Job Summary:
The Inpatient Transitions of Care (TOC) Pharmacy Technician will function as an integral part of the medication management patient care team in collaboration with TOC Clinical Pharmacy Specialists. The Inpatient TOC Pharmacy Technician will be responsible for completing the following tasks to reduce medication-related errors and ensure high-quality patient care. Conducting a best possible medication history (BPMH) on high-risk patients with specific disease states serviced by the TOC Team (Heart Failure, Chronic Obstructive Pulmonary Disease, and Diabetes Mellitus) admitted to an inpatient unit at Wellstar Cobb Medical Center Documenting medication discrepancies for further assessment by the TOC clinical pharmacy specialist Assessing patient medication compliance Completing an insurance coverage assessment Completing prior authorization requests Providing information related to patient assistance programs Facilitating utilization of the Dispensary of Hope (DOH) and Meds-to-Beds programs Completing follow-up care calls post-discharge

JOB SUMMARY
Interprets, obtains, prepares/compounds, labels, and distributes pharmacy products for patient use while taking into consideration patient age and any other relevant information. Depending on department, may also prepare IV medications using special equipment supplies and techniques for patient administration and processes refill requests for non-monitored pharmacy medication such as enteral products/supplies or respiratory medications.
