Family Practice - With OB Physician

Looking for a Family Practice or Internal Medicine Physician to work in Phoenix, AZ. This need is looking for someone to start as soon as credentialed and would be needing someone for a full 6 months of coverage. There would be an option to extend past the 6 months. Please see below details:
Location: Phoenix, AZ
Dates coverage is needed: as soon as onboarding is complete (with 6 month option to extend)
Schedule/Shifts Times: 4 ten hour shifts during the M-F week. No weekends required
Call Requirements: None
% inpatient / % outpatient: Outpatient only
Patient Volume: 12- 15 patients per day average
Patient Ages: adults only
Types of Cases: standard outpatient care, chronic illness treatment
Support Staff: yes (several Physicians, mid-levels, RN s and specialty Physician support)
Procedures Required: None
Type of Charting: EMR - RPMS
Reason for Need: staff shortage
Credentialing Timeframe: 4-6 weeks on average may take longer
Additional Info: any State License ; minimal gyn, only paps and pelvic exams for symptoms as needed. No suturing.
