of patient discharges and transplant teaching
• Attends once a week outpatient Transplant clinics: RN orders labs, performs injection teaching, coordination of care

Schedule appropriate non-invasive laboratory tests and participates in the interpretation of the results and the institution of appropriate therapy based off physician guidances

  Reviews discharge instructions to hospitalized patients
  Works with hospital case manager and social worker regarding patient discharge needs
• Answers telephone inquiries from patients and/or visiting nurses after hospital discharge

• Participates in teaching programs of the Abdominal Organ Transplant Program
• Additional responsibilities as needed.

Patient Care and Monitoring:

• Monitor Transplant recipients after surgery for complications or rejection and report to Transplant
  Physician
• Review lab results and biopsy reports
• Assess patient symptoms and clinical status
• Coordinate follow-up visits and routine post-transplant evaluations

Medication Management:

• Monitor drug levels
• Adjust medications based on provider orders
• Educate patient of medication changes and adherence

Provide Education and support to lifestyle changes, infection prevent, medication schedules, emotiona support, provide resources, and warning signs of complications inpatient and outpatient.

The Home Therapy Registered Nurse, as qualified by federal and state regulations, provides patient training and ongoing support for all patients choosing a home dialysis modality.  This position is accountable for providing quality care to all patients and is committed to delivering superior customer service in all internal and external interactions.  

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.  Other duties and tasks may be assigned.

• Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements as they pertain to home dialysis program and patient care requirements.
  
• Train patient (and/or care partner) in the practice of self-care Peritoneal Dialysis or Hemodialysis upon meeting federal and state regulations governing Registered Nurse qualifications.
  
• Conduct home visits to assess the patient's home environment per policy; initially and minimally annually thereafter.  Routinely evaluate patient performance and assess the home environment more frequently as needed to improve care.
  
• Communicate results of patient assessment, reassessment, and ongoing monitoring to the physician, team members, and others as appropriate to the individual needs of the patient.
  
• Document all nursing services in the Electronic Medical Record including but not limited to training sessions, routine and non-routine in-person interactions, and phone conversations.  Documentation should accurately reflect the patient status and nursing interventions and be written to ensure continuity of care.
  
• Participate in infection control monitoring, implementation, and recording as requested.
  
• Be familiar with emergency equipment and all emergency operational procedures.  Communicate and regularly review Emergency Preparedness procedures with all home patients, including but not limited to emergency disconnection from dialysis equipment, what to do and who to contact if displaced from home.
  
• Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions and place orders in timely manner to avoid fees and penalties.
  
• Continually seek to improve patient and quality outcomes through tracking and trending of program and patient performance, use of critical thinking skills and root cause analysis to improve patient and quality outcomes.  Assist in obtaining data for the continuous quality improvement activities.
  
• Obtain routine and non-routine laboratory tests as ordered and communicate critical lab values and urgent patient needs to the responsible physician or physician extender in a timely manner.
  
• Complete and document monthly review of patient medication profiles.  Administer medications as ordered by the physician.
  
• Responsible for performing on call nursing services, nights and weekends, on a rotational basis as needed or assigned.
  
• Flexible with staffing locations and hours to accommodate patient and USRC home program needs.
  
• Regular and reliable attendance is required for the position.
  

Home Hemodialysis Only:

• 
  
• Coordinate home evaluation with technical services department to ensure that the necessary electrical, plumbing and drainage requirements for proper equipment operation are met prior to patient acceptance into home program.
  
• Ensure required and appropriate water sampling is complete per policy and product requirements.  Monitor and report water sample and culture results.           
  

QUALIFICATIONS/REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Requirements include:

• Current RN license in applicable state.  License must be maintained as current and in good standing. 
  
• 12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality for which the nurse will provide home dialysis training, except in states with a more stringent requirement such as Texas, which requires at least 6 months experience in the specific home modality.
  
• Previous experience in providing nursing care to patients on maintenance dialysis preferred.
  
• CPR certification required prior to patient care assignment.
  
• Confirmation of ability to distinguish all primary colors.
  
• Must successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency applicable for the modality assigned.

SUMMARY

The Acute Registered Nurse provides acute dialysis care and treatment to patients within a hospital environment. This position performs duties as assigned by the Charge Nurse or Acute Manager, including assisting and supervising Patient Care Technicians and Licensed Vocational Nurses. Acute nurses may report directly to an Acute Manager, Administrator or the first line of regional operational management which may be an Area Director, Regional Director, or Vice President of Operations, depending on the region/demographics.

· Assist with clinical and operational processes to improve patient health and minimize hospitalizations and rehospitalization.

Preferred qualifications: Familiarity and knowledge of the CNYRO Long Term Care Program’s 81 skilled nursing home facilities, and familiarity working in federal electronic databases; i.e., ASPEN, including ACO and ACTS.

Responsibilities:

· Participates in onsite complaint investigations and surveillance activities in the field when requested.

· Performs onsite survey activities in accordance with federal policies and procedures and at the direction of the team leader.

· Assures that all information gathered during survey is based on thorough interviews, observations and clinical and other pertinent document reviews.

· Participates in interdisciplinary survey discussions, arriving at decisions through team consensus.

· Conducts investigations of resident and facility complaints at direction of the team leader and program managers.

· Provides direction in the appropriate interpretation and application of federal and state regulations at the discretion of the program director, program manager and team leader.

· Assists other surveyors in clarifying resident/patient medical issues and identifying standards of nursing and medical care to ensure facility compliance with state and federal regulations while working collaboratively with the Complaint Team Manager and survey team members to ensure that surveys are conducted in accordance with Long Term Care Program standards and CMS survey process.

· Assists as needed with onsite and offsite surveillance of NYS Adult Day Health Care Programs.

· Engages in daily activities to maintain quality assurance for the nursing home, adult day health and complaint team surveillance programs.

· Assists in pre-survey review of complaint investigations.

· Organizes abbreviated survey case files, opens and closes abbreviated survey files utilizing the ASPEN/Complaints/Incidents Tracking System, ASPEN and ASE-Q as well as Microsoft Word.

· Assists in maintaining Complaint Tracking Logs.

· Assists with maintain contact with surveyors in the field and assists surveyors by obtaining pertinent medical records and identifying, contacting, and interviewing pertinent witnesses.

· Summarizes investigations (abbreviated surveys) for reporting purposes in accordance with federal principles of documentation.

· Closes complaints with letters following supervisory review.

· Participates in other surveillance activities, including federal and state training programs.

· Responsible for responding to provider and complainant inquiry from the CNYRO designated complaint team phone line - returning calls to complainants via this line.

· Responds to requests from the Office of the Attorney General as necessary.

· Assists in maintaining file of complaints awaiting supervisory review and closure.
