Summary : Under the supervision of the LPN, RN, or nurse leader, the Certified Nursing Assistant (CNA) performs patient-centered, evidence-based nursing care activities to meet the physical and psychosocial needs of the patient. Through collaborative practice with all members of the care team, is responsible for patient outcomes that meet the high quality of care provided by ThedaCare. Contributes to the goals of the department by being accountable for the delivery of compassionate, safe care and demonstrated commitment to continuous growth and professional development. Job Description:KEY ACCOUNTABILITIES:
Performs delegated patient care activities using evidence-based practice and specific competencies for the patient population served.
Assists in meeting the care needs of the patient in collaboration with the care team to ensure quality, cost-effective safe patient care practices.
Demonstrates clinical expertise in the provision of care in the clinical specialty assigned and performs all functions of the Certified Nursing Assistant (CNA), which are age appropriate, developmentally sensitive, and culturally specific.
Respectfully addresses situations or decisions to ensure patient concerns and needs are met.
Identifies ways to improve the patient's experience of care, streamline care processes, and lower costs to improve patient, family, and team member satisfaction.
Identifies and escalates patient risk to prevent crises.
Demonstrates professional and therapeutic relationships with patients, families, team members, and providers.
Reinforces patient education to improve care outcomes and safe patient practices.
Performs and documents patient cares including personal hygiene, mobility, activities of daily living, and vital signs, and ensures the care environment promotes patient safety.
Provides care to patients including collecting body fluids for testing and cultures, administering medications if applicable, and providing patient care as defined within the nursing plan of care and/or physician orders, within the scope of the CNA role.
QUALIFICATIONS:
High school diploma or GED preferred.

Valid State of Wisconsin CNA Certification
American Heart Association Healthcare Provider Basic Life Support (BLS) certification required
PHYSICAL DEMANDS:
Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance
Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
Manual dexterity and hand-eye coordination to perform patient care procedures

Certified Nursing Assistants provide basic nursing care to patients within the scope of the nursing assistant responsibilities and perform basic nursing procedures to promote patient comfort and support under the direction of the RN Case Manager.
Initiates delegated tasks based on demonstrated competence in performing nursing activities within their scope of practice. Documents and provides appropriate data to the RN and/or LPN for analysis and validation of patient's response to care or treatment and when significant change occurs in the patient's condition. Contributes to safety and quality care by relaying information via charting and communicating to team members. Informs licensed nurse of any changes in patient condition/needs, of patient complaints, and of patient/significant other educational needs. Assists with baths, ambulating, feeding, skin care, personal and oral hygiene and making occupied and unoccupied beds. Performs dressing changes, enemas, ostomy care, toileting, turning, cough and deep breathing, catheter care, collection of biological specimens, measuring and recording intake and output of fluids. Performs other duties as assigned.
Location: ThedaCare Homecare and Hospice cover numerous central Wisconsin counties split up into teams by location.
This position requires a weekend rotation of every 12 weeks and 1 holiday per year.

Job Summary:
Using independent decision making and critical thinking skills, the Pharmacist promotes medication that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.
EXPERIENCE DESCRIPTION:
A minimum of 1 year hospital or 3 years of other pharmacy experience is preferred.
EDUCATION DESCRIPTION:
Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.
SPECIAL SKILLS DESCRIPTION:
Demonstrate effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. The inpatient psychiatric unit has a total to 19 adult beds and intake team. The Behavioral Health Director provides strategic, clinical, and operational leadership for behavioral health services across the organization. Experience:
• 36 months of role experience is required with some in the last 12 months.
• Must have at least 10 months of Medical Surgical Unit experience.
• 3 months of travel experience is required from any number of months.
• 3 months of experience with Epic is preferred from last 12 months.
Requirements:
• Candidates must have a Wisconsin license or compact license (required for submission).
• This role may require floating to additional units and locations
• Travel only, local not allowed. Candidates must live >50 miles from facility to be submitted.
• 6 months gap required between for Staff at Program: Staffency Program - ThedaCare and no current placement allowed at Program: Staffency Program - ThedaCare.

Additional Details:
An interview with the unit manager may not be available prior to offer. If you do not interview with the unit manager, you may interview with a third-party clinical nurse manager.
Note: If a candidate has NP licensure, ThedaCare will only consider them for an NP role, not a lower position.
Preferred Skills/Experience:
• Any ICU/IMC (intermediate care) experience.
• Experience in stroke and post-op back/spine/cervical
Required Credentials: Associate Degree or Certificate
Special Requests: Any missed shift must be made up within 2 weeks of the absence.
Shift & Scheduling:
• D/N Variable (open to straight NOC candidate)
• Shift time: days) /1730-0600 (nocs)
• Weekend: Every other weekend
• Holidays: Holiday rotation (depending on need of the unit)
• On Call/Call Back Requirements: None
Floating Requirements: Throughout facility within skillset, To all ThedaCare Facilities within a 60 mile radius.
9 days maximum time off
1 reference from last 3 years (Charge/Lead reference type) - required for submission
1 reference from last 12 months (Manager/Supervisor reference type) - required for submission
