We are hoping you will join us as a Home Care LVN (PACE) and help shape the future of healthcare where you'll be an integral part of our PACE‐ Clinic team, helping to strive for excellence while we serve our member health with dignity, respecting the value and needs of each of our members through collaboration with our providers, community partners and local stakeholders. This position has been approved to be Full Office.
- If telework is approved, you are required to work within the State of California only and if Partial Telework, also come in to the Main Office in Orange, CA, at least two (2) days per week minimum.
Under the direct supervision of the Home Care Supervisor, the LVN for the Program for All‐Inclusive Care for the Elderly (PACE) for home care will be responsible for the development and implementation of home care services for program participants, including the coordination of durable medical equipment (DME) and community‐based care. Together, we are building a stronger, more equitable health system.
Your Contributions To the Team:
- 95% ‐ Home Care Services

- Assists the team in carrying out department responsibilities and collaborates with others to support short‐ and long‐term goals/priorities for the department.
- Evaluates the home environment and identifies the different areas where the participant needs home care staff to assist.
- Works with the Interdisciplinary Team (IDT) to develop specific plans of care to address the participant's home care needs.
- Receives reports from home care staff and communicates participant changes to the IDT in a timely manner.
- Schedules and coordinates home care staff for in‐home care delivery and the implementation of all personal care services to ensure that quality services are provided to meet participant needs.
- Coordinates all DME provisions in the home setting and collaborates with the IDT on DME authorization requests.
- Completes initial and annual evaluations of the home environment for coordination of care and completion of participants care plan.
- Documents participant changes appropriately in the medical records and communicates participant changes to the IDT.
- Maintains current knowledge of standards and best practices for home care.
- Takes the lead by coordinating and integrating services within the department, with other departments and with the IDT's primary functions.

Do You Have What the Role Requires?
- High school diploma or equivalent PLUS 3 years of nursing experience working with the frail or elderly population within the last 3 years in a health care or managed care setting required; an equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.
- CPR and First‐Aid Certification required or must be obtained within six (6) months of the date of hire.
- Valid driver's license and vehicle or other approved means of transportation, an acceptable driving record and current auto insurance will be required for work away from the primary office 40% of the time or more.

ESSENTIAL FUNCTIONS

Summary
Seeking an experienced Deputy Program Manager for Construction to support large-scale design-build projects. This role is responsible for overseeing program execution, coordinating construction activities, and ensuring projects are delivered on schedule, within budget, and aligned with overall program objectives.
Key Responsibilities
- Lead program-level coordination of construction activities across multiple projects.
- Oversee implementation of program policies, procedures, and standards.
- Serve as primary liaison between internal teams, clients, and external stakeholders.
- Manage contract scope, budget, staffing, and overall project performance.
- Lead multidisciplinary teams including project managers, inspectors, schedulers, estimators, and safety personnel.
- Coordinate closely with design, program controls, and stakeholder groups.

Qualifications
- Bachelor’s degree in Construction Management, Engineering, or related field.
- 15+ years of construction experience, including 5+ years in a leadership role.
- Proven experience delivering large-scale, complex construction projects (high-rise preferred).
- Strong understanding of construction cost control, scheduling, estimating, procurement, and engineering principles.
- Ability to read and interpret contracts, drawings, specifications, and schedules.
- Strong leadership, communication, and stakeholder management skills.
- Proficiency with Microsoft Office and construction/project management software.
- Ability to perform in a fast-paced, team-oriented environment.
Preferred Experience
- Design-build project delivery experience.

REQUIRED QUALIFICATIONS:
Physician Assistant: NYS license and certification as a Physician Assistant. Bachelor's degree from an accredited school of Physician Assistants.
PREFERRED QUALIFICATIONS:
At least one year of clinical experience working as a Physician Assistant. Previous experience in cardiology and/or critical care.
BRIEF DESCRIPTION OF DUTIES:
This position serves as a healthcare professional who, under the direction and supervision of a licensed physician, will provide care to adult patients suffering from advanced heart failure in both the inpatient and outpatient settings. The advanced practice practitioner will perform medical examinations, make diagnoses, dispense medications, order standard laboratory and x-ray procedures, evaluate procedure results and refer abnormal findings to a physician. In addition, it's expected that this practitioner will maintain a current level of knowledge relative to professional practice and will meet/maintain the requirements necessary for licensure and certification.
Perform pre-procedure workups including history and physical, ordering relevant tests and coordinating services needed on procedure day and post procedure care
Implement patient-centered care plans and manage patients with acute or chronic heart failure.
Ensure patients receive optimal Guideline-Directed Medical Therapy (GDMT) and manage patient symptoms to reduce readmissions and hospitalizations.
Order and interpret diagnostic tests including echocardiograms and stress tests.
Educate patients about self-care including weight management, diet and fluid intake.
Assist with patient callbacks, prescription renewals and other relevant clinical duties.
