Summary
Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient family centered approach. Performs skilled treatments and interventions as ordered by the physician. Job Duties
- Delivers patient care based on the medical plan of treatment established by the physician and protocols using a patient family centered approach. Performs comprehensive subjective and objective ongoing assessments of patient's status that includes physical, psychosocial and environmental parameters. Relays significant changes in patient status to the physician, RN Case Manager, and other members of the interdisciplinary care team as appropriate in a time period consistent with patient needs.
- Reviews, evaluates, and as appropriate, revises the established plan of care to reflect progress toward achieving patient outcomes/goals in collaboration with the RN Case Manager and Home Health Care Navigator.
- Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning. Promotes patient/caregiver autonomy. Evaluates effectiveness of teaching and modifies education based on patient needs and goals.
Minimum Qualifications
- Specialized Diploma Nursing or
- Associate’s Degree Nursing
- 1 year recent experience providing direct care in a healthcare setting
- Knowledge of patient education techniques and principles
- Ability to take initiative and function independently without direct supervision and as a member of the care team
- Ability to travel to patient care assignments
- Knowledge of Home Health Conditions of Participation

Physical Demands

Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Then Energy Efficiency Field Technician will be based in or near Houston, TX to support implementation of utility energy efficiency programs. This role conducts field assessments, verifies contractor work using specialized tools and procedures, and ensures timely, accurate upload of inspection data, photos, and documentation into program databases. The position requires regional travel, efficient route planning, with occasional overnight travel within Texas and neighboring states. Responsibilities may expand with training and demonstrated proficiency.
Key Responsibilities
- Representing Frontier Energy and its utility clients positively and professionally to all program participants, including homeowners, tenants, property owners, contractors, and public service agencies.
- Interacting with homeowners, tenants, and others to arrange and conduct field visits.
- Conducting on-site assessments of single-family homes, multifamily apartment complexes, and other facilities to determine what energy efficiency measures are eligible for participation in various programs.
- Conducting post-installation verification inspections to ensure contractors are accurately representing work performed.
- Accurately recording data and documentation from site visits, and reporting this information in a timely manner to other team members.
- Interacting with program databases to identify needed assessments and inspections, conducting phone outreach to customers to schedule inspections, informing others of the scheduled inspections, and interacting with customers and clients politely and professionally; and,
- Facilitating and supervising the installation of “direct install” measures, such as screw-in LED bulbs and water efficiency measures
Preferred Skills
- Prior knowledge and/or experience in building energy and/or energy efficiency concepts and measures.
- Associates degree or higher in building science, construction management, or other related fields.
- Verbal fluency in Spanish and English. Written fluency in English.

The workers of Electric Operations ensure the delivery of safe and reliable electric service to our customers. Electric Operations is responsible for every aspect of PG&E's electric distribution and transmission operations, including planning, engineering, maintenance and construction, asset management, business planning, restoration and emergency response.
This classification is represented by IBEW Local 1245, which has over 100 years of experience representing employees at PG&E. A coworker who has the qualifications of a Cable Splicer, is familiar with the underground system and has the experience and ability to analyze cable and equipment troubles quickly and to restore services promptly by replacing fuses, switching, disconnecting defective cables or apparatus, making repairs, etcetera. May be required to perform other underground work such us preparing cable racks, pulling in and racking cables, maintaining equipment in the underground system, etc. Shall install and maintain services, meters, and equipment including conduit and wiring up to the meter on customers' premises and shall handle trouble in such installations.
