Interventional Cardiologist

WVU Medicine – United Hospital Center seeks an interventional cardiologist to join a well-established group with a strong reputation for providing excellent general and advanced cardiology care to a patient base of close to 300,000. The clinic is located in a medical office building connected to the hospital, for ease of movement between the two. There are three state-of-the-art cath labs situated on the second floor of the hospital, between a 20-bed critical care unit and surgical services. The interventional cardiology program is robust, with a very experienced staff.
Duties: Successful candidates will diagnose and treat patients with cardiovascular disease as well as congenital and structural heart conditions through catheter-based procedures.
Qualifications: Successful candidates must have an MD or DO degree or foreign equivalent and be eligible to obtain a state medical license. Applicants must have completed an accredited residency in Internal Medicine and fellowships in General Cardiology and Interventional Cardiology. Applicants must be board certified or eligible in General and Interventional Cardiology by the American Board of Internal Medicine. All qualifications must be met at the time of appointment.

A Brief Overview

Registered Polysomnography Technologist- Level II performs polysomnography, diagnostic and therapeutic services, Home Sleep Apnea Testing, patient care and education and works under the general supervision of the Supervisor and Medical Directors to provide comprehensive evaluation and treatment of sleep disorders. May serve as a preceptor to PSG Trainee and/or Technician in all age groups and settings. May need to travel to various sleep lab locations based on department needs to cover patient care and other duties as assigned.
What You Will Do
* Perform appropriate scoring of both adult and pediatric sleep studies.
* Perform testing procedures PSG, titration studies, MSLT, MWT, PAP Nap, HSAT, other sleep procedures, and provide effective respiratory care according to UHC Sleep Center protocols.
* Identifies and corrects machine malfunctions.
* Collaborates with Sleep Center staff, and members of other disciplines to ensure a strong team and contribute to the achievement of the operational goals of the assigned area.
* Provides education and age appropriate care to patients and families through the entire age spectrum, including 0 through 18 years, adults, and geriatric populations.
* Takes responsibility for adherence to professional standards, self-development, and participation in quality improvement activities.
* Competent in the proper use of PPE.

In this RN Field Clinical Care Manager role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.
There will be travel expectations throughout advertised boroughs.

Primary Responsibilities:
- Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care
- Perform the NYS UAS Assessment in the member's home at least twice per year and as needed
- Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
- Manage the care plan throughout the continuum of care as a single point of contact
- Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
- Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:
- Current, unrestricted RN license for the state of New York
- 2+ years of relevant clinical work experience
- 1+ years of experience of community case management experience coordinating care for individuals with complex needs
- Experience in long-term care, home health, hospice, public health or assisted living
- Proficiency with MS Word, Excel and Outlook
- New York state issued ID or ability to obtain one prior to hire
- Reside in New York state
- Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
