PRIMARY JOB DUTIES

The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.

He/she will provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care
He/she may work in a community, long-term care environment, or hospital
He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team
In all settings, he/she will provide in-person visits or telehealth visits, as appropriate. Care will consistently comport with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation
In the role as hospice NP, he/she will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
He/she will also contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
He/she will also help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage or for when young adolescents are incorporated into adult hospice teams
In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
Other activities related to quality improvement projects may also be required, as appropriate

Essential Work Functions:
  • Assess patient needs and develop individualized hospice care plans in collaboration with physicians and care teams
  • Administer pain management interventions, medications, and treatments to ensure patient comfort
  • Educate and support families and caregivers, helping them understand symptom management and end-of-life care
  • Monitor changes in patient condition, adjusting interventions to maintain quality of life
  • Coordinate care with hospice physicians, social workers, chaplains, and other team members to provide holistic support
  • Document all care provided, symptom changes, and family interactions accurately and in compliance with hospice regulations
  • Ensure adherence to hospice best practices, safety protocols, and regulatory requirements
  • Offer emotional and psychological support to both patients and their loved ones during the end-of-life journey
  • Communicate regularly with the hospice care team to ensure continuous, compassionate care
  • Complete charting and documentation requirements while meeting visit and productivity goals
  • Adhere to infection control, safety, and healthcare regulations

  • The ability to read, write, and communicate in the English language
  • Ability to read/comprehend written instructions, follow verbal instructions, and proficiency in PC skills
  • Physical Abilities - Must be able to remain in a stationary position, move about, move equipment (50-100lbs), push, pull, and bend
  • Interpersonal Skills - Must be able to work effectively with a variety of personnel (professional and ancillary) to present a positive attitude and professionalism
  • Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment
  • Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail
  • Sensory - Must possess visual acuity and ability to effectively communicate
