The Home Therapy Registered Nurse, as qualified by federal and state regulations, provides patient training and ongoing support for all patients choosing a home dialysis modality.Β This position is accountable for providing quality care to all patients and is committed to delivering superior customer service in all internal and external interactions.Β Β
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.Β Other duties and tasks may be assigned.
- Knowledge of and comply with federal, state, local laws and regulations, including health care professionals practice act requirements as they pertain to home dialysis program and patient care requirements.
- Train patient (and/or care partner) in the practice of self-care Peritoneal Dialysis or Hemodialysis upon meeting federal and state regulations governing Registered Nurse qualifications.
- Conduct home visits to assess the patient's home environment per policy; initially and minimally annually thereafter.Β Routinely evaluate patient performance and assess the home environment more frequently as needed to improve care.
- Communicate results of patient assessment, reassessment, and ongoing monitoring to the physician, team members, and others as appropriate to the individual needs of the patient.
- Document all nursing services in the Electronic Medical Record including but not limited to training sessions, routine and non-routine in-person interactions, and phone conversations.Β Documentation should accurately reflect the patient status and nursing interventions and be written to ensure continuity of care.
- Participate in infection control monitoring, implementation, and recording as requested.
- Be familiar with emergency equipment and all emergency operational procedures.Β Communicate and regularly review Emergency Preparedness procedures with all home patients, including but not limited to emergency disconnection from dialysis equipment, what to do and who to contact if displaced from home.
- Ensure clinic and patient supply inventory and usage is appropriate for patient prescriptions and place orders in timely manner to avoid fees and penalties.
- Continually seek to improve patient and quality outcomes through tracking and trending of program and patient performance, use of critical thinking skills and root cause analysis to improve patient and quality outcomes.Β Assist in obtaining data for the continuous quality improvement activities.
- Obtain routine and non-routine laboratory tests as ordered and communicate critical lab values and urgent patient needs to the responsible physician or physician extender in a timely manner.
- Complete and document monthly review of patient medication profiles.Β Administer medications as ordered by the physician.
- Responsible for performing on call nursing services, nights and weekends, on a rotational basis as needed or assigned.
- Flexible with staffing locations and hours to accommodate patient and USRC home program needs.
- Regular and reliable attendance is required for the position.
Home Hemodialysis Only:
- Coordinate home evaluation with technical services department to ensure that the necessary electrical, plumbing and drainage requirements for proper equipment operation are met prior to patient acceptance into home program.

QUALIFICATIONS/REQUIREMENTS:Β To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Requirements include:
- Current RN license in applicable state.Β License must be maintained as current and in good standing.Β
- 12 months of clinical experience in providing nursing care and 3 months of experience in the specific modality for which the nurse will provide home dialysis training, except in states with a more stringent requirement such as Texas, which requires at least 6 months experience in the specific home modality.
- Previous experience in providing nursing care to patients on maintenance dialysis preferred.
- CPR certification required prior to patient care assignment.
- Confirmation of ability to distinguish all primary colors.
- Must successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency applicable for the modality assigned.

SUMMARY
The Acute Registered Nurse provides acute dialysis care and treatment to patients within a hospital environment. This position performs duties as assigned by the Charge Nurse or Acute Manager, including assisting and supervising Patient Care Technicians and Licensed Vocational Nurses. Acute nurses may report directly to an Acute Manager, Administrator or the first line of regional operational management which may be an Area Director, Regional Director, or Vice President of Operations, depending on the region/demographics.

Job Summary:
- Provides direct patient care to behavioral health patients in accordance with the established policies and procedures of the facility.
- Performs initial and on-going bio-psychosocial assessments according to accepted standards of nursing practice. Develops a nursing plan of care consistent with the master treatment plan.
- Participates in treatment team conferences to assist in planning and revising goals, objectives and interventions appropriate to the age-related and problem-specific needs of each patient.
- Implements nursing plan of care for assigned patients and conducts and/or co- leads group therapy sessions for patients.
- Evaluates patient's response to interventions and revises nursing plan of care as needed. Collaborates with the treatment team to revise goals, objectives and interventions appropriate to the changes in patient status.
- Performs appropriate assessments of patients per hospital policy and identifies physical, psychosocial, cultural spiritual and age-related needs in order to provide individualized care of the patient.
- Develops and documents a comprehensive plan of nursing care in collaboration with the patient and family to meet the physical, psychosocial, spiritual, cultural and age-related needs of patients in order to facilitate the delivery of care.
- Provides direct patient care according to physician's order and appropriate protocols as evidenced by documentation in patient's medical record and direct supervisor or peer observation.
- Evaluates and recognizes changes in patients' physical and psychosocial status and revises the plan of care as necessary.
- Delegates aspects of care to other nursing personnel, consistent with their level of education and expertise in order to meet patients' needs and to maximize staff performance.
- Demonstrates the knowledge and skills necessary to provide care appropriate to behavioral health patients and to interact with the interdisciplinary team in order to achieve the desired outcomes for patients.

Qualifications
- Associate Degree in Nursing (ASN) required, Bachelor of Science in Nursing (BSN) preferred.
- MustΒ be able to both individually and as a team member successfully manage out ofΒ control patients using the methods outlined in the MOAB training
- Licensed Registered Nurse credentialed from the Connecticut Department of Public Health obtained prior to hire date or job transfer date

The Career Readiness Manager at Urban Dove is tasked with leading the successful execution of Urban Doveβs Career Readiness Programs, reporting directly to the Director of College and Career Readiness. This role encompasses managing up to ten staff members, overseeing the Career Readiness curriculum, and coordinating internship opportunities. Adept in youth development leadership, the ideal candidate is committed to making a meaningful difference in underserved areas, prioritizing Urban Dove's Core Values of Teamwork, Leadership, and Communication. Additionally, the Career Readiness Β Manager supervises the Urban Dove HiRisers after-school program and summer camp, focusing on the growth and safety of camp staff and participants through sports, academic enrichment, and field trips, from a pre-camp orientation in June to a sleep-away camp finale in August.Β
ABOUT THE ORGANIZATION:
Urban Dove Team Charter School (UD Team) is a distinctive network of sports-based high schools catering to over-aged, under-credited youth. We fuse a robust academic curriculum with sports-based youth development, ensuring our graduates receive a diploma, a stepping stone for higher education, and essential job skills.
CORE RESPONSIBILITIES:
- Manage Career Readiness program, including workshops, school-based internships, and HiRisers after-school program and summer camp.
- Oversee the Career Readiness Associates and HiRisers youth staff, and coordinate with staff supervising interns.
- Evaluate and enhance Career Readiness workshops, events, and professional development for Career Readiness staff.
- Conduct regular staff meetings and address performance issues, focusing on improvement.
- Develop and update the career readiness curriculum and track workforce development data.
- Guide students towards workforce pathways and provide necessary intervention and counseling.
- Oversee the Summer Youth Employment Program, manage working papers, and offer job opportunities.
- Maintain workforce development databases and liaise with internship agencies and school sites.
- Organize and manage outreach, retention efforts for afterschool, recess, and summer camp participants.
- Coordinate and oversee special events, retreats, and camps in collaboration with program staff.
- Ensure departmental adherence to school and regulatory policies, including parent engagement.
- Manage department resources, monitor key school activities, and mentor a small group of students.
