Job Summary and QualificationsAs a Registered Nurse, you will be responsible for delivering high-quality, patient-centered care in line with the requirements of the department and the standards of practice for the relevant state and specialty. Collaborating with medical providers and the care team, you will provide personalized, comprehensive, and compassionate care, following established nursing models such as "Assess, Perform, Teach, and Manage." You will also act as an advocate for patients, families, and caregivers, embodying the organization's vision, mission, and values to ensure an outstanding patient experience and positive clinical outcomes.
What you will do in this role:
- Assess the patient's condition during admission and each scheduled shift, promptly identifying and reporting any changes in patient status.
- Perform procedures, monitoring, or other functions as ordered by the medical provider(s), and ensure thorough and timely documentation of care administration in the patient's medical record.ย
- Administer prescribed medications, monitor the patient for therapeutic response, and take appropriate action in the event of an unintended response to the medication.
- Provide exceptional care by responding promptly to patient requests, proactively anticipating patient needs, and resolving them.
- Educate patients, families, and caregivers about the patient's medical condition, treatment plan, medications, possible side effects, and follow-up measures, ensuring complete understanding by translating complex medical terminology.
- Required Education: Bachelor of Science in Nursing or enroll in a BSN or advanced degree program and obtain a BSN within 5 years of hire date into RN position. Grandfathering will be accepted between departments and Mission Health entities. Exemptions, or extensions, may be approved by the facility CNO and reported out to the CNO Council members. Northwest Family Medicine - St. Maries Clinic is seeking a compassionate, patient-focused RN to join the team!
Under the supervision of the Practice Supervisor, the Registered Nurse directs and/or provides professional nursing care for an assigned group of patients across the age continuum. Utilizes the nursing process of assessment, planning, implementation, and evaluation to direct or provide nursing care. Responsible for delivering patient care services in a Family Medicine setting. This position will focus on assessing, identifying, and solving nursing care and pre-operative problems during the pre-operative phase. This position will assist with emergency procedures, providing medications, and initiating intravenous fluid replacement. Demonstrates knowledge of equipment, instruments, best nursing practices, standards, and protocols. The Registered Nurse will need to project a professional demeanor and appearance while maintaining the confidentiality of the Hospital, patients, and employees as appropriate. Able to work under pressure and in situations that demand patience, tact, stamina, and endurance. Other duties as assigned.
Essential Functions and Qualifications:ย
- Operates all required equipment and machinery accurately and safely.
- Sets priorities to facilitate the timely and efficient flow of patents throughout the day.
- Conducts an individualized patient assessment, prioritizing the data collection based on the patientโs immediate condition or needs within timeframe specified by client facilityโs policies, procedures, or protocols.
- Start and monitor IVโs, administer pre-operative medications, perform pre-operative instruction, and explain patient's rights and procedures.
- Develops plan of care that is individualized for the patient reflecting collaboration with other members of the healthcare team.
- Assisting with emergency procedures, providing medications, and initiating intravenous fluid replacement.
- Performing physical assessments, taking vital signs, and documenting findings in the electronic health record (EHR).
- Collecting a thorough medical history from patients and family members upon admission.
- Initiates emergency resuscitative measures according to resuscitation protocols.
- Maintains confidentiality in matters related to patient, family, and facility staff.
- Provides care in a non-judgmental, non-discriminatory manner that is sensitive to patient and familyโs diversity, preserving their autonomy, dignity and rights.
- Maintains current competency in nursing specialty.
- Employees are expected to comply with all regulatory requirements, including Joint Commission Standards.
- Is familiar with organization, department, and job specific Environment of Care areas, including Life Safety, Hazardous Materials Communications, Emergency Preparedness, Infection Control and Medical Equipment Failure.

Responsibilities
Rooming patients: Collecting pertinent information regarding the patient's chief complaint, family history, social history, surgical history, and medication reconciliation upon arrival for a patient visit. Telephone Interactions: As directed, completing necessary telephone communications with patients, families, pharmacies, and/or other health care facilities. Gathering information and maintaining patient information via telephone per department specific processes and policies. Administrative tasks including, but not limited to, patient check in or check out, prior authorizations, scheduling appointments, verification of benefits, and coordination of referrals and testing. Office procedures as directed and per department specific competencies. May perform procedures such as ear wash, wound care, bladder scanning, suture or staple removal, reading results of negative TB skin tests or steering patient to the care of a department nurse when TB screening appears to have indications of a positive read, and other diagnostic testing at the point of care. Provide patient with written instructions regarding procedures, medications, or follow up instructions at the direction of the provider. CMA staff may retrieve and provide condition specific education from the internet, internal website, or printed materials per the provider directions and orders. Administer medications per the provider order. Exceptions - biologic or Schedule II, III medications. Provide assistance with prescription medication refills per department protocols. Call medications into pharmacies, notify patients of refills, and documents actions within the EMR per provider orders. Maintain department specific competencies related to point of care testing, equipment handling, and specialty training. Update and maintain different registries within the department. The compensation range for this position is $18.33per hour - $29.69per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The PCP will be required to demonstrate the ability to function both independently and in collaboration with other health care professionals. The PCP will work closely with the applicable managers and medical directors to ensure compliance with guidelines along with participating in risk and quality management programs, clinical meetings and other meetings as required that promote patient health and company goals.
The PCP will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
- The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
- The PCP will take full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, though will still stay involved in, and be responsible for, the detailed care of the patient.
- It is expected that the PCP will engage with the hospitalist whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed or not).
- The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
- The PCP leads their care team consisting of care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office.
- For patients that are unable to come to the office-in hospital, SNF, LTC or homebound, PCP will engage with the transitional care team and others including case managers, acute and transitional-care physicians, and other resources that may be available depending on the market.
- PCP will have an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
- Performs other duties as assigned and modified at manager's discretion.

200 Medical-Surgical is a 30-bed unit that cares for patients admitted from the Emergency Department, Post-Anesthesia Care Unit, Critical Care Units, and physician offices. The team serves a broad age range, from young adults to patients over 100 โ offering diverse and valuable clinical experiences.
The unit specializes in medical, surgical, and gynecologic/oncology care. Common procedures include hysterectomies, mastectomies, bowel resections, and various oncology surgeries. Chemotherapy is occasionally administered, and patients mayrequiretelemetry monitoring or oxygen support.

Position Details: The clinical specialist will join an on-site faculty member to provide direct patient care in the Parkview Womenโs and Childrenโs Hospital.
In preparation for pediatric service line growth and changes, continued pharmacy expertise is needed in this area.
Currently this location is undergoing many facility updates (room renewal, expansion, and technology updates) along with an expansion of care for pediatric gastroenterology, endocrinology, cardiology, and hematology/oncology specialist services.

As part of an established pediatric pharmacy program, this specialist will benefit from close relationships with physicians and other care providers in both the inpatient and outpatient pediatric settings.

This role will have a targeted role in the level III neonatal intensive care unit with additional coverage of other pediatrics areas for weekend pediatrics coverage.
