Primary Responsibilities:

• Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care

• Manage the care plan throughout the continuum of care as a single point of contact

• Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:

• Current, unrestricted RN license in the state of Hawaii 
• 2+ years of experience working within the community health setting OR in a health care related role
• Experience working directly or collaborating services for long-term care, home health, hospice, public health or assisted living
• Intermediate level of proficiency with MS Word, Excel and Outlook
• Willing or ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
• Current access or ability to obtain internet access via a landline

• Full oversight of the specialty sale to achieve health plan market sales goals and maintain profitability

The RN Clinical Care Coordinator will be the primary care manager for a panel of members with complex medical/behavioral needs. The primary duty of the Renewal Account Executive is to retain existing agent and broker groups through exceptional service in order to promote health care sales and market UnitedHealth Care products. The Renewal Account Executive focuses on the small business market segment, those customers employing up to 50 employees.

Duties: Successful candidates will diagnose and treat conditions affecting the urinary tract and reproductive system that will include medication management as well as performing surgical procedures. The physician will work both in the inpatient and outpatient settings.

Qualifications: Successful candidates must have an MD or DO degree or foreign equivalent and be eligible to obtain a state medical license. Applicants must have completed an accredited residency in Urology. Applicants must be board certified or eligible in Urology by the American Board of Urology. All qualifications must be met at the time of appointment.
