We are seeking an Epic MyChart Certified IT Analyst Senior to support a growing Digital Consumer Experience team focused on Epic MyChart and patient-facing technologies. This role is ideal for someone who combines strong healthcare application support experience with Epic MyChart expertise, integrations, and digital patient engagement tools.
Youβll work in a collaborative Agile environment supporting and enhancing applications that directly impact the patient experience, including MyChart, telehealth workflows, patient messaging, and digital care pathways.

Role Responsibilities
Working within Agile and other IT frameworks, the IT Analyst Senior will:

This is a technical position providing analysis, configuration, maintenance, modification and monitoring of all Epic CachΓ© and SQL databases. The ideal individual understands and supports best practices and requirements for data storage, database architecture, tuning, maintenance and reporting, ensuring hardware and software compatibility and optimization. This individual assists in and supports personnel in determining databases are available, backed up regularly, and can be restored. Participates in, and evaluates installations (server and database binaries), tests, and implementations of system hardware, clustering technologies, software, manufacturerβs maintenance releases and utilities. Under direction of the Physician Assistant Trainer and Supervisor and Clinic Medical Director, the Physician Assistant (PA) will provide dermatologic care and education to patients with a variety of skin conditions. The PA will perform physical exams, diagnose skin conditions and will develop treatment plans, provide health counseling, prescribe medication, and may perform minor surgery, including, but not limited to, removal of skin tags, cysts, benign tumors, actinic keratosis, malignant skin tumors, laser surgery, laser treatment procedures (for example, excimer laser for psoriasis). The PA will perform biopsies and simple to complex excisions, and may assist in major medical procedures such a Mohs, laser or cosmetic procedures. The PA will see both new and follow-up patients as assigned by the Clinic Medical Director and Physician Assistant Trainer and Supervisor. Other duties include ordering medications, lab work, imaging, and other requirements of appropriate dermatologic care, reviewing progress of patients, attending and participating in staff conferences to discuss the surgical and medical conditions of patients. He/she may also be asked to draft referral letters to outside physicians and request for insurance companies for pre-authorization for certain medications and prior to surgery. The PA will also return patient phone calls and MyChart messages as needed or as assigned by the Clinic Medical Director and Physician Assistant Trainer and Supervisor. He/she may triage patient phone calls and electronic messages in the EHR, renew or fill prescriptions as needed for both his/her own patients as well as for other PAs and physicians in the practice. May also perform cosmetic procedures (such as, but not limited to, laser hair reduction and Fraxel laser, fillers, chemical peels, etc.) as well as phototherapy treatment, if the need arises. Duties are to be performed in consultation with the Physician Assistant Trainer and Supervisor.

- Valid Physician Assistant license to practice under the Physician Assistant Act in the State of California.

The Department of Dermatology is an academic/clinical department consisting of clinical and research faculty, volunteer clinical faculty, residents and clinical fellows, research personnel and administrative staff personnel. The department's goals and objectives are to achieve excellence in patient care, teaching, research and public service. Patient care must receive the highest priority with the best possible service in all areas of dermatologic care. In addition, the teaching of residents, medical students and postgraduate continuing medical education physicians must be facilitated. PhDs and MDs are joined in mutual research efforts to seek new knowledge in the treatment and prevention of skin disease and other cutting-edge research. Job Duties The Childbirth Center Discharge Coordinator will participate in boost rounds with the pediatric, obstetric, lactation, care management and neonatology teams and facilitate discharge needs for all mother/infant dyads on the Childbirth unit.
This position is grant-funded position.
This position is 40 hours weekly; Days.
A minimum of 2 years RN experience is required Job Duties: Ensure referrals are scheduled including early intervention, subspecialist appointments, developmental hip clinic, radiology, home nursing, maternal BP and wound checks, Neonatal Growth and Development, etc.
Coordinate follow up appointments, when plausible, for mother and infant inclusive of family input.
Ensure timely PCP follow up for infants within AAP recommend timeframe.
Facilitate current Proven Care target of 11AM priority discharges from the Childbirth Center.
Function as a liaison with CYS or POSC (Plan of Safe Care) coordinator in arranging meetings with families and providers.
Send PCP a copy of the POSC, which may not be finalized at time of discharge.
Provide discharge education for caregivers (DOH Shaken Baby video, Safe Sleep education, etc).
Notify RN or provider if caregivers have questions about education.
Arrange new discharge equipment, when needed (car seats, car beds, cribs, phototherapy blankets, breast pump) Screen for maternal risk of postpartum depression.
Notify the provider if mother is at risk.
Coordinate inventory of phototherapy blankets, notify GHIS of missing blankets and/or when blankets are issued to patients.
Ensure WIC scripts have been completed.
Ensure all paperwork has been completed for patients and caregivers such as work notes, homebound education, FMLA paperwork.
Enroll newborns (and women if not already) in MyChart.
This will facilitate the use of that platform to reinforce education.
Work with providers to ensure new prescriptions for discharge are complete.
Arrange (discharge)transportation, when necessary, as well as to follow up appointments.
Ensure Discharge Summaries are completed and faxed to PCP, if non-Geisinger provider.
Liaison with outpatient social work for families with ongoing needs that a "warm hand-off" has occurred.
Arrange Ronald McDonald temporary housing for parent if infant needs to stay inpatient longer than mother.

The Patient Access Representative is responsible for the entire scope of the registration process for patients seen at St.
