Position Overview:

The medical assistant at NSIPA plays a pivotal role in advancing member health outcomes and quality performance across the IPA’s provider network operating at the intersection of clinical support, care coordination, and community health. This hybrid role is focused on proactive member engagement, closure of clinical care gaps, coordination of health-related social needs (HRSN) services under the Medicaid 1115 waiver, and identification of non-utilizing members who have disengaged from the healthcare system. The MA serves as a critical link between members, providers, and the broader care team to ensure equitable access to preventive primary care services.

 

Roles & Responsibilities:

Care Gap Closure

·      Identify members with open quality care gaps using health plan reports, registry data, and EHR dashboards.

·      Proactively contact members via phone, text, mail or in person to close care gaps including preventative screenings, lab work, immunizations, and chronic condition management follow-ups.

·      Coordinate and schedule needed services in collaboration with the care team and provider offices.

·      Document all care gap closure activities accurately and in a timely manner within the EHR and care management platforms.

Annual Wellness Visit (AWV)

·      Identify members due for annual wellness visits through care gap files or health plan performance reports.

·      Facilitate AWV workflows with providers, ensuring screenings, referrals, and follow-up orders are completed and documented.

·      Conduct targeted outreach to members to schedule and confirm AWV appointments.

Outreach to Non-Utilizing Members

·      Identify members who have not sought medical services within a defined period (non-utilizers).

·      Design and execute targeted outreach campaigns to reengage non-utilizing members and connect them to primary care and preventive services.

·      Partner with community health workers and care navigators to conduct field-based outreach for members who are difficult to reach through standard channels.

·      Document all outreach attempts and outcomes, escalating complex cases to care management teams as appropriate.

1115 Waiver & HRSN Services Coordination

·      Identify members’ Health‑Related Social Needs (HRSN) using validated screening tools and document findings in the HER.

·      Transfer members with identified HRSN needs to the 1115 team or to one of the screeners/navigators on the 1115 team for further assessment and support.

·      Collaborate with the 1115 team, social workers, and community health workers to ensure members receive seamless navigation and timely access to services.

Provider Practice Assistance

·      Assist provider with intake responsibilities for members under NSIPA.

·      Ensure proper documentation and coding standards are being followed.

·      Address VBC payer performance opportunities and develop action plan to improve performance.

 

Skills & Competencies:

·      Ability to engage diverse member populations with empathy and cultural competence.

 

Required Qualifications:

Summary
Performs comprehensive subjective and objective initial and follow up assessments of patients status that includes physical psychosocial and environmental parameters and formulates an individualized plan of care according to physician orders that incorporates the analysis of assessment data and current scientific findings. Performs skilled treatments and interventions and provides patient education aimed at achieving patient goals/outcomes.

Job Duties

  • Responsible for determining home health as the appropriate level of care for the patient as well as skilled need for services ordered based on home health Conditions of Participation.
  • Performs comprehensive, subjective, and objective initial and ongoing assessments of patient status that includes physical, psychosocial and environmental parameters and formulates an individualized plan of care according to physician orders that incorporates analysis of assessment data and current scientific findings.
  • Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient family centered approach.
  • Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning. Promotes patient/caregiver autonomy. Evaluates effectiveness of teaching and modifies education based on patient/caregiver needs/goals.


Minimum Qualifications

  • Bachelor’s Degree Nursing or
  • Associate’s Degree Nursing with 2 years experience in the home care setting
  • 1 year recent experience providing direct care in a healthcare setting and
  • 1 year Home Health experience in OASIS data set collection
  • Knowledge of patient education techniques and principles
  • Ability to take initiative and function independently without direct supervision and as a member of the care team
  • Ability to travel to patient care assignments
  • Knowledge of Home Health Conditions of Participation
  • Knowledge of and skill in completing OASIS data set collection with a high level of accuracy
  • American Heart Association Basic Life Support - State of Pennsylvania Upon Hire
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire
  • DL - Driver's License_PA - State of Pennsylvania Upon Hire


Preferred Qualifications

  • AMB-BC- Ambulatory Care Nursing ANCC - State of Pennsylvania within 3 Years


Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Summary
Ensures and/or provides patient care in accordance with the RN job description. Assumes 24-hour accountability for the direction and management of patient care within assigned area. Manages material, financial, and human resources within the assigned unit.

Job Duties

  • Oversees administrative and operational issues for the designated care delivery team.
  • Functions as a department team leader to support unit and hospital goals and objectives.
  • Uses effective and appropriate communication styles with patients, significant others, visitors, staff, and professional colleagues.
  • Supports the Culture of Safety by following established policies and procedures which include identification and timely patient safety event reporting.
  • Ensures staffing to meet patient care needs and operational guidelines.
  • Fosters accountability of staff for adherence to policies/procedures/professional practice.
  • Participates in coaching, counseling, and mentoring of unit personnel.
  • Supports the Professional Practice Model and participates in efforts related to the key components.
  • Assumes unit, clinical, and administrative responsibilities in the absence of the Director and under the supervision of the Patient Care Services management staff.
  • Ensures and/or provides patient care in accordance with all responsibilities in the RN job description.
  • Ensures completion of the unit's schedule in a timely and equitable manner.
  • Utilizes evidence based research findings within patient care delivery, communicates research findings to co-workers and/or the professional community, attends research continuing education offerings, assists with data collections, and serves as a participant in ongoing research studies.
  • Provides clinical, consultative, technical, or informational services as needed to staff, other disciplines, patients, and/or families.


Minimum Qualifications

  • Bachelor’s Degree Nursing
  • 3 years Registered Nurse in an acute care hospital setting.
  • Ability to multi-task and prioritize work.
  • Proficient in clinical practice protocols for medical procedures and patient care.
  • Skilled in monitoring/assessing the performance of self, other individuals, or organizations to make improvements or take corrective action.
