Position Title: Supported Living Program Case Coordinator

Job Description

As a Supported Living Service Case Coordinator, you will assist consumers with a variety of services and trainings to enable them to live independently in their community. Our program serves people from Boston to Southeastern Massachusetts. Case Coordinators are the difference in the lives of our consumers on a regular basis. Tempus takes a person-centered approach to all services.
Essential Functions
- Develop Supported Living Service Plans
- Maintain communication with the Personal Care Attendant (PCA) (or other service) provider.
- Assist consumers in the hiring, training, scheduling and supervision of their Personal Care Attendants, sign onto the PCA program Service Agreement if surrogacy is required. Be thoroughly detailed in the understanding of this document and assist consumer comply also.
- Assist Consumers in obtaining housing if needed.
- Assist consumers in setting up and maintaining appropriate records regarding Personal Care Attendants (PCA), finances and medical issues.
- Assist consumers with accessing community resources such as health care, recreation, transportation and adult education.
- Encourage and assist consumers in the development of relationships with other members of the community.
- Maintain confidential records according to program guidelines.
- Train consumers annually on human rights and how to obtain assistance on human rights violations.
- Assist consumers with Transitional Assistance services through the Money Follows the person (MFP) and Acquired Brain Injury (ABI) waivers.
- Follow MRC Community Living Program manual standards, as well as other regulatory documents related to the position.
- Must report all suspected incidents of consumer sexual/physical abuse and neglect to the Disabled Person Protection Commission (DPPC).
Job Requirements
Required Education
- Bachelor's degree and/or at least two years' experience serving people with disabilities
Competencies
- Familiarity with community services, the ability to understand and implement independent living philosophy and the ability to relate and empathize with people with disabilities and help them maximize their lives is required.
- Being resourceful to solve complex issues at times.
- Objective report writing.
Preferred Experience
- Training and supervisory experience is helpful.
- Significant experience in Personal Care Attendant (PCA) services, case management services, and disability service delivery systems is preferred.
- Good communication, organization and writing skills are required.
Work Environment

This is a great opportunity for a Registered Nurse (RN) to work a flexible schedule in a unique program, Personal Care Management Evaluation Department, that makes a truly wonderful difference in the lives of children and adults with a wide range of disabilities. This position does require working in the community. Travel within the coverage area is required for this position. Must have a valid driver’s license and reliable transportation.

Tempus Unlimited is seeking an experienced, outgoing, professional individual to provide functional evaluations for consumers living in the community under the direction of Tempus Unlimited, Inc. The Occupational Therapist will perform assessments/evaluations and make recommendations to MassHealth for PCA services in the home setting. The ideal candidate is a self-starter, able to work independently out in the community, and is an organized individual who has a desire to work with people with disabilities.
Essential Functions
- Conduct evaluations for Personal Care Attendant (PCA) services based on MassHealth Standard Operating Procedures and regulations
- Evaluate all consumers in consideration of: range of motion, movement analysis, fine motor skills, sensory motor processing and functional activities of daily living
- Present findings and recommendations of the evaluations to MassHealth
- Recommend special or adaptive equipment as necessary to meet the needs of the child or adult at home, at school, and in adaptive settings
- Maintain up-to-date data (progress notes, reports, statistics, quarterly reports and any other reports deemed necessary) in the manner set forth and defined by the agency
- Inform Program Manager and other staff of new developments in the field of occupational therapy
- Maintain an up-to-date weekly appointment calendar in Outlook shared with Manager
- Follow all HIPAA rules regarding privacy and maintenance of records to be secured and held at Tempus Unlimited
- Must report all suspected incidents of consumer sexual/physical abuse and neglect to the Disabled Person Protection Commission (DPPC)
- Attend monthly agency and department meetings
Required Experience

A community based Senior Skills Trainer for our Personal Care Attendant Program. The Skills Trainer will educate consumers on how to successfully manage their consumer-directed hands-on care. The Tempus Electronic Visit Verification (EVV) Compliance specialist has a key role in supporting the state and the agency partners in ensuring PCA program participants are compliant with the use of EVV and the EVV policies. This role is responsible for implementing and overseeing the Tempus FI aspects of the EVV Compliance policies set forth by MassHealth and ensures Tempus FI satisfies contract requirements.
This role involves a broad range of tasks including: preparing compliance reports and dashboards for the state, agencies and internal teams, monitoring EVV compliance metrics and supporting CMS-related reviews and compliance assessments, overseeing/initiating needed changes to the PowerBI dashboard, researching non-compliance cases, assisting with corrective action plans and/or process improvements, and communicating across departments, with MassHealth and other contracted agencies. This role and associated activities will evolve as EVV compliance requirements increase with the maturity of the implementation.

Operations
- Monitor the effectiveness of current compliance procedures and tools and lead the efforts to continuously improve to achieve the needed results e.g., identifying breakdowns and root causes and communicating and implementing changes
- Support the development of any new EVV Standard Operating Procedures
- Participate in EVV governance, change-control and system enhancement discussions
- Understand barriers or root causes of why individuals are not compliant and drive remediation plans
- Work with system vendor and EVV SME to resolve technical issues affecting compliance
Research Escalations and Support Communications
- Perform compliance-related research to answer questions or determine if an issue is non-compliant
- Support requests received by MassHealth/Program Integrity Team
- Support agencies and/or work in partnership with EVV Support
- Support state-led outreach and communications related to EVV compliance expectations
- Perform escalation follow-up for non-compliant individuals as necessary
- Oversee the communication plan for EVV Compliance:
- Draft, modify, and distribute EVV compliance communications
- Send PCA non-compliance list to the EVV Support team who assist with PCA outbound calls
- Create lists for Everbridge compliance messages and collaborate with IT to monitor the process.
- Send address list to Mailing Department for warning letter preparation
- Manage all incoming/outgoing EVV compliance-related emails and escalating any observed trends or case specific
Training
- Collaborate with the training and knowledge management teams to ensure EVV guidance is accurate and contract aligned
- Provide EVV Compliance training to Tempus operations teams and other individuals as necessary
- Review EVV job aids, FAQs, scripts and training content for compliance accuracy
Risk Management
- Identify compliance risks and recommend process improvements to strengthen compliance and reduce/eliminate inefficiencies.
