Job Summary:
The Connecticut Orthopedic Institute and Ayer Neuroscience Institute are highly dedicated to improving the lives of our patients. The Nurse NavigatorΒ serves as a patient advocate, liaison, and advisor/educator to guide patients throughout the continuum of care for their surgical experience beginning in the pre-operative phase up to a full year post-operatively to mitigate risks and promote positive outcomes. This position builds relationships with patients, families and the internal clinical team to improve patient outcomes, satisfaction and transitions through the continuum of care.

The Float Pool at Hartford Hospital, also known as the STAR Team, is comprised of experienced, highly skilled Nursing professionals. Registered Nurses on the STAR Team have a variety of skills, allowing them to transition from one specialty to another with confidence and ease. Our team is broken down into two groups, Med/Surg and Critical Care. The Med/Surg team floats between approximately 18 units (including the Step Down units) while our Critical Care RNs primarily float between the six ICUs at Hartford Hospital (MICU, SICU, Cardiac ICU, Neuro ICU and Cardiothoracic ICU). Our RNs rely on extensive nursing skills, strong communication and collaboration skills to assimilate seamlessly into different units and provide the highest levels of personalized, compassionate care to our patients.

CB3 is a medical-surgical unit, specializing in the care of liver and kidney transplant patients pre and post-surgery. The unit is one of the most diverse units in the organization caring for a diverse mix of complex medical and complex surgical patients.
Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Nurse Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff. Clinical Supervisor- Hospice
Supervises the program to ensure that a high quality, cost-effective service is delivered by the department and that is in keeping with Hartford HealthCare at Home's strategic plan as well as the Programβs goals and objectives. The Surgical-Trauma Intensive Care Unit is a 14-bed unit. As a Level 1 Trauma Center, the mainstream patient population includes motor vehicle trauma, orthopedic injuries, vascular surgical, abdominal surgical, liver and kidney transplant, and extensive esophagectomy patients. Under the supervision of the rounding Surgical-Trauma Intensivist, the unit has a team of Physician Assistants, Advanced Practice Registered Nurses, Registered Nurses and surgical house staff physicians consisting of interns and residents.
Job Summary
The Nursing Manager is a Registered Nurse (RN) who has responsibility for the supervision of patient care, clinical personnel, and unit operations. This position supervises a work group to ensure that a high quality, cost-effective patient and family centered care is provided in support of the hospital's strategic plan, and the function's goals and objectives.
This position works collaboratively with functional leadership and is responsible for the oversight of daily operations of the assigned department including off shift and weekend supervision of nursing and non-licensed staff. This position manages resources, quality improvement, payroll, creating and maintaining colleague schedules and monitors patient throughput to facilitate efficient clinical operations. In addition, this position is responsible for a safe and therapeutic unit environment, which includes working in the department as staff RN to assist as needed.

Job Description
Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. - Provides office-based nursing care in collaboration with provider

- Communicates with provider regarding patient needs, nursing assessments, and recommendations
- Takes direction and initiates actions (cross/additional training) that will allow seamless patient care
- Collects and documents current health status and medical history and assists in scheduling patient office visits
- Reviews patient records, including preoperative records, to ensure completeness and reviews outstanding issues with provider
- Demonstrates independent nursing actions based on assessment and problem identification
- Under direction of provider, calls in prescription medications to pharmacies and observes and documents patientβs response to pertinent medications
- Documents accurate and ongoing assessment of patientβs needs, problems, capabilities, limitations, progress toward goals, and response to care
- Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner
- Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy

Reporting to the Director of Clinical Services, this role ensures the accuracy, compliance, and quality of Outcome and Assessment Information Set (OASIS) assessments by reviewing for regulatory and policy adherence. The individual in this role provides clinical staff with guidance on documentation, utilization, and patient care decisions to support optimal clinical and operational outcomes. They review visit utilization for care appropriateness and collaborate with clinicians on necessary modifications, reporting any potential over or underutilization to leadership. The role requires timely completion of workflows in Homecare Homebase (HCHB) and flexibility to meet agency needs.
Collaboration with teams is key to identifying trends and developing action plans to address them. Participation in quality and performance improvement initiatives is expected, along with supporting the agencyβs compliance with data submission requirements for regulatory and payor standards.
JOB RESPONSIBILITIES
Reviews OASIS assessments for accuracy, completeness, and regulatory compliance, providing recommendations to ensure quality and reliability.
Guides staff on clinical, documentation, and utilization decisions to improve OASIS accuracy and care quality, recommending cost-effective corrections that support clinical and operational goals.
Reviews visit utilization for appropriate patient care, collaborates and consults clinicians on needed adjustments, and reports utilization concerns and financial risks to the clinical manager. Ensures timely completion of HCHB workflows and maintains a flexible schedule to support agency needs.
Collaborates to identify documentation trends, supports improvement planning, participates in quality initiatives, and helps ensure compliance and accurate data submissions across all settings.
Qualifications
Degree from an accredited educational program leading to licensure as a Registered Nurse (RN), Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP). Required
Bachelor's degree in nursing, master's degree in PT, OT, SLP preferredΒ
2 years as certified home health clinical management or OASIS reviewer
Homecare Homebase experience required
