The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. In this role you will have the ability to achieve work life balance. No on-call, no weekends, and no holidays required. This position requires travel through the states of Connecticut & Rhode Island.

Primary Responsibilities:

• Provide clinical coverage for the primary APC during periods of PTO or short/long term absences
• Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions
• Manage members during a short term, sub-acute rehabilitation episode
• Conduct comprehensive, monthly/routine, and/or acute care visit assessments
• Effectively manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD 10 conditions
• Communicate and collaborate with the interdisciplinary care team

Required Qualifications:

• Certified APC through a national board
• For NPs: Graduate of an accredited master’s degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC), Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP), Adult/Family or Gerontology Nurse Practitioners (ACNP), with preferred certification as ANP, FNP, or GNP
• For PAs: Graduate of an accredited Physician Assistant degree program and currently board certified by the National Commission on Certification of Physician Assistants (NCCPA)
• Active and unrestricted APC license in the State or Connecticut or Rhose Island and the ability to attain the licemse you do not possess.
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
• Ability to gain a collaborative practice agreement, if applicable in your state
• Ability to complete physical requirements, including moving a 30-pound bag and ability to perform physical assessment
• Access to reliable transportation
• 1+ years of clinical experience in practice (long-term care setting preferred)

Preferred Qualifications:

• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Proficient computer skills including the ability to document medical information with written and electronic medical records

**PLEASE NOTE** Employees must be in an active regular status. Employees must remain in role for a minimum of 12 months from the date of hire /rehire/transfer. If an employee leaves Home and Community, the student loan repayments will cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full benefit of the student loan repayments.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). Community Based Clinician - Intensive Family Services

We have an exciting opportunity for a Community Based Clinician to join our Intensive Family Services program in Somerville!

Our ideal candidate will have a Master's Degree in a clinical mental health discipline and familiarity working with: children and families and also home and community-based experience. The clinician will provide direct, intensive community-based support, advocacy and stabilization for the children and families.

Working from a variety of therapeutic modalities, Riverside Community Care emphasizes a family systems approach that works with every member of the family to help meet therapeutic goals. You will provide in-home and in-community stabilization and clinical intervention through youth and family psychotherapy, support, service planning, advocacy and linkage services to youth, including children with severe emotional disturbances (SED), adolescents, and families in the community. You will provide culturally competent services to a diverse population, including children and families frequently at risk due to multiple stressors, such as inadequate community supports and resources, language barriers, poverty and homelessness.

Nurse Care Manager / Registered Nurse – Behavioral Health Community Partners

Riverside is seeking a Registered Nurse to join our growing Behavioral Health Community Partners (BH CP) team for a hybrid position based out of our Needham area office. We have a unique opportunity to expand your experience on an interdisciplinary team and coordination with clients living with psychiatric or mental diagnosis.

The BH CP team provides services to people served within in the community. Care Coordinator - BHCP

Riverside is seeking a passionate Care Coordinator to join our Behavioral Health Community Partners (BHCP) team!

This is a unique opportunity to join a growing team of dedicated behavioral health professionals.

As a Care Coordinator, the employee will provide person-centered and recovery-oriented care management and coordination to eligible MassHealth consumers served in the community. Peer Specialist - Program for Assertive Community Treatment

Riverside Community Care is seeking a Peer Specialist to join our community-based PACT program in the Norwood/Needham/Dedham area.

PACT provides individuals referred by the Department of Mental Health or MBHP with access to community-based rehabilitation, clinical treatment, employment, housing, wellness and peer services, with ancillary supports that will be coordinated in partnership with the individual. Services are designed to be flexible based on the needs of the persons served, and to promote resiliency and recovery for all participants. PACT is a 24/7 program and services are provided 7 days per week, 365 days per year.

This is an exciting opportunity to guide the recovery focused culture within this team.

The Peer Specialist will work as a member of the PACT team to inspire that recovery is possible through individual and group conversations, guiding individuals through their own recovery process, and assisting individuals with learning about and accessing recovery resources and communities. The adjunct behavior analyst provides consultative services to individuals, families, caregivers, and/or providers served by The Department of Developmental Services in the DDS Charles River West Area Office located in Waltham, MA. The behavior analyst completes formal and informal assessments, collaborates with support teams in the DDS area office and/or other providers, and establishes interventions in accordance with the state Positive Behavior Support regulations. This job is a hybrid position. This position requires 2 - 3 days (the days can vary each week) in the DDS Charles River West Area Office. It also requires in-person observations in the communities served by the area office.

Our ideal candidate will have patience, compassion, and creativity when providing consultative services to adults diagnosed with developmental disabilities, autism spectrum disorder, and/or cognitive disabilities.

Community Crisis Stabilization Clinician - Weekends

The CCS Clinician works within the Adult CCS component of the larger Community Behavioral Health Center (CBHC) and is primarily responsible for conducting comprehensive assessments inclusive of the use of standardized assessment tools for suicide risk, brief crisis counseling, individualized risk management/safety planning, psychoeducation, discharge planning, and care coordination.

In this exciting role, the Clinician will:

Community Crisis Stabilization Counselor

The Community Crisis Stabilization programs provide both short and long-term care for persons experiencing mental health, substance use disorder, and co-occurring disorders who might otherwise require treatment in an inpatient setting and would benefit from structured stabilization, support, care coordination, and treatment planning.

We are seeking a Community Support Program - Justice Involved (CSP-JI) Navigator for Connect2Recovery in Norwood!
