Supports the Hospice Associate Director of Patient Services by providing leadership and mentoring guidance to the nursing staff on a day-to-day basis to ensure the provision of Hospice and Palliative Care services are consistent with the philosophy, goals, and policies of the program and MJHS. Schedules staff assignments in conjunction with the Assoc. DPS in accordance to the (NHPCO) and MJH staffing ratios. Empowers RN Case Managers through mentoring excellence in pain and symptom management along with focusing on patient and family goals of care. Monitors and evaluates the quality and completion of nursing documentation according to the documentation policy and procedure and state and federal regulations. Makes community presentations in conjunction with Admissions/Outreach Departments.
- Graduate from an accredited School of Nursing, BSN or BS degree preferred.
- Minimum of five years of Hospice nursing experience.
- Supervisory or leadership experience preferred.
- New York State RN licensure required.
- National Board Certification in Hospice and Palliative Care (CHPN) preferred or obtains certification within one year of hire.
- Word, Outlook, and Windows experience required.

- Baccalaureate Nursing Degree from an NLN-Accredited School of Nursing

Responds to changes in members’ condition/sentinel events (i.e. level of care changes, changes in family support, changes in housing, etc.) by revising the PCSP and communicating changes with interdisciplinary care team and PCP.
Responds to requests for service within Medicare/Medicaid regulated time frames utilizing appropriate correspondence as per DOH/CMS regulations and departmental workflows.
Participates in and presents cases at the Interdisciplinary Care Team meetings to discuss any changes in member’s condition/sentinel events (i.e. level of care changes, changes in family support, changes in housing, etc.) identified during assessment process.
Collaborates with interdisciplinary care team members, such as Primary Care Physician, Social Worker, Specialists, Hospice/Palliative team, and Chief Medical Director when complex social/medical focus areas are identified during the assessment/care planning process as per unit workflows.
Educates members on Advanced Directives/Health Care Proxy/Power Of Attorney (POA) and helps to facilitate the completion of such documentation.
Bachelor’s degree social work or related field required, MSW or LMSW preferred.
Minimum three to five years clinical social experience in long term care, managed care, home care, geriatrics or hospice preferred.
Prior experience in an interdisciplinary service delivery environment preferred.
Working knowledge of Medicare and Medicaid regulations preferred.

Details and requirements for this opportunity:

- Practice Setting: Inpatient/Outpatient, rotation assignments at two locations
- Types of Cases: rotation assignments to inpatient consult service, echo (echocardiogram) /TEE (transesophageal), EKG (electrocardiogram) reading, outpatient clinic and possible CCU (critical care unit) rotation based on experience. Rotations are 1 week each. If on Critical Care Unit, rotation would be a 6-day week.

- Carry and use a firearm. Maintaining firearm proficiency is mandatory.

401(k) Plan Administrator
The 401(k) Plan Administrator is an office-based leadership role responsible for the strategic oversight, governance, and administration of the Company’s 401(k) Plan, supporting approximately 20,000 employees nationwide. This hands-on leader ensures the effective management, regulatory compliance, and operational integrity of the plan while delivering a high level of service to participants and stakeholders.
Key Responsibilities
The Administrator oversees all plan administration and customer service functions, including but not limited to:
- Ensuring the 401(k) Plan is operated in strict accordance with Plan Documents and maintaining all required plan documentation
- Ensuring compliance with applicable federal and state laws and regulations, including IRS, ERISA, and DOL requirements, and overseeing the timely and accurate completion of all required reporting and filings
- Overseeing annual compliance testing, government filings (including Form 5500), and required participant notices
- Documenting, maintaining, and continuously improving administrative procedures to enhance efficiency, accuracy, and internal controls
- Serving as the primary point of contact for plan vendors, recordkeepers, trustees, auditors, and external advisors
- Conducting regular data audits to analyze payroll and 401(k) plan data, including auditing payroll deductions for deferrals, loan repayments, eligibility status changes, and related transactions
- Approving hardship withdrawals and termination distributions in accordance with plan provisions
- Ensuring accurate eligibility tracking and vesting calculations for rehires and acquired employees
- Coordinating plan mergers and acquisitions, including due diligence, integration, and compliance alignment
- Partnering with the Benefits Administrator and Payroll teams to ensure seamless plan operations and accurate data flow
- Collaborating with the recordkeeper to design and implement robust employee communication and education strategies
The Administrator serves as a key fiduciary partner, ensuring the plan operates in the best interest of participants while aligning with company objectives and industry best practices.
Qualifications
- Minimum of ten (10) years of experience managing 401(k) plans, including compliance, regulatory filings, and audit oversight (IRS/ERISA/DOL)
- Strong knowledge of retirement plan governance and fiduciary responsibilities
- Advanced proficiency in spreadsheet and data analysis tools; ability to analyze and audit large data sets
- Experience working with HRIS systems and/or external payroll providers
- Experience supporting plan mergers and acquisitions preferred
- Advanced degree preferred
- SHRM-SCP/SHRM-CP, SPHR/PHR, or other relevant industry certification preferred
