The Senior Community Care (SCC) program is a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting. The APC within SCC provides care to our highest-risk health plan members and is part of an interdisciplinary team. In this role you will have the ability to achieve work life balance. No on-call, no weekends, and no holidays required. This position requires travel through the states of Connecticut & Rhode Island.
Primary Responsibilities:
- Provide clinical coverage for the primary APC during periods of PTO or short/long term absences
- Manage members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, and interventions
- Manage members during a short term, sub-acute rehabilitation episode
- Conduct comprehensive, monthly/routine, and/or acute care visit assessments
- Effectively manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD 10 conditions
- Communicate and collaborate with the interdisciplinary care team

Required Qualifications:
- Certified APC through a national board

- Active and unrestricted APC license in the State or Connecticut or Rhose Island and the ability to attain the licemse you do not possess.
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
- Ability to gain a collaborative practice agreement, if applicable in your state
- Ability to complete physical requirements, including moving a 30-pound bag and ability to perform physical assessment
- Access to reliable transportation
- 1+ years of clinical experience in practice (long-term care setting preferred)
Preferred Qualifications:
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs

Job Description Summary:
This position ensures organizational excellence related to customer service and quality of care while promoting excellence in the cultural experience at assigned hospital. This position requires ability to act quickly and work in high stress situations.

30% Clinical AssessmentConducts patient/family/significant other interviews, chart reviews, and professional collaboration to obtain and interpret information pertaining to patient/client biopsychosocial functioning. Assessment to include, but not be limited to: developmental and age-specific needs; ability to care for self; ability to care for dependents; ability to meet basic custodial and financial needs; ability to engage in meaningful social and emotional relationships; and ability to comprehend and comply with treatment recommendations. Develops care plan, including discharge plan, incorporating medical plan of care. Develops a plan of intervention based upon assessment to resolve identified problems and/or to support and enhance biopsychosocial functioning. Through individual, family, or group modalities, employs clinical intervention skills including, but not limited to, treatment planning; continuum of care planning (including discharge planning); supportive counseling; crisis intervention; specialized problem oriented planning and intervention; and educational counseling.
25% Population Management
Staff is accountable for coordinating clinical and financial outcomes of an assigned population. Works collaboratively with interdisciplinary staff and external organizations to achieve timely, cost effective management of patient care. Monitors for quality and type of services delivered to patients, ensuring management within established parameters. Functions as a liaison to the payer to manage the post-discharge financial resources and ensure expected treatment process, progress and discharge plan. Monitors and ensures completeness of all assigned patients in areas of treatment plan and timely reports.
10% Documentation and Regulatory Compliance
Thoroughly documents assessments and interventions in patient’s medical record and information systems in accordance with department standards. Complies with internal and external documentation requirements as related to specific patient/system needs and regulatory requirements.

The Community Care team is a multidisciplinary service including Registered Nurse (RN) Community Care nurses, Licensed Practical Nurse (LPN) Community Care nurses, Community Social Workers (CSW) and Community Health Coordinator (CHC) who work with our highest complexity patients and their primary care physicians to meet their medical and social needs with the aims of fully engaging them in our intensive primary care model and maximizing their healthy time at home.
Intensive Community Manager will serve as a clinical lead for a Community Care team. They will coordinate the teams efforts to stabilize our highest risk patients, with special areas of focus including safe transitions of care from facilities back to our primary care teams, stabilization of our highest risk ambulatory patients and outreach to patients who are assigned to us but are not engaged in care.
This person will perform initial assessments and design comprehensive plans of care for many of these patients. This professional will also provide clinical supervision to other team members in delivering the plan of care and in other tasks necessary to meet their needs and engage them in care. EDUCATION AND EXPERIENCE CRITERIA:
- Associate degree in Nursing required.

- A valid, active Registered Nurse (RN) license in State of employment required.
- A minimum of 2 years clinical work experience required.
- A minimum of 1 year of case management experience in community case management experience highly desired.
- Certified Case Manager certification is preferred. Certification through the Commission for Case Manager Certification (CCMC) or the American Association of Managed Care Nurses (CMCN) desired
- This position requires possession and maintenance of a current, valid drivers license.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment

- Assesses patient pain regularly to promote effective pain management, including reassessments after appropriate intervention.

- Performs exceptional care by responding promptly to patient requests. Strives to anticipate patient needs and resolve them proactively.
