5C Medical Teaching Care team consisting of the Medical nursing staff, Hospitalists and specialized consultants. We use your expertise in assessing a patient’s physical and emotional status, past health history, health practice, and both the patient’s and the family’s knowledge of their disease process and treatment. The RN will be responsible for providing direct patient care, administering medications, collaborating with healthcare team members, evaluating patient conditions, and ensuring patient safety and well-being.

Unit

5B is a 40-bed medical unit with semi-private rooms & private rooms. The Medical Telemetry Unit provides scheduled and emergent care/procedures on a 24-hour basis. The area is to accommodate patients who need inpatient care for a variety of treatments, diagnoses/testing including, but not limited to COPD exacerbation, pneumonia, ETOH withdrawal, opioid withdrawal, nausea/vomiting/abdominal pain, altered mental status, osteomyelitis, back pain, diabetes management, and cellulitis.

The patient population cared for includes adult and geriatric patients. The conditions of patients cared for in this area can be described as fair and serious. The patients are received/transferred/admitted from home, the emergency department, and physician’s offices as well as from other units within the hospital setting.

This exciting curriculum integrates:

This unique 26-week program is a combination of classroom and clinical experiences. Participants will work under the supervision of experienced emergency nurses for the length of the program.  Upon program completion, the participant will be hired as a staff member in one of our 3 Emergency Departments within ChristianaCare.  This program has been devised by the nurses in the Emergency Department at Christiana Hospital. This program allows for a smooth transition from GN to the ED staff RN role into ED nursing, for those candidates qualified. The 26-week preceptor led program will be followed by continued residency meetings during months 8, 10, and 12 to further develop as professional nurses and integral members of our caregiver team.



ChristianaCare has 3 Emergency Departments:

  • ChristianaCare Newark Campus,
  • ChristianaCare Wilmington Campus
  • Middletown Emergency Department  

Christiana Care Newark Campus is the only Level 1 Adult Trauma Center between Philadelphia and Baltimore.  ChristianaCare Wilmington Campus is a Level 3 Adult Trauma Center.  Middletown Emergency Department is a freestanding ED.  Our Emergency Departments see approximately 195,000 patients annually, servicing an average of 500 patients per day in combined visits from all 3 departments. There are 162 patient rooms, including state-of-the-art trauma resuscitation facilities between the 3 departments.

Upon successful program completion, you will be hired as a staff nurse in one of the 3 Emergency Departments within ChristianaCare. Nurse residents are required to pass State Boards and be licensed as a RN in the State of Delaware prior to starting position. Upon hire, a commitment agreement to work at the ChristianaCare will be signed. Shadow experiences within the hiring critical care units at our Newark, Wilmington, and/or Cecil campuses will be provided for these candidates prior to date of interview.

The RN Case Manager will coordinate care and drive patient progression to establish a discharge plan. This includes functioning as an interdisciplinary team member, creating, implementing, and developing all aspects of discharge planning to establish appropriate, timely, and effective transitions throughout the care continuum.

The Care Management Model: 

Our Care Management Triad Team Model is a collaboration between the following:

  • RN Case Manager - Manages patient care, drives patient progression, and establishes a discharge plan.
  • Social Worker – Resolves psycho-social barriers and supports discharge needs.
  • Utilization Management – Reviews patient status for appropriateness and anticipated payer coverage.

CARE MANAGEMENT:

  • Identify patients who have post-acute care, placement, and complex discharge planning needs based on a comprehensive assessment that includes physical, as well as psycho-social factors/needs.
  • Anticipate, initiate, and establish a discharge plan for patients with post-acute care needs, collaborating with the physician, nurse, and other health care providers, the patient, their family/primary caregiver(s), third-party payers, and employer, following established clinical guidelines, standards, and pathways.
  • Review the admission assessment and collaborate with the primary nurse and other health care providers to ensure a multidisciplinary care plan is in place to meet identified patient care needs and desired outcomes.
  • Identify system issues that serve as barriers to care.  Participates in the development and implementation of strategies to remove the obstacles and facilitate patient progression.

If you are passionate about making a difference in patient care and have experience in Transition Care or Discharge Planning, along with a background in an acute care hospital setting, we would love to hear from you.

As part of our Weekender Incentive Program (WIP), you are required to work 3 out of 4 weekends in a four-week schedule. This consists of a 12-hour day shift on Saturdays and Sundays, along with one rotating Friday evening shift from 4 to 8 p.m. Additionally, there will be an alternating holiday shift every other year.

  • Utilization Management – Reviews patient status for appropriateness and anticipated payer coverage.

CARE MANAGEMENT:

  • Identify patients who have post-acute care, placement, and complex discharge planning needs based on a comprehensive assessment that includes physical, as well as psycho-social factors/needs.

  • Anticipate, initiate, and establish a discharge plan for patients with post-acute care needs, collaborating with the physician, nurse, and other health care providers, the patient, their family/primary caregiver(s), third-party payers, and employer following established clinical guidelines, standards, and pathways.

  • Review the admission assessment and collaborate with the primary nurse and other health care providers to ensure a multidisciplinary care plan is in place to meet identified patient care needs and desired outcomes.

  • Identify system issues that serve as barriers to care.  Participates in the development and implementation of strategies to remove barriers and facilitate patient progression.

  • An active RN license in DE or Compact State is required.

This is a flat-rate position. With benefits, the WIP rate is $35 plus a $10 shift differential.
