The Social Worker for the Program for All‐Inclusive Care for the Elderly (PACE) will be responsible for direct social work care management services, such as participant screening, case management, counseling and referrals of participants to PACE. Together, we are building a stronger, more equitable health system.
Your Contributions To the Team:
- 70% ‐ Care Management
- Participates in a mission driven culture of high‐quality performance, with a member focus on customer service, consistency, dignity and accountability.
- Participates in the Interdisciplinary Team's (IDT) initial, six (6) month and annual assessments, as well as care planning of the participant's case in accordance with industry and departmental standards.
- Documents participant changes appropriately in the medical record in accordance with industry and departmental standards.
- Communicates effectively and openly with other IDT members on participant status and needs.
- Assesses patients for high‐risk situations such as suicidal/homicidal ideation and abuse. Reports appropriately in accordance with legal and ethical standards of practice and engages in safety planning with participants and/or family.
- Develops and leads group counseling and participant support group activities.
- Refers participants and families to appropriate community agencies or facilities, serves as a liaison with such organizations and as an advocate for participants.
- Assists in the completion of advance directives, mortuary arrangements, funeral planning and/or facility placement.
- Aids patients/representatives in ascertaining third‐party and financial resources and planning for financial coverage for services as necessary.
- Addresses any psychosocial needs that may require social work support and intervention (e.g., anxiety, depression and other behaviors), both at the center and at home.
- Works with PACE participants and the IDT to develop an appropriate discharge plan for disenrolling from PACE services.
- May be required to perform home visits to private homes of CalOptima Health's PACE participants; frequent travel in Orange County.
- Support PACE participants with care coordination needs, such as transportation, family communication, scheduling visits to the center, supporting with warm handoff process to our community partners.
- Represents and supports the participant's care management needs by being an advocate for their care, supporting clinical needs such as supportive counseling, developing care plans to manage all psychosocial needs (i.e., establishing support systems, coping mechanisms, assessing depression, anxiety, cognition) and developing interventions.
- 25% ‐ Support Services
- Assists the team in carrying out department responsibilities and collaborates with others to support short and long‐term goals/priorities for the department.
- Consults with and advises staff members regarding the relationship of social, emotional and cultural factors to health and medical care and the availability of social services in the community.
- Attends and participates in in‐services, staff meetings and case conferences.
- Helps support the new enrollee process of enrollment into the PACE program (e.g., communicates to IDT and ensures all service gaps are covered prior to enrollment).
- Participates in program improvement for the Social Work department.
- Works with the Quality Improvement (QI) department to support grievances, appeals and advocacy consistent with participant's rights.

Do You Have What the Role Requires?
- Master's degree in social work from an accredited college or university PLUS 2 years of experience in a health‐related field required; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
- 1 year of experience working with the elderly population with relevant experience within the last 3 years required.
- Bilingual in English and in one of CalOptima Health's defined threshold languages (Arabic, Farsi, Chinese, Korean, Russian, Spanish, Vietnamese).
- CPR and First‐Aid Certification required or must be obtained within six (6) months of the date of hire.
- Valid driver's license and vehicle or other approved means of transportation, an acceptable driving record and current auto insurance will be required for work away from the primary office 30% of the time or more.

Role Summary:
Under the supervision of the Operations Director, plans, organizes and implements in home services for PACE participants and families. Responsibilities include but are not limited to: Coordination of the day-to-day operations of the In Home Services for your team of participants. Act as the liaison between the contracted home health agency and the PACE program coordinating homecare Nurses, Home Health Aides, Homecare Services Non-Skilled Aides and the Homecare Schedulers. Required Education:
Bachelor of Science in Nursing. Nurses with an Associate Degree or Diploma in nursing must sign agreement to obtain BSN within 6 years of the hire date in to position. Nurses who are nearing the end of their professional career may be exempted from this requirement with CNO System Council approval.
Required License:

The Position

Provides exceptional patient care in collaboration with anesthesiologists, surgeons and other physicians to deliver anesthesia for medical and surgical procedures. Serves as the focal professional for the continuity of care in patient care within scope of practice standards, as well as specialty specific standards to address and resolve problems that may arise. Overview: A call CRNA is part of a core team of dedicated CRNAs who provide timely anesthetic and emergency care for patients in a 24/7 model. This position is full-time and distinguished from non-call CRNAs who have a regular, patterned schedule. A call CRNA position entails a flexible, non-pattered schedule with in-house call responsibilities including off-shifts, weekends, and holidays. A call CRNA possesses excellent clinical skills, a commitment to patient safety, and the ability to work effectively in a fast-paced environment. A call CRNA is eligible for the call compensation program in recognition of the position and commitment to the core team. Schedule Requirements: Availability to work off-shifts, weekends, and holidays as part of the restricted in-house call rotation in partnership with other members of the core call team. Qualifications
EDUCATION
Certified Registered Nurse Anesthetist Completion of Certified Nurse Anesthetist Training Program Graduate of a Nurse Anesthesia Educational Program accredited by the American Association of Nurse Anesthetist Council on accreditation of Nurse Anesthesia Programs. Current State of Connecticut Nursing License, Advanced Practice Registered Nurse (APRN) licensure in the State of Connecticut.
EXPERIENCE
Prefer clinical working experience as a CRNA within the given specialty/service line recommended, but none required. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice as {an APRN, PA, CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible.
LICENSURE
Certified Registered Nurse Anesthetist AANA Certification, State of CT RN and APRN License, ACLS DEA Registration and State of CT Controlled Substance Registration
SPECIAL SKILLS
CRNA's must have sharp concentration and focus, along with the ability to stay calm under stressful conditions. CRNAs must be able to communicate clearly and effectively with surgeons and the surgical support team, as well as with patients and their families. A calm and reassuring manner is one way to inspire trust in patients, who may be apprehensive about surgery, as well as the surgical team. Anesthetic patient management must be immediate and responsive. Flexibility and adaptability are required.
PHYSICAL DEMAND
A CRNA must be able to demonstrate intellectual-conceptual, integrative and quantitative abilities; skills in observation, communication and motor functions; and mature behavioral and social attributes. Sensory- CRNA must be able to detect and interpret changes in monitoring alarms and equipment. Functional vision, hearing. no impairment of sense of smell and tactile sensation must be adequate. CRNA must be able to observe a patient accurately at a distance and close.Communication-CRNA should be able to speak, hear and observe. CRNA must be able to communicate effectively- includes speech, reading and writing.Motor -CRNA should have sufficient motor function to elicit pertinent information. CRNAs should be able to stand for long periods of time and wear lead as needed for fluoroscopy, X-Ray cases. CRNAs must have the ability to lift 25 pounds and reach above and below shoulder height. CRNA must be able to negotiate patient care environments and be able to move self/patients between anesthesia locations. Coordination of both gross and fine muscular movements, equilibrium, and functional use of the senses of touch and vision is required. Intellectual-Conceptual, Integrative and Quantitative Abilities-Abilities include measurement, calculation, reasoning, analysis and synthesis. Problem-solving, the critical skill demanded requires all of these intellectual abilities. CRNA should be able to comprehend 3-dimensional relationships and understand the spatial relationships. Behavioral and Social Attributes -CRNA must possess the emotional health required for full utilization of intellectual abilities, the exercise of good judgment, the prompt completion of responsibilities. CRNA must be able to tolerate physically taxing workloads and to function effectively under stress and able to adapt to changing environments, display flexibility and learn to function in the face of uncertainties inherent in the clinical problems of many patients.

This role is ideal for someone who is analytical, organized, and eager to take ownership of complex projects in a fast-paced federal grant environment. This is a contract to hire role and fully onsite.
Key Responsibilities
- Manage a portfolio of grant projects from application through closeout.
- Track reporting requirements and maintain accurate project documentation.
- Review project materials for compliance with federal regulations
- Support audits, monitoring reviews, and internal reporting needs.
- Coordinate with Finance on budget verification, reimbursement requests, and financial documentation.
- Monitor project performance, identify risks, and escalate issues as needed.
- Communicate with internal teams, state partners, and grantees on program requirements and status updates.
- Contribute to improving internal processes and grant administration systems.
Qualifications Include:
- 3–4 years of experience in grant administration, project management, or program coordination.
- Working knowledge of federal grant regulations

- Experience with tracking systems (Excel, SharePoint, etc.).
