Position Details:
- Provide specialized outpatient care for patients with sleep-related chronic health conditions in accordance with established scope and standards of practice.
- Conduct follow-up visits including CPAP management, medication management, review of sleep study results, and evaluation of sleep therapy options.
- Collaborate with the care team to support optimal patient outcomes in sleep health.
- Preferred experience as an advanced practice provider in Sleep Medicine, Respiratory care, Cardiology, Pulmonology, or Internal Medicine.
- Schedule consists of four 10-hour shifts per week.
- Epic EMR utilized system-wide.

Requirements for Physician Assistants
- Master's Degree in Physician Assistant Studies
- Unrestricted license to practice as a Physician Assistant in the State of Colorado.

About this Position

LTS Speech & Language Pathologist: Mon - Fri 8:30a - 3:30pm
Compensation: $45/hr.
The Speech/Language Pathologist is responsible to assess, plan, implement and evaluate patient care programs designed to help prevent speech, language, cognitive, communication, voice, swallowing, fluency, and other related disorders; and assist patients in reaching their maximum function. Care is provided in accordance with School policies, procedures, IEP plans of care, and established standards of practice.
Essential Functions:
- Participate in screening of students and complete all corresponding paperwork.
- Provide direct and indirect speech and language therapy services and consultation to students in the school setting.
- Complete evaluations, reports and design student plans based on individual need.
- Communicate the needs of students with appropriate school personnel and parents to facilitate acquisition of goals.
- Schedule and/or attend meetings and invite team members.
- Attend parent/teacher conferences and participate in in-service workshops.
- Support a full range of services to help students develop the communication skills they need to succeed in the classroom and beyond.
- May supervise professional and non-professional personnel providing patient care. Provides supervision to others through task assignments, direction, evaluation and feedback of assigned tasks.

- Successfully completed an approved national examination in speech-language pathology

JOB SUMMARY
The Counselor provides clinical assessment/problem identification, crisis interventions, individual, family and group therapy and case management. Develops and monitors treatment plans, monitors treatment progress, documents significant interactions, and maintains clinical charts and other reporting data. Must be licensed or provisionally licensed and requires a Master’s degree in a mental health field.
ESSENTIAL FUNCTIONS
- Provides assessment, diagnosis, and treatment for behavioral health clients.
- Provides clinically appropriate therapy, utilizing best practices whenever possible.
- Is actively engaged in the department, solves problems, participates in strategic planning, and contributes positively to workplace moral.
- As assigned by manager and location participates in on call crisis rotation after hours and assists with walk-in crises during business hours.
- Works positively and collaboratively with courts, law enforcement, physicians, hospital staff, Department of Family Services, Probation Offices, Campbell County School District and other community agencies as applicable.
- Maintains clinical documentation in a manner as required by department policy, the Standards for the Operation of Community Mental Health and Substance Abuse Programs, and the Medicaid Policies and Procedures Manual, and in accordance, as applicable with SAMHSA TIPS and TAPS.
- Maintains competence in multicultural issues and demonstrates respect and understanding for people from diverse cultural backgrounds.
- Adheres to established policies and procedures for Behavioral Health services, Kid Clinic and Campbell County Memorial Hospital.
- Actively engages in departmental quality assurance and utilization management activities and processes to include monthly chart reviews.
- Provides community-based psychotherapy to individuals, groups and families as applicable.
- Maintains professional licensure through required educational and training experiences.
- Participates in activities to promote mental health and/or the prevention of substance abuse.
- Must be free from governmental sanctions involving health care and/or financial practices.
- Complies with the hospital’s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.

JOB QUALIFICATIONS
- Education
- Master’s degree in Social Work, Counseling, or Addiction Studies
- Licensure
- Must be licensed or license-eligible in the State of Wyoming as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Addictions Therapist (LAT).
- If not fully licensed at the time of hire, candidate must obtain full or provisional licensure within sixty (60) days of hire.
- Licensed professionals must adhere strictly to the ethical code and scope of practice of their respective discipline and maintain licensure in good standing throughout employment.

The Anesthesiologist Assistant assists the anesthesiologist and cares for patients before, during and after procedures. This position functions as the extension of the physician and are responsible for performing critical measures to maintain the safety of the patient to ensure optimal outcomes.
Minimum Education
- Master of Science Anesthesia or Master of Medical Science.
Required Licenses/Certifications
- Certified Anesthesiologist Assistant NCCAA
- Licensed in the State of Vermont.
- BLS Certification through American Heart Association.
Required Skills, Knowledge and Abilities
- Excellent written and verbal communication and interpersonal skills required to deal with patients from diverse socio-economic backgrounds.
- Basic Microsoft desktop application and navigation skills.
