Summary:
The Director Pharmacy manages the medication use process in collaboration with the Medical Staff and Nursing Services by organizing the processes and functions of the Pharmacy Department efficiently, economically, and professionally in accordance with Hospital and Departmental policy and applicable federal and state legal requirements. This job fosters the development of Pharmaceutical Care through process improvement of distributive and clinical pharmacy services. Assists in the leadership of the pharmacy department through the development and implementation of plans and goals that are in accordance with CSFCH goals and CHRISTUS Christian philosophy and values. Responsible for the cost-effective management of SFCH resources through prudent inventory and supply control and personnel utilization, while maintaining quality service and patient care. Responsibilities:

Job Summary:
Prepare, compound, dispense medications, and perform other technical duties in the pharmacy department under direct supervision of a Pharmacist. Must demonstrate competency through supervised observation and/or any other necessary annual required competencies. Core Responsibilities and Essential Functions:
Medication Distribution
* Single Dispense - must prepare medications and/or IV for dispensing, select correct medication, correct strength, and correct form. Fill, label, and initial medication bag for a pharmacist verification.
* Bulk Dispense- Must fill multiple patient specific medications in accordance with department procedure.
* Transportation- bulk fill, first dose, and missing doses will be delivered securely to the appropriate units, while practicing appropriate procedure to ensure confidentiality for all patients.
* Missing Doses- upon request, must properly generate label, fill and have checked by a pharmacist, then dispense to the appropriate floor or unit.
* Product Preparation- must properly prepare, measure, count, medications including compound ointments, creams, oral solutions, oral solids, IV, etc.
(ADC) Automatic Dispensing Cabinet
* User- must be capable of accurately adding new users to the ADC system.
* Product- must properly load ADC with correct medications after medication is checked by a pharmacist. Monitor par levels and increase or decrease as needed.
* Reports- must be able to generate needed reports to fill, track, and identify/report all discrepancies.
* Minor Maintenance- must trouble shoot minor problems with ADC.

Job Summary:
The major responsibilities of this position are drug distribution, clinical intervention, education, administration, drug information and research.
Core Responsibilities and Essential Functions:
Clinical Intervention
* a. Maintains competency in all aspects of drug therapy for patients of all age groups.
* b. Provides pharmaceutical care by assisting other health care providers or providing direct patient care by:
* Assessing patient demographics
* Reviewing patient medication history
* Developing pharmacotherapeutic plans
* Developing drug monitoring plans
* Implementing new drug regimens
* Documenting medication related issues
* Interpreting laboratory data
* Reviewing patient medication profiles and making recommendations for drug therapy management
* Makes patient care rounds focusing on drug therapy issues
* Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions
* c. Provides pharmacokinetics consultations as required, specifically addressing drugs with narrow therapeutic indices.
* d. Participates in drug therapy management during codes.
* e. Maintains and updates a patient profile with demographics, diagnosis, allergies, and current medications.
* f. Discusses medication order clarifications with the prescriber, document changes in patient and pharmacy records.
* g. Reconciles pharmacy and nursing medication records daily.
* h. Provides on-call coverage for pharmaceutical care as requested.
* i. Identifies and proceeds with cost avoidance in drug therapy by promoting more cost effective regimens, drugs, and by reducing waste.
Drug Distribution
* a. Residents provide the equivalent of approximately 20 hours per month (excluding orientation and transition rotation) of pharmacy practice staffing during the residency program.
* b. Promotes use of WHS formulary by converting non-formulary orders to formulary when possible.
* c. Resident is scheduled to work their required shifts primarily in the Central Pharmacy or Resource Zone to gain a working knowledge of hospital pharmacy operations.
* Checks unit dose medications for accuracy
* Reviews and verifies medication orders for appropriateness and checks first doses of IV admixtures and unit dose medications prepared by technicians
* Ensures proper technique and accurate preparation of all pharmaceutical products, including oral, IV admixtures, chemotherapeutic, and investigational agents
* Ensures the timely and accurate dispensing of drugs and solutions from the pharmacy area
* Responsible for the accurate filling and dispensing of medication orders used by inpatients
* Provides drug information to health care professionals and to patients
* Reviews physician orders for possible therapeutic problems, contraindications, interactions, allergies, and formulary status of the drug
* Provides computer order entry for IVs and other computer entry as needed
* Ensures compliance with controlled substance distribution and maintenance system
* Is able to practice in a variety of areas within the pharmacy as required by workload
Education
* a. Precepts Pharm.D. clerkship students consistent with academic credentials and clinical experience.
* b. Provides presentations, publications, and other informative activities on drug-related topics to the health care community and general public.
* c. Participates in continuing education programs, meetings, training programs and related activities.
* d. Educates patients on admission or discharge medications as necessary or requested.
Drug Information and Research
* a. Participate as a member of WHS Pharmacy and Therapeutics (P&T) Committee.
* b. Monitoring patient outcomes in relation to medication use. Conducts target drug programs and medication use evaluations as needed.
* c. Detects, prevents, monitors, documents, and reports ADEs and MEs.
* d. Provides drug information to health care professionals and to patients.
* e. Assist in formulary maintenance by critiquing new entities, comparing medication classes, and developing therapeutic interchanges, when applicable.
* f. Assist in developing drug clinical practice guidelines (CPGs) as needed.
* g. Complete a research project designed to improve the services of the department
Administration/leadership
* a. Documents clinical interventions (pharmacokinetic consultations pharmacotherapeutic consultations, anticoagulation consultations, patient consultations) and cost avoidance as part of the Pharmacy Services and hospital documentation programs.
* b. Assist in preparing for and maintaining compliance with regulatory agency (e.g. The JC) standards/requirements.
* c. Participate on committees, teams, or task forces relating to area of preceptors individual practice.
* d. Meet with pharmaceutical sales representatives to obtain/assess clinical data.
* e. Attend Directors meetings (e.g. Pharmacy Directors, Partners).
* f. Assist with coordinating clinical pharmacy activities, schedules, programs and projects.
* Performs other duties as assigned
* Complies with all WellStar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
- High School Diploma General or GED General
- Enrolled or Completion of an accredited college of Pharmacy.
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
- RPH - Registered Pharmacist or RPH-P - Reg Pharmacist Provisional (180 Days) within 180 Days.
Additional License(s) and Certification(s):
- Licensed by or eligible for licensure with the Georgia State Board of Pharmacy upon hire.
- If not licensed at the start of residency, the candidate must be licensed by August 31st of their residency year.

The Pharmacy Technician Medication Coordinator position at Genoa is an essential role that impacts the lives of an underserved population that needs a voice. This onsite position is a liaison between a partner clinic and a Genoa mental health specialty pharmacy.
Primary responsibilities include building strong relationships with the clinic partners and communicating with consumers to assist them with their medication plan, insurance and the full services of the Genoa pharmacy. The hours for this position are: Monday-Friday: 8:30am-5pm, Closed for Lunch: 12:30pm-1pm
Primary Responsibilities:
- Communicates with all consumers of the mental health center regarding the medication services Genoa provides
- Recruits and enrolls consumers utilizing enrollment forms and copy the consumer's insurance card
- Facilitates the collection of prescriptions to be faxed to the pharmacy for dispensing
- Ensures all consumer insurance information is up to date in the Pharmacy system and is properly charged for the medications dispensed
- Assists consumers in contacting the pharmacist regarding all their medication questions and ensure a prompt response to their questions
- Assists clinic staff and pharmacy with prior authorizations and Patient Assistance programs as needed
- Monitors compliance by contacting the consumer at least monthly to ensure compliance and determine refill needs as applicable
- Prescription refill management by utilizing the no refill report and contacting the appropriate prescriber to ensure timely refills are completed and dispensed to the consumer
- Medication delivery when needed and appropriate. {Only applicable in states that are allowed.}
- Checks for expiration dates (both on consumer's medications as well as house account products/standing order meds.)
- Ensures that medications are stored properly (refrigerated drugs are kept in the refrigerator, pen-tips are not stored on insulin pens, etc.)
- Checks for discrepancies (dose changes, discontinued medications, etc.)

Job Summary:
Prepare, compound, dispense medications, and perform other technical duties in the pharmacy department under direct supervision of a Pharmacist. Must demonstrate competency through supervised observation and/or any other necessary annual required competencies. Must be proficient in all responsibilities and duties of certified pharmacy technician.
Core Responsibilities and Essential Functions:
Medication Distribution
* Single Dispense - must prepare medications and/or IV for dispensing, select correct medication, correct strength, and correct form. Fill, label, and initial medication bag for a pharmacist verification.
* Bulk Dispense- Must fill multiple patient specific medications in accordance with department procedure.
* Transportation- bulk fill, first dose, and missing doses will be delivered securely to the appropriate units, while practicing appropriate procedure to ensure confidentiality for all patients.
* Missing Doses- upon request, must properly generate label, fill and have checked by a pharmacist, then dispense to the appropriate floor or unit.
* Product Preparation- must properly prepare, measure, count, medications including compound ointments, creams, oral solutions, oral solids, IV, etc.
(ADC) Automatic Dispensing Cabinet
* User- must be capable of accurately adding new users to the ADC system.
* Product- must properly load ADC with correct medications after medication is checked by a pharmacist. Monitor par levels and increase or decrease as needed.
* Reports- must be able to generate needed reports to fill, track, and identify/report all discrepancies.
* Minor Maintenance- must trouble shoot minor problems with ADC.
* Restock- must accurately fill medication, fluids and stock outs as needed.
* Recall- must assist the pharmacist and inventory specialist in retrieving recalled medications.
* Controlled Substances- Must follow all policies, procedures, and applicable laws with controlled substance medications.
IV Admixture
* Aseptic technique- Demonstrate knowledge of aseptic technique including; proper hand washing and proper use of IV Room attire. Understand Joint Commission, hospital, and departmental policies and procedures. Must complete an aseptic technique competency to show compliance with USP standards, conducted by supervisor or Lead Pharmacy Technician.
* Small volume- Demonstrate knowledge of preparing small volume admixtures
* Specialty solutions- Demonstrate knowledge of preparing epidural, intrathecal, intravenous, intramuscular, sterile syringes and chemotherapy (when applicable)
* Syringes- Demonstrate knowledge of preparing neonatal syringes (when applicable)
* Large volume- Demonstrate knowledge of preparing large volume solutions
* Room cleaning - Demonstrate knowledge of policies and procedures regarding maintaining clean room environment.
* Safety and Compliance Follows all safety and compliance standards for safely compounding medications. Always ensure that all requirements and procedures of United States Pharmacopeia are met.
Miscellaneous Activities:
* Repackaging and bar-coding medications- must accurately label all repackaged items per Georgia Board of Pharmacy Rules and Regulations.
* Properly provide customer service to customers and other healthcare professionals via in-person or virtual means.
* Process any charges: late charges, supplemental, OR charges, nuclear and exception report.
* Ordering supplies and obtaining purchase orders using the appropriate ordering system.
* Must be proficient in the distribution and accountability of Controlled Substance Medications.
* Code Carts / Kits - Must restock code carts and other kits returned to pharmacy according to departmental procedure and ensure required inspections are completed.
* Must perform monthly unit and pharmacy inspections and ensure that assigned pharmacy or medication area is clean including temperature and humidity monitoring.
* Must perform out date tracking on medications in various locations of the facility.
* Train and mentor pharmacy technicians under lead pharmacy technician
