Job Summary

We are seeking an Advanced Practice Provider (APP) for the Duke Department of Obstetrics and GynecologyDuke Women's Health Cary in Cary, NC and Duke Women's Health Holly Springs in Holly Springs, NC clinic locations. Job Duties & Responsibilities

Obstetric and Postpartum Care

Will provide obstetric care to prenatal patients from the new obstetric visit throughout pregnancy and including the postpartum visit as follows:

• Performance of the new obstetric exam with identification of problems complicating pregnancy or pre-existent to pregnancy.
• Initiation of plan of care/diagnostic testing/interpretation of labs/treatment plan based on individual patient medical/surgical history.
• Patient education relating to pregnancy, childbirth and postpartum care.
• Performance of FHR assessment, NST interpretation, pelvic exams for antepartum and labor assessment and triage patients.

Gynecological Care

Will be responsible for management of gynecological patients for routine care and/or treatment of common gynecological problems including but not limited to:

• All methods of family planning and counseling for reproductive life planning.
• Routine Well Woman Exams and Pap Smears.
• Screening and treatment for sexually transmitted infections.
• Treatment for symptoms of Menopause/perimenopause.

JOB SUMMARY:

Collaboratively coordinates and provides care of the hospice patient population. Assesses staff educational needs related to the provision of clinical care services. • Provide direct patient care and may perform medical acts as a nurse practitioner under the supervision of the supervising physician in the home, community-based care setting, or hospice inpatient care facility or partner facilities. The supervising physician will be available by pager, mobile phone, or other electronic means.
• Assist in the coordination and development of the care plan collaboratively with the clinical staff, patient, family, physician and medical director, and establishes mutual goals.
• Assess and provide patient/family teaching based on patient and caregiver's needs.
• Participate in on-call rotation for medical coverage of hospice.
• Participate in weekly interdisciplinary patient care conferences. Establishes, reviews, updates, and summarizes care plans in keeping with Medicare CoPs and Licensure requirements.
• Partners with DHCH to identify learning needs of DHCH Staff and provides appropriate education in collaboration with the Directors of Patient Services/Hospice/Home Health/Infusion and the Clinical Nurse Educators.
• Participate in community educational projects.
• Participate in the performance Improvement Program of DHCH.
• Perform complex diagnostic and therapeutic procedures in diagnosing medical and surgical problems and prescribing necessary treatment and services for quality patient care; record normal and abnormal data and tentative conclusions for review by the physician.
• Coordinate and schedule laboratory studies and diagnostic procedures; explain the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
• Monitor patient's status, maintain a record of the patient's condition and consult with supervising physician when patient's response does not meet anticipated and/or predetermined criteria.

- Employed practice with Michigan State University teaching affiliation
- Expectations: 80% outpatient and 20% inpatient mix.

Target patient load averages 12-16/day in the clinic
