· Congestive Heart Failure Nurse Practitioners autonomously manage a patient panel of recently discharged and chronic acute exacerbation patients in an outpatient ambulatory clinic setting

Job Description
- Provides office-based nursing care in collaboration with surgeon

Job Summary:
Provides direct patient care to behavioral health patients in accordance with the established policies and procedures of the facility.
Responsibilities:
- Performs initial and on-going bio-psychosocial assessments according to accepted standards of nursing practice. Develops a nursing plan of care consistent with the master treatment plan.
- Participates in treatment team conferences to assist in planning and revising goals, objectives and interventions appropriate to the age-related and problem-specific needs of each patient.

Our 66 bed Emergency Department has a Trauma Level II designation and is a primary Stroke and STEMI center. Job Description
Responsible for coordinating the individualized care of Structural Heart patients throughout the care continuum, eliminating barriers to healthcare and enhancing patient and physician satisfaction. Serves as a member of the multidisciplinary Heart team, functioning as a patient advocate, educator, key point of contact and liaison between the patient and all other appropriate services after discharge. Coordinates and works collaboratively to ensure efficient transition of care for transferred Structural Heart patients.
· Provides support to patients and caregivers through guidance, direction and liaison in both inpatient and outpatient settings.

This position is an essential member of the nursing clinical team.
The RN certified in wound, ostomy and continence provides expert clinical care, consultation/care coordination, education, quality improvement, leadership and research to the organization in relation to patients/ families and colleagues with wound, skin, ostomy and continence needs. This position is responsible for the assessment, planning, implementation, and evaluation and care of wounds (acute and chronic), fistulae, pressure ulcers, skin breakdown, ostomy care and incontinence.
Specifically, the WOCN RN Acute Performs the following:
· Coordinates care for populations of patients, identifies trends, develops evidence-based protocols/treatments and refines protocols based on emerging evidence, products, therapies and outcomes.
· Develops and implements and refines educational programs for patients, families, colleagues and community partners.
· Provides formal and informal consultation and serves as resource for patients, healthcare team and leadership.

Direct Patient Care
Provides expert consultation and/or hands-on care for individual patients with acute or chronic wounds, ostomy or continence issues.
• Incorporates specialized evidence-based knowledge and skills to provide excellence in assessment, therapeutic interventions, prevention and health maintenance for patients with select disorders of the integumentary, gastrointestinal and genitourinary systems.
• Differentiates characteristics of wound, ostomy, continence clinical presentations and attributes accurate etiology in development of treatment plans.
• Serves as a consultant to provide potential solutions for complex clinical cases to improve patient care and outcomes.
• Establishes and coordinates multidisciplinary and comprehensive management plans.
• Conducts ongoing re-evaluation and modifies the strategies and interventions as needed.
• Notifies supervising clinician and Team when the wound deteriorates or fails to progress.
Collaborates with healthcare team to provide interdisciplinary referrals to complement the treatment plan.
Care Coordination
Coordinates care for populations of patients with wounds, ostomy and continence conditions.
• Collaborates with Healthcare Teams and leadership to develop best practice cross-setting care coordination using standardized prevention and treatment protocols: Coordination of care involves consideration of care trajectory across multiple settings (pre-op, acute and post-acute).
Education
• Develops and implements educational programs and protocols for patients, staff and other health care providers that address the prevention and management of wound ostomy and continence health conditions.
• Arranges learning opportunities for RN Skin Champion.
• Provides orientation for nurses and other health care team members.
Quality Improvement / Safety
• Collects and interprets wound, ostomy and continence-related data for continuous quality improvement, safety initiatives, surveillance and regulatory compliance.
• Leads or provides guidance in the leadership of initiatives to reduce harm and improve outcomes for patients with wounds, ostomy and continence conditions.
• Participates/ leads research and quality improvement projects to investigate or validate interventions and workflow related to wound ostomy continence care.
Qualifications
Education
• Baccalaureate degree in Nursing. Master’s degree in nursing or higher preferred.
Experience
• Minimum of 2 Years with a minimum of 2000 hours of progressive wound and skill experience in clinical practice.
• 5 or more years of active clinical nursing experience preferred.
• Experience in EPIC EHR preferred.
Licensure, Certification, Registration
• Licensed Registered Nurse from the CT DPH.
• Enrollment in a WOCN Society Accredited Educational Program within one year of hire.
• Completion of board certification in wound/ ostomy/continence through WOCNCB within a defined timeline following enrollment in educational program.
• BLS certification.
• Completion of a WOCN Society accredited educational program in wound / ostomy/ continence preferred.
• Current board certified in wound/ ostomy / continence through WOCNCB preferred.

Job Summary:
Functioning within the framework for professional nursing practice, the Invasive Cardiology Manager, responsible for the assigned units Electrophysiology Labs, Cath Labs, and Cardiac Procedural Holding, supports and assists the Director of Clinical Operations, Heart and Vascular Institute (HVI).
This position has 24-hour accountability for managing resources (human, material and financial), to ensure quality, cost-effective delivery of patient services for specific patient populations, in a defined area across the continuum. This is accomplished by direct action, delegation, coaching and support of unit-based shared governance councils.
Reports to Director of Clinical Operations, Heart and Vascular Institute (HVI).
Responsibilities:
· Ensures compliance with regulatory and accreditation requirements.
· Conducts hiring, training, directing, development and evaluation of staff.
· Identifies and resolves issues affecting the delivery of patient care services for the assigned unit(s).

Program Description:

St. Vincent’s has implemented a comprehensive transition to practice program for novice operating room nurses. This program aligns with Hartford Healthcare’s values of excellence, safety, caring and integrity by offering a blended and multimodal approach to ensuring novice OR nurses master the necessary competencies to provide high quality and safe patient care.
Candidates are hired into the OR Transition to Practice Program with a start date of February 23rd, 2026.
The OR Transition to Practice Program is open to new graduate registered nurses and experienced nurses (non-OR experience). This 12-month long program is divided into two main phases:
Introduction to Perioperative Nursing: During the first 3 months of the program, learners can expect to be immersed in a multimodal learning experience through evidence-based learning modules (AORN Periop 101), classroom/group learning, simulation and clinical experiences to master the foundational perioperative nurse competencies. Students will learn the scrub nurse role during this time. At completion of phase one of the OR RN Transition to Practice Program, the learner will complete their Periop 101 final exam.
Clinical Orientation: The remaining 9 months are spent immersed in the clinical environment mastering the unit-based nursing competencies. This focus of this portion of the program is on the circulating nurse role in each of the surgical specialties including general, colorectal, gynecology, plastics, bariatrics, urology, surgical oncology, vascular, thoracic, robotics, neuro, spine, orthopedics, and podiatry. Additional components of this portion of the program include professional practice, professional development and professional governance.
To facilitate the ideal learning experience, the schedule during the orientation period will be Monday – Friday 6:30am-3:00pm. During the final weeks of the orientation period, the orientee will have the opportunity to be exposed to evening, night, and weekend shifts.
Throughout the entirety of the program, learners can expect to be mentored by their unit-based preceptors, management team and members of the Nursing Professional Development team (Unit Nurse Educators). You will meet regularly to ensure you are supported and set up for success in your new role.
New graduate nurses will also be enrolled in our Hartford Healthcare Graduate Nurse Residency program to facilitate their transition to professional nursing practice.
The Role of the OR Nurse:
Operating Room (OR) nurses, also known as perioperative nurses, play a crucial role in providing care before, during, and after surgery. They work closely with the surgical team to deliver safe, high-quality care and are responsible for ensuring patient safety throughout the perioperative environment. OR nurses may take on various roles in the surgical team, depending on their responsibilities.
Scrub Nurse: Scrub nurses work directly within the sterile field and assist the surgical team by preparing instruments, equipment, and sterile supplies, passing instruments to the surgeon during the procedure, maintaining a sterile environment, and monitoring instruments and supplies used during surgery to ensure nothing is misplaced.
Circulating Nurse: Circulating nurses manage the overall environment of the operating room by ensuring all surgical tools, supplies, and equipment are available and working, monitoring the patient’s safety and well-being throughout the surgery, maintaining documentation and records of the procedure, communicating between the surgical team and other departments (such as pathology or radiology) and anticipating the needs of the surgical team, and providing additional instruments, medications, or equipment as necessary.

OR General Surgery: The Operating Room (OR) General Surgery provides care to a diverse group of patients across multiple surgical specialties. These specialties include general, colorectal, gynecology, plastics, bariatrics, urology, surgical oncology, vascular, thoracic, robotics, neuro, spine, orthopedics, podiatry, and trauma. The Core OR team is also responsible for ensuring 24/7 operating room coverage, offering staff a range of alternative scheduling options. Additionally, there are numerous opportunities for team members to engage in innovation and continued learning with new equipment and procedures.
Qualifications
- Current Connecticut nursing license required prior to start date.
- Basic Life Support (BLS) certification required at time of hire.
- Exposure to perioperative nursing practice environment preferred (Summer Student Nurse Fellow, Capstone Rotation, employment in perioperative services in a non-licensed capacity).

The Nursing Professional Development (NPD) Practitioner is a Registered Nurse with NPD practice judgement and expertise who influences professional role competence and growth of learners. This position works in collaboration with nursing leadership to foster and influence competency management, clinical nursing education, professional role development, policy standardization, collaborative partnerships, scientific inquiry, transition to practice and onboarding/orientation of new Hartford HealthCare colleagues. The area of coverage for this position is Labor & Delivery/Maternal Newborn. Key areas of responsibility are:

Qualifications
Education:
