This position is an essential member of the nursing clinical team.

 

The RN certified in wound, ostomy and continence provides expert clinical care, consultation/care coordination, education, quality improvement, leadership and research to the organization in relation to patients/ families and colleagues with wound, skin, ostomy and continence needs. This position is responsible for the assessment, planning, implementation, and evaluation and care of wounds (acute and chronic), fistulae, pressure ulcers, skin breakdown, ostomy care and incontinence.

 

Specifically, the WOCN RN Acute Performs the following:

 

·        Coordinates care for populations of patients, identifies trends, develops evidence-based protocols/treatments and refines protocols based on emerging evidence, products, therapies and outcomes.

·        Develops and implements and refines educational programs for patients, families, colleagues and community partners.

·        Provides formal and informal consultation and serves as resource for patients, healthcare team and leadership.

·        Promotes the profession of nursing and professional practice.

·        Represents the organization in a positive and professional manner.

·        Advocates for the rights, ethical standards, and safety of patients and healthcare team.

·        Is visible, accessible and committed to communicating effectively with staff.

 

Essential Functions/Responsibilities:  

 

Direct Patient Care

Provides expert consultation and/or hands-on care for individual patients with acute or chronic wounds, ostomy or continence issues.

•                  Incorporates specialized evidence-based knowledge and skills to provide excellence in assessment, therapeutic interventions, prevention and health maintenance for patients with select disorders of the integumentary, gastrointestinal and genitourinary systems.

•                  Differentiates characteristics of wound, ostomy, continence clinical presentations and attributes accurate etiology in development of treatment plans.

•                  Serves as a consultant to provide potential solutions for complex clinical cases to improve patient care and outcomes.

•                  Establishes and coordinates multidisciplinary and comprehensive management plans.

•                  Conducts ongoing re-evaluation and modifies the strategies and interventions as needed.

•                  Notifies supervising clinician and Team when the wound deteriorates or fails to progress.

 

Collaborates with healthcare team to provide interdisciplinary referrals to complement the treatment plan.

Care Coordination

Coordinates care for populations of patients with wounds, ostomy and continence conditions.

•                  Collaborates with Healthcare Teams and leadership to develop best practice cross-setting care coordination using standardized prevention and treatment protocols: Coordination of care involves consideration of care trajectory across multiple settings (pre-op, acute and post-acute).

 

Education

•                  Develops and implements educational programs and protocols for patients, staff and other health care providers that address the prevention and management of wound ostomy and continence health conditions.

•                  Arranges learning opportunities for RN Skin Champion.

•        Provides orientation for nurses and other health care team members.

 

Quality Improvement / Safety

•                  Collects and interprets wound, ostomy and continence-related data for continuous quality improvement, safety initiatives, surveillance and regulatory compliance.

•                  Leads or provides guidance in the leadership of initiatives to reduce harm and improve outcomes for patients with wounds, ostomy and continence conditions.

•                  Participates/ leads research and quality improvement projects to investigate or validate interventions and workflow related to wound ostomy continence care.



Qualifications

Education

•                  Baccalaureate degree in Nursing. Master’s degree in nursing or higher preferred.

Experience

•                  Minimum of 2 Years with a minimum of 2000 hours of progressive wound and skill experience in clinical practice.

•                  5 or more years of active clinical nursing experience preferred.

•                  Experience in EPIC EHR preferred.

Licensure, Certification, Registration

•                  Licensed Registered Nurse from the CT DPH.

•                  Enrollment in a WOCN Society Accredited Educational Program within one year of hire.

•                  Completion of board certification in wound/ ostomy/continence through WOCNCB within a defined timeline following enrollment in educational program.

The Assistant Nursing manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. Qualifications

Education

Job Summary:

Functioning within the framework for professional nursing practice, the Invasive Cardiology Manager, responsible for the assigned units Electrophysiology Labs, Cath Labs, and Cardiac Procedural Holding, supports and assists the Director of Clinical Operations, Heart and Vascular Institute (HVI).

This position has 24-hour accountability for managing resources (human, material and financial), to ensure quality, cost-effective delivery of patient services for specific patient populations, in a defined area across the continuum. This is accomplished by direct action, delegation, coaching and support of unit-based shared governance councils.

Reports to Director of Clinical Operations, Heart and Vascular Institute (HVI).


Responsibilities:

·        Ensures compliance with regulatory and accreditation requirements.

·        Conducts hiring, training, directing, development and evaluation of staff.

·        Identifies and resolves issues affecting the delivery of patient care services for the assigned unit(s).

·        Develops and implements policies and procedures to ensure efficient and effective delivery of health services in a unit.

·        Monitors and adheres to budget. Approves or monitors expenditures, purchases and other actions to ensure compliance with budget guidelines.

·        Collaborate with the department Clinical Coordinators and Nursing Professional Development Department to develop programs designed to assist the staff nurse in the progression of novice to expert in the practice of nursing.

·        Systematically evaluates the quality and effectiveness of nursing practice and services.

·        Evaluates factors related to safety, outcomes, effectiveness, cost and social impact when developing and implementing practice innovations.

·        Provides leadership and direction to nursing unit staff and has 24-hour accountability ensuring all quality, financial, and service benchmarks as well as compliance with environmental care standards are met.

·        Other Duties as assigned.

Licensure / Certification / Registration:

Required Credential(s):

Denial Resolution

·       Conduct a thorough review of medical records, coding and clinical documentation to validate or appeal payer denials.

·       Prepare, document and submit appeals for DRG denials, ensuring appeals are well-supported with clinical evidence, coding guidelines, and regulatory requirements.

·       Work closely with the Clinical Documentation Improvement (CDI) and Coding teams to ensure accurate DRG assignment and enhance documentation practices that support appropriate reimbursement.

·       Ensure that all DRG denial and appeal activities comply with federal, state, and payer-specific regulations, including maintaining knowledge of ICD-10-CM/PCS coding guidelines and CMS regulations.

·       Maintain accurate records of denial appeals in the designated software, including the status of appeals, timelines, and outcomes.

·       Monitor appeal deadlines to ensure timely submission of all required documentation and compliance with payer appeal windows.

·       Contribute to revenue protection efforts by successfully overturning inappropriate denials and reducing the financial impact of DRG downgrades.

·       Meet departmental performance goals, including Key Performance Indicators (KPIs) related to denial turnaround times, appeal success rates, and denial reduction targets.

·       Analyze denial patterns to identify root causes and collaborate on preventive strategies.

·       Proactively address discrepancies between payer policies, regulatory standards and internal processes to prevent future denials.

Position Description:


The OR Nurse functions as Circulator/Scrub Nurse in the Operating Room. Organizes and administers individualized, professional patient care to patients within the Operating Room. Provides nursing care in accordance with the objectives, policies, procedures and protocols of the hospital and AORN, ASPAN and SGNA standards. Participates in patient teaching; incorporates the needs and behavior of the patient’s specific age/cultural groups in planning and providing care. Participates in quality improvement activities.


The Neuro Operating Room (OR) team provides highly specialized care for patients undergoing neuro and spine surgeries. Conditions treated include brain tumors, vascular malformations, essential tremors, degenerative spine disease, chronic back pain, neuro/spine trauma, spinal cord injuries, infections, hydrocephalus, and complications from acute stroke. Nurses play a critical role in delivering this specialized care, ensuring patient safety and well-being throughout these complex procedures.

Job Summary 
Heart Failure Transitional Care Nurse Coordinators participates in an expanded nurse role as an integral member of the Heart Failure Team. They are at the forefront of the Heart Failure Disease Management Program and work in a collaborative environment with strong patient-centered interdisciplinary teams, focused on a patient-centered care model in a richly diverse work space. The nurse coordinator must possess our institutional values of integrity, kindness, safety, nurse excellence, leadership, professionalism, and loyalty while providing high quality safe care. Under the direction and general supervision of the Heart Failure Program Manager, the registered nurse coordinator uses high-level critical thinking skills and expertise in the safe administration of various heart failure intravenous medications aimed at decongestion while also balancing and providing the framework of a robust comprehensive advanced heart failure management program. The Heart Failure Transitional Care Nurse Coordinator utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. With a goal of reducing readmissions and improving quality of life for patients, coordinators use expert knowledge and the nursing process to develop a plan of care which includes standards of practice to meet the physical, emotional, spiritual, cultural, and educational needs of the patient and family. The coordinator provides specialty and individualized care, implementing interventions including heart failure education, medication education, dietary education and on-going counseling, promotion of self-care, and adherence to a therapy plan. 

Key Areas of Responsibility 
• Independently obtains and performs assessments while safely triaging and guiding a patient’s clinical course. Presents patient history and clinical presentation to a Provider. Establishes, implements, and evaluates an individualized plan of care with timely and appropriate clinical advice and interventions. 
• Perform extensive review of medications and eliminates inadvertent inconsistencies across transitions in care. Communicates effectively with Providers to validate and verify medications and ensure medication safety and improved effective medication reconciliation and follow-up care. 
• Facilitates coordination of care across the continuum and collaborates with Advanced Practice Providers and Physicians to utilize therapy protocols. Performs IV and medication therapy, assessment, and evaluation of the patient 
• Participates in the overall program growth, referral base, outreach, protocol establishment and re-evaluation 
• Participates in research and present programmatic milestones and accomplishments in various venues 
• Leads heart failure support groups, educational sessions, and develops Heart Failure newsletters, and other activities/ materials as needed. 
• Coordinates the Heart Failure patient experience throughout the Hartford Healthcare continuum based on the individual needs of the patient. 

 



Qualifications

Education: 
Bachelor of Science Nursing Degree required 
Experience 
Minimum of 3 years’ acute care experience preferred. 
Minimum of 2-5 years’ heart failure experience preferred. 
Licensure, Certification, Registration

The Emergency Department at Hartford Hospital is one of the busiest EDs in Connecticut, treating patients of the highest acuity in the region.  Comprised of 108 licensed beds, with an additional 19-bed ED Observation Unit, HHED treats an average of 108,000 patients annually.  The ED works closely with the 7 Medicine inpatient units within Hartford Hospital to promote a collaborative continuum of care for our patients as they transition through their care within the hospital.  Our Medicine inpatient units care for a diverse patient population, allowing for RNs to gain a broad range of experience within the nursing field.


RN Bridge Program


A Medicine-to-ED Bridge Program has been established to provide nurses with potential future interest in working in an Emergency Department an opportunity to first build foundational nursing skills and knowledge within one of our Medicine units.  Upon completion of orientation on a Medicine unit, and successfully demonstrating competence in practice for the first 12 months, the RN will then begin a gradual transition to the ED setting with support of leadership from both the Medicine and ED teams.


Program components:



  • Creation of individualized development plan with managers of both Medicine unit and ED to track goals and progress
  • Collaborative coaching sessions with each RN and managers of both units at regular intervals through the program
  • Experienced RNs from both the Medicine unit and the ED assigned as mentors
  • Transitional orientation to care for inpatients in the Emergency Department
  • After completing each step of the bridge program, RNs will be evaluated for readiness to transition fully to the ED
