JOB TITLE: Clinical Review RN (Medicaid Cost Outlier)
Nurse Background: 2+ years of acute care/medical surgical experience required. Interqual/MCG experience a plus.
GENERAL RESPONSIBILITIES: This individual will complete the full spectrum of activities related to Utilization or Quality reviews as assigned. They will utilize their knowledge and expertise of the review program to conduct clinical level review, supporting Medical Review Analysts, and Physician Consultants to ensure an appropriate and accurate process.
DUTIES:
1. Conduct utilization reviews up to and including the appeal level. This includes chart screen, complete electronic worksheets, enter required information and make level one denial decisions when necessary.
2. Conduct quality and clinical study data collection reviews. This includes chart review, complete detailed electronic data worksheets.
3. Act as a resource for the administrative staff in training, problem solving, and clarifying procedures. Will provide technical assistance and conduct/participate in staff huddles.
4. Participate in collaborative training specific to clinical study objectives.
5. Other activities as may be deemed necessary.
QUALIFICATIONS:
1. Licensed as a Registered Professional Nurse in New York State.
2. Knowledge and experience with electronic medical records including utilization, quality, and clinical charting.
3. Ability to oversee, problem solve and work collaboratively with peers, medical, analytical, and administrative support staff.
4. Excellent written and verbal skills.
5. Ability to work independently with little supervision.
6. Ability and desire to be flexible, innovative, and creative.
EDUCATION & EXPERIENCE:
1. Baccalaureate degree in Nursing or graduate of an approved Registered Professional Nurses training program and licensed to practice in the State of New York.

- Ensure adherence to Agency policies and procedures and making sure those are accessible to employees and contractors during hours of operation.
- Perform other functions as requested by the regional director which may include the following:
- Participate in interviewing, selection, and ongoing evaluation of clinical personnel as requested by the Regional Director
- Personnel training, education, and competency validation
- Review and evaluate clinical documentation and charts for accuracy and completeness
- Participate in all Agency performance improvement and quality improvement initiatives including but not limited to quarterly medical record review
- Collect, document, and submit data on infections, occurrences, complaints and grievances, and performance improvement activities.
- Review Nurse shift reports for adherence to policy and for opportunities for performance improvement.
- Home chart completeness.
- Timeliness of staffing cases post referral
- Equipment tracking and ensuring supplies and equipment are available and in working order
- Oversight and management of fiscal affairs Implementation of state specific regulations

ChristianaCare is currently seeking a Statewide Registered Nurse to support the CMS GUIDE program. The Statewide GUIDE Nursing Care Delivery Specialist is a registered nurse who provides clinical care coordination, education, and triage support across all three GUIDE program sites in Delaware. This role supports consistent delivery of CMS clinical care requirements while supporting care navigators and maintaining collaborative relationships with beneficiaries' existing medical providers. Primary Function
The nurse will provide quality, safe, evidenced-based nursing care to achieve optimum outcomes both independently and as a valued team member of the practice. The nurse will follow the Nursing Process when providing patient care and be part of a multidisciplinary team serving patients enrolled in the Guiding an Improved Dementia Experience (GUIDE) Model Program.
Principal Duties and Responsibilities
- Demonstrates knowledge & skills necessary to provide care appropriate to adult and geriatric patients virtually or on-site within the practice, including knowledge of growth development, the ability to obtain and interpret information to identify patient needs and provide care needed
- Participates in a relationship-based care approach to care delivery
- Performs Nurse Visits such as Advance Care Planning, Transitional Care Management and Medication Reconciliation.
- Triages patient telephone messages and ensures appropriate disposition under the direction of the provider
- Functions as a liaison between patients/caregivers, physicians and staff to provide accurate communication
- Collaborates with team members to provide a smooth, organized and efficient flow of patients
- Responds to patient crises situation
- Provides patient/caregiver education and wellness counseling
- Performs medication history
- Supports the work of the MA
- Assists with staff education and evaluation of clinical competencies for new and existing employees (MA)
- Assists with orientation of new personnel to ensure that they are properly trained to support patient and provider.

- Will be required to make a minimum of one home visit as part of the GUIDE multidisciplinary team.

Education and Experience Requirements:
- A graduate of a state approved or National League of Nursing accredited School of Nursing
- RN license or eligible for licensure in the state Delaware
- Two years’ experience as a RN, physician office experience preferred
- BSN preferred; ADN considered with agreement of completion of BSN in 3 years
Special Requirements:
- Current RN licensure in the state in which the work is performed
- BLS certification required

Job Summary and QualificationsAs a Registered Nurse in the PACU/ Recovery you will provide excellent patient care to individuals following operative and invasive procedures. - Participates in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE participants.

This hands-on experience will include educational sessions, job shadow, assist in patient care.
May provide patient care in a CNA role as approved by manager (see CNA position description).Responsibilities:ASSISTS NURSING IN CARRYING OUT THE NURSING PLAN OF CARE.Vital signs are completed and reported to nursing within 1 hour of scheduled time.
I/O's are collected and reported to nursing 30 minutes prior to end of shift.
All assigned baths are completed within the shift assigned.
Notifies nurse of any significant changes in patient condition promptly.
Ambulates and assists patient with ADLs within assigned time frame.
Monitors for signs of skin breakdown and communicates to nursing staff when indicated.
Obtain and record patient weight and height as indicated.
Keep patient bed low, locked and side rails up when patient is in bed.
Check patient food trays for accuracy.
Prepare patient's environment for eating.
Assists the patient in preparation to eat their meal, ie.
sitting up in bed or chair, washing the patient's hands, cleaning the bedside table.
Reports patient's intake to nurse.
Assists patient with feeding as necessary.HELPS TO PROMOTE STEWARDSHIP OF RESOURCES AND ENVIRONMENT OF CARE.Returns all equipment to SPD ASAP and at least by the end of the shift.
Assist with the monitoring of refrigerator / freezer logs.
Cleans refrigerator, microwave and other areas when indicated.
Maintains the integrity of the PYXIS Machine and inventory through pushing the "take" button.
Is respectful of linen supplies and mindful of appropriate usage of pads, etc.
Protects the environment of the patient care area as well as the hospital campus from vandalism and liter.SERVICE EXCELLENCEConsistently uses appropriate "scripting" as instructed when communicating with patients and their families.
Models service excellence in caring for patients providing "above and beyond" service consistently exceeding patients and families expectations.
Strives to anticipate the needs of patients and takes initiative to meet / exceed the patient's needs or wishes.
Responds to call light promptly.
Acts as a "team player" assisting co-workers regularly when time permits.PERFORMS CLERICAL DUTIES RELATED TO PROCEDURES, AS IT RELATES TO A PATIENT CARE UNIT.Checks food trays within 10 minutes of arrival to floor.
(If appropriate for unit.) Ensures that all charts are supplied with appropriate forms for documentation.
Relays messages to appropriate caregiver within 5-10 minutes of receiving message.
Prioritizes orders in the order entry system in a timely and accurate manner.
All reports are posted in a timely and accurate manner.
Ensures that diet/tube feeding changes are completed and credited every shift.ASSISTS NURSING IN CARRYING OUT THE NURSING PLAN OF CAREAssist the nurse with patient assessment rounds.
Collect and label voided urine, sputum and stool specimens.
Remove urinary catheters and cleanse catheter site.
Assist the nurse with wound care and dressing changes.
Perform glucose checks utilizing unit glucose monitors.
Perform secretarial duties such as taking off orders and answering call lights.
Document I/O's and vital signs using the computer charting system.Requirements:Must have completed fundamentals in nursing Current CPR CertificationRequirements:Associate's Degree in Nursing required BLS requiredWork Schedule:PRNWork Type:Per Diem As Needed
