Job Description

As an Overnight Individual and Home Support worker you will be assisting Consumer in a variety of ways that enable them to live independently in their community. Tempus takes a person-centered approach to all services. This is a part time overnight with some awake and asleep.

  • $20 an hour
  • Thursday and Friday, 12am to 6am every other week

  • 12 hours over two weeks
  • PCA and IHS experience required
  • Hoyer lift experience required
  • 2-day orientation in Stoughton office

Travel

  • Must have a valid driver's license
  • Reliable transportation

Essential Job Functions (as determined by the supervisor)

  • ADL Tasks
    • Cleaning: maintaining cleanliness throughout the home as needed, i.e.: dusting, wiping down counters, sweeping, vacuuming, dishes. Bathroom: disinfect toilets and sinks, wash floors.
    • Laundry: wash, dry, fold, and put away clothes, towels and bedding
    • Assist Consumer with the administering and taking of daily medications.
    • Meal Prep / Cooking: Consult with Consumer about types of food he enjoys and plan weekly menus. Work together to create a grocery list within his budget, and encourage healthy choices.
    • Shopping: Establish a system for replacing household supplies from local stores in a timely manner.
  • Adaptive Equipment: Consumer uses the following adaptive equipment that staff will need to become familiar with and use on a regular basis:
    • Hoyer lifts (both motorized and manual)
    • Motorized wheelchair
    • Adjustable bed / bed rails
    • Automatic door openers
  • Other
    • Support overall mission and objectives of Tempus Unlimited
    • Must report all suspected incidents of consumer sexual abuse, physical abuse and neglect to the Disabled Person Protection Commission (D.P.P.C.).
    • Perform other duties as assigned by Supervisor or designee.

Competencies

  • Familiarity with community services, the ability to understand and implement independent living philosophy and the ability to relate and empathize with people who have disabilities and help them maximize their lives is required.
  • Being resourceful to solve complex issues at times.
  • Objective report writing. Staff will be required to update a task sheet for each shift in a communication log.
  • Must report all suspected incidents of consumer sexual/physical abuse and neglect to the Disabled Person Protection Commission (DPPC).
  • ISS shifts will require calling in to track your time at the beginning and end of your duration.

Preferred Experience

  • Experience in Home Support Services.
  • Good communication, organization and writing skills are required.

Required Education and Certifications

  • High School degree and/or experience serving people with disabilities.
  • CPR / First Aid Certification.

As a Supported Living Service Case Coordinator, you will assist consumers with a variety of services and trainings to enable them to live independently in their community. Our program serves people from Boston to Southeastern Massachusetts. Case Coordinators are the difference in the lives of our consumers on a regular basis. Tempus takes a person-centered approach to all services.

Essential Functions

  • Develop Supported Living Service Plans
  • Maintain communication with the Personal Care Attendant (PCA) (or other service) provider.
  • Assist consumers in the hiring, training, scheduling and supervision of their Personal Care Attendants, sign onto the PCA program Service Agreement if surrogacy is required. Be thoroughly detailed in the understanding of this document and assist consumer comply also.
  • Assist Consumers in obtaining housing if needed.
  • Assist consumers in setting up and maintaining appropriate records regarding Personal Care Attendants (PCA), finances and medical issues.
  • Assist consumers with accessing community resources such as health care, recreation, transportation and adult education.
  • Encourage and assist consumers in the development of relationships with other members of the community.
  • Maintain confidential records according to program guidelines.
  • Train consumers annually on human rights and how to obtain assistance on human rights violations.
  • Assist consumers with Transitional Assistance services through the Money Follows the person (MFP) and Acquired Brain Injury (ABI) waivers.
  • Follow MRC Community Living Program manual standards, as well as other regulatory documents related to the position.

This is a great opportunity for a Registered Nurse (RN) to work a flexible schedule in a unique program, Personal Care Management Evaluation Department, that makes a truly wonderful difference in the lives of children and adults with a wide range of disabilities. Essential Functions

  • Perform initial evaluations and annual reevaluations of members who are applying for personal care services in accordance with MassHealth regulations 130 CMR 422.422(C).
  • Submit completed evaluation within the specific timeframe.
  • Respond to hours adjustment requests in a timely manner.
  • Respond to MassHealth inquiries and offer professional input regarding PCA evaluation as requested. Learn and adhere to the PCA Program standards.
  • Schedule reevaluation visits and update your calendar accordingly.
  • Document progress notes in data base in a timely manner.
  • Communicate with PCA Eval Dept Assistants in regard to no shows, no contacts, hospitalizations, termination requests and any other concerns.

The ideal candidate is a self-starter, able to work independently out in the community, and is an organized individual who has a desire to work with people with disabilities.

Essential Functions

  • Conduct evaluations for Personal Care Attendant (PCA) services based on MassHealth Standard Operating Procedures and regulations
  • Evaluate all consumers in consideration of: range of motion, movement analysis, fine motor skills, sensory motor processing and functional activities of daily living
  • Present findings and recommendations of the evaluations to MassHealth
  • Recommend special or adaptive equipment as necessary to meet the needs of the child or adult at home, at school, and in adaptive settings
  • Maintain up-to-date data (progress notes, reports, statistics, quarterly reports and any other reports deemed necessary) in the manner set forth and defined by the agency
  • Inform Program Manager and other staff of new developments in the field of occupational therapy
  • Maintain an up-to-date weekly appointment calendar in Outlook shared with Manager

Job Description

This is a community-based Enrollment Specialist for our Pennsylvania Self Directed Program. The Enrollment Specialist will educate participants and their direct care workers on how to successfully enroll and utilize services under the Community HealthChoices Participant-Directed Services Program.

  • $21 an hour
  • 35 hours a week full time
  • Monday - Friday
  • 8:30am to 4:30pm

Essential Job Functions

  • Accept and outreach to participant within 3 days of referral to determine how the packet will be delivered to the participant: mail, email or in person
  • Provide in-person participant orientation within 15 days of referral
  • Train participants and direct care workers the skills needed to enroll and manage this self-directed program

Qualifications

  • One year experience providing services for persons with disabilities
  • Experience or education in teaching is helpful
  • Knowledge of community resource that participants are involved with
  • Excellent communication, organizational, and writing skills
  • Basic computer skills in Microsoft Office/Outlook and the ability to navigate databases

Required Education

  • Bachelor’s Degree preferred; GED or High School Diploma required

The Tempus Electronic Visit Verification (EVV) Compliance specialist has a key role in supporting the state and the agency partners in ensuring PCA program participants are compliant with the use of EVV and the EVV policies. This role is responsible for implementing and overseeing the Tempus FI aspects of the EVV Compliance policies set forth by MassHealth and ensures Tempus FI satisfies contract requirements.

This role involves a broad range of tasks including: preparing compliance reports and dashboards for the state, agencies and internal teams, monitoring EVV compliance metrics and supporting CMS-related reviews and compliance assessments, overseeing/initiating needed changes to the PowerBI dashboard, researching non-compliance cases, assisting with corrective action plans and/or process improvements, and communicating across departments, with MassHealth and other contracted agencies. This role and associated activities will evolve as EVV compliance requirements increase with the maturity of the implementation.

  • Perform escalation follow-up for non-compliant individuals as necessary
  • Oversee the communication plan for EVV Compliance:
    • Draft, modify, and distribute EVV compliance communications
    • Send PCA non-compliance list to the EVV Support team who assist with PCA outbound calls
    • Create lists for Everbridge compliance messages and collaborate with IT to monitor the process.
    • Send address list to Mailing Department for warning letter preparation
    • Manage all incoming/outgoing EVV compliance-related emails and escalating any observed trends or case specific

Training

  • Collaborate with the training and knowledge management teams to ensure EVV guidance is accurate and contract aligned
  • Provide EVV Compliance training to Tempus operations teams and other individuals as necessary
  • Review EVV job aids, FAQs, scripts and training content for compliance accuracy

Risk Management

  • Identify compliance risks and recommend process improvements to strengthen compliance and reduce/eliminate inefficiencies.

Job Description

Tempus Unlimited, Inc. is a nonprofit organization that provides community-based services to empower children and adults with disabilities to live as independently as possible in the least restrictive environment. The agency, through its programs and services, encourages the inclusion of people with disabilities into the mainstream of society, including social, recreational, family and work activities.

The in-person face to face Regional Liaison is primarily responsible for working as liaison in the Salem office assisting Consumers and Personal Care Attendants (PCA) with inquiries and problem resolution. This role will extend to providing general in-person support to visitors for questions and documentation related to the overall Fiscal Intermediary (FI) program. The regional liaison will also be trained and capable of providing training and support for the Tempus EVV solution. Finally, the Regional Liaison will coordinate with the Consumer Advisory Board coordinator to address concerns and review opportunities within that region. Assure ongoing compliance with Tempus quality and FI contract requirements.

  • Full time position, 35 hours a week
  • In person role every day
  • Monday - Friday, 8:30 am to 4:30pm
  • 2 day required orientation is held in Stoughton, Massachusetts

Essential Functions

  • Act as a liaison between Consumers/Participants and/or PCAs/DCWs and Tempus
  • Respond to Consumer/ Participant and/or PCA/DCW inquiries and complaints
  • Provide in-person assistance, either scheduled or ad-hoc, related to FI documentation or program related questions
  • Help resolve inaccuracies or discrepancies related to reporting of activity time
  • Work with applicable parties to determine PA/WA status and if new PA/WA needs to be requested
  • Work with applicable parties to address and resolve PCA/DCW payment issues
  • Respond to issues that have been escalated by Consumers/Surrogates/Administrative Proxies/Waiver
    Participants and/or PCAs/DCWs.
  • Receive visitors and determine their needs and direct them to the appropriate person
  • Maintain visitor and ID badge register. Issue ID badges where appropriate
  • Deal with inquiries and provide accurate information
  • Perform administrative work, including supporting other departments, as needed and as time allows.
  • Must report all suspected incidents of consumer sexual/physical abuse and neglect to the Disabled Person Protection Commission (DPPC).
