Qualified Intellectual Disability Professional

This position requires someone who has had a Drivers License for at least 1 year (post permit) and a clean driving record that meets Sevita's insurability policy.

Bring your passion and experience to make a positive difference in the lives of others as aΒ QIDP (Qualified Intellectual Disability Professional) with REM Community Services!Β The QIDP provides oversight and support of individual programming and needs in assigned ICF programs.Β The QIDP will work closely with facility and day program staff to ensure consistency and ensure service design and delivery, providing each individual with an active appropriate treatment program.Β Β The QIDP will also ensure that any discrepancies or conflicts between programmatic, medical, dietary, and vocational aspects of the individualβs assessment and program are resolved and will ensure follow-up to recommendations for services, equipment, or programs.Β

WHAT YOU WILL BE DOING:
- Write and update individualβs POC, programs, data sheets, corresponding informed consents and staff training sheets; prepares for and participates in Plan of Care/ IDT team meetings
- Conducts training on individual programming for facility and day program staff
- Completes ongoing required documentation for each individual served, such as monthly documentation and progress notes.
- Ensures that all records are accurate, up-to-date, and in compliance with legal and corporate requirements.
- Ensures follow-up to recommendations for services, equipment, or programs; and ensures that adequate supports and assistive devices are present to promote independence.
- Attend and participate in facility and vocational program staff meetings
- Conduct observations of all shifts at each facility and at the day program; provides feedback and recommendations to supervisorsΒ and staff regarding program implementation / observations
- Ensure Comprehensive Function Assessmentβs (CFA) are completed and reviewed annually; update as individualβs needs change
- Maintains knowledge of clinical standards and ensures compliance in terms of treatment interventions with all regulatory, contractual, corporate, and legal requirements.
- Assists with all legal, licensing, regulatory, and certifications activities as needed. Monitors plan of correction for ongoing compliance.
- Develops and maintains working relationships with all clients, families, funding sources and regulatory agencies, as appropriate, and monitors their satisfaction with services.

SUMMARY
Administers physical therapy treatments to patients and works under the direction of and as an assistant to the Physical Therapists.

ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:Β
- Assures all program participants receive scheduled PT (Physical Therapy) treatments and documents therapy session or reason for inability to attend.
- Performs active, passive and/or resistive therapeutic exercises, general therapy principles on program participants.
- Performs therapeutic massage on program participants as needed.
- Performs general mobilization and/or passive stretch to program participants, within LIMITS OF SAFETY regarding joint/muscle stretch.
- Instructs, motivates and assists program participants in learning and improving functional activities.
- Provides gait training and instructs program participants in the use of canes, crutches, walkers, braces and splints. Trains program participants at homebound and community level, including endurance training
- Measures ROM, length and girth of body parts, vital signs, and MMT (manual muscle tests) to assist Physical Therapist in evaluating program participants and/or to determine the effectiveness of the program plan
- Confers with staff members and other health care members to exchange, discuss and evaluate program participant information for modifying and coordinating current treatment program and participate in treatment planning with trans-disciplinary team as requested.
- Ensures adherence to accreditation standards, licensure standards, professional code of ethics, and confidentiality.
- Demonstrates knowledge of indications and contra-indications of physical therapy modalities and techniques.
- Asses PT equipment and has a good working knowledge of physical therapy equipment and therapeutic exercise treatments.
