The Registered Professional Nurse is responsible for the delivery and coordination of patient care provided by the caregiving team. S/he assures that competent, compassionate patient care is uniformly provided to customers including referring facilities, hospital staff and patients in the inpatient, ambulatory, and community settings.

Clinical Duties:

  • ο»Ώο»ΏAssists the attending Transplant surgeons, house staff, and Transplant staff with the direct care of hospitalized patients and outpatients on a rotating basis with the other clinical care coordinators, including: facilitation of patient discharges and transplant teaching
  • ο»Ώο»ΏAttends once a week outpatient Transplant clinics: RN orders labs, performs injection teaching, coordination of care

Schedule appropriate non-invasive laboratory tests and participates in the interpretation of the results and the institution of appropriate therapy based off physician guidances

  • ο»Ώο»ΏSchedules appropriate surgery and tests for patients
  • ο»Ώο»ΏDischarge Planning
    Reviews discharge instructions to hospitalized patients
    Works with hospital case manager and social worker regarding patient discharge needs
  • ο»Ώο»ΏAnswers telephone inquiries from patients and/or visiting nurses after hospital discharge
  • ο»Ώο»ΏEducation of health care workers and the public regarding organ transplantation
  • ο»Ώο»ΏParticipates in teaching programs of the Abdominal Organ Transplant Program

Medication Management:

The NPD practitioner is responsible for supporting learners within the context and framework of the Nursing Professional Development, Scope and Standards practice as approved by the American Nurses Association. This position works in collaboration with nursing leadership to foster and influence competency management, clinical nursing education, professional role development, policy standardization, collaborative partnerships, scientific inquiry, transition to practice and onboarding/orientation of new Hartford HealthCare colleagues. The practitioner is an essential member of the nursing leadership team who promotes the alignment of regional NPD and practice excellence with the HHC system as a whole.

  • Minimum of 3 years of active clinical nursing experience.

EDUCATION/CERTIFICATION:

  • Must be a graduate of an accredited school of Nursing and have a current Registered Nurse license with the state of Connecticut.

  • Must be trained in de-escalation techniques, and receive certification within the orientation period of hire.

  • Must receive RQI certification within three (3) months of hire and maintain clinical competency during employment within this role.Β

Qualifications

  • Cardiopulmonary Resuscitation (CPR) Certification is required.
