This is a part-time on-site role for a Case Manager at Lakeside Manor in Staten Island, NY. The Case Manager will be responsible for coordinating care plans, conducting assessments, writing notes and ensuring quality care delivery.

Qualifications

• Case Management, Care Coordination, and Assessments skills
• Excellent communication and interpersonal skills

Job Title: Health Service Reviewers (RN)

Pay (openings for each location/market):

• Albany up to $52/hr
• Central Islip up to $60/hr

Overview: These RNs will be doing a mix of standard quality audits, complaint initiated investigations, and more. When they are onsite, the amount of time that they are at the location is dependent on the audit that is required. It is expected that Health Service Reviewers will be traveling onsite about 85% of the time.

These individuals will be traveling to IDD housing to do state required Recertification (must be done every 15 months) or investigating specific complaints (disease outbreak, falls, etc.).

Travel: 85% of this role is traveling to sites. It is more location based and they will be traveling to the counties that surround their location. If anyone is traveling and not able to return home, they are able to coordinate accomodations through the travel office and miles/food will be reimbursed at the federal rate. If they are not onsite, they can work from home or in the DOH office.

Summary: Based in NY, working at the direction of the New York State Department of Health (NYSDOH), Office of Aging and Long-Term Care, this individual will conduct surveillance and investigation activities related to Intermediate Care Facilities for Intermediate Care facilities for Individuals with Intellectual Disabilities (ICF/IDD). Duties include but are not limited to participating in surveys or complaint investigations, document finding, draft Statement of Deficiencies (SOD) within specified timeframes, testifying in administrative hearing ad needed.

The position is majority travel and will be onsite at facilities.

Qualifications:

• Strong interpersonal skills with the ability to communicate professionally with colleagues, supervisors, providers, medical and administrative personnel and residents/patients.
• Excellent communication (verbal & written) skills.
• Ability to work independently with minimal supervision.
• Ability to relate effectively to clinical and administrative personnel and patients.
• Computer proficiency with the ability to learn and understand new review programs and monitoring tools.

Order Picker: You will pick and prepare customer orders accurately and timely to meet daily production goals. At times you will be picking above ground level while adhering to safety procedures.

Restocker: You will scan labels, cut boxes, and restock shelves to prep your work area for the next shift while adhering to safety procedures.

Bulk Selector: You will be responsible for selecting product, pulling, and packing totes, as well as cutting cases of merchandise and/or stocking product. You will move full totes and cases onto a conveyor belt while adhering to safety procedures.

Custodian: You will be responsible for general cleaning and basic maintenance duties both inside and outside of the facility. In this role, you will clean and sanitize restrooms, vacuum, and remove spots from carpet, sweep and mop floors, clean and dust chairs, partition walls and other miscellaneous furniture, clean windows and brass and collect and remove trash from the warehouse while adhering to safety procedures.

• Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area

• Primary coverage areas include but are not limited to: Springfield MA

• Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month

• Execute a strong visit completion rate, pacing 35+ completed visits per month

• Have the option to provide availability Monday - Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m.

Other role expectations and requirements:

• Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)

This pay range represents the base pay for each In-Home Health Evaluation (and associated visit) completed by a physician. The number of In-Home Health Evaluations (and associated visits) completed per day may vary. Pay is dependent on a variety of factors including experience, education, geography and other relevant criteria. This position is eligible for other reimbursement opportunities, including pay per completed Diagnostic and Preventative Services (DPS), as ordered, in addition to the base pay listed above. Duties:

• The Medical Director for Addition Medicine and Mental Health Services is responsible, in collaboration with the Executive Director for establishing Hoag Memorial Hospital Presbyterian as a center of excellence in addition medicine and mental health care. The role oversees the strategic development, integration, and delivery of service across inpatient, outpatient, residential, and community-based settings.
• Designing and implementing comprehensive, evidence-based programs that support individuals across the continuum of care for substance abuse and mental health conditions.
• Enhancing access to integrated behavioral health services, including education, prevention, treatment, recovery, support, and crisis intervention.
• Expanding and improving clinical services for patients and families with a focus on quality, safety, and outcomes.
• Clinical oversight over direct medical services and multidisciplinary teams including supervision of clinical protocols, coordination of integrated care pathways, compliance with regulatory standards and promoting continuous quality improvement.
• Provide professional leadership, mentorship, and development of providers through clinical guidance, performance coaching and fostering a culture of continuous learning, collaboration and excellence in addiction medicine and mental health care.
• Leading education and outreach initiatives to raise awareness and reduce stigma around addictions and mental health targeting both the community and mental healthcare professional.
• Clinical coverage will be required as part of the team and leadership role.

Requirements:

• Ideal candidate would possess a minimum 10 years administrative and clinical experience in addiction medicine and health services
• Proven leadership in addiction within a treatment center, department, division or hospital
• Experience designing and implementing performance management systems in complex operational environments
• Strong problem-solving, analytical, communication and interpersonal skills.
• Knowledge and experience with insurance reimbursement, coding and laws and regulation affecting the provision of substance use and mental health in California
• Knowledge of DHCS licensing and certification
• Preferred: Advanced training or degree in medical leadership and management (MPH, MBA, MMM, fellowship in leadership, etc.)
• CA Medical License
• Board Certified Psychiatrist
• Fellowship Trained in Addiction Medicine & Board Certified in Preventative Medicine (Addiction Medicine) preferred.
