Summary:
Under the supervision and/or direction of the Registered Nurse, the Nurse Extern II performs various direct patient care activities in accordance with the Nursing Department policies and procedures. The Medical Assistant will perform clinical and administrative duties in an ambulatory care setting under the supervision of a physician/provider and/or a registered nurse. Duties may include venipuncture, medication and vaccine administration, measurement, and documentation of vital signs, and assisting providers with patient exams, and telephone and clerical support.

Primary Duties:
- Assists providers with patient preparation, office procedures, examination and education of patients. Supports the clinical care team, assuring optimal best-practice patient flow; manages patient volume. Reviews and prepares for scheduled patient visits. Screens all patients for preventative care needs, ongoing support, barriers to care, safety concerns, and potential fall risk. Conducts surveillance for transitions of care, urgent/emergent care, or follow up tasks. Conducts motivational interviews including patient screening/ coaching on disease management, motivation to change, learning care needs, and community outreach needs. Identifies potential behavioral health needs of patients and notifies provider of findings.
- Reports actual/ potential problems concerning patients, families and associates. Participates in performance improvement initiatives to improve care quality and safety. Attends training for safety, infection control, etc. Ensures ongoing compliance by maintaining and auditing temperature logs, ensuring medication supplies are current/unexpired, checking and maintaining emergency equipment, area free of clutter, and other related tasks. Participates in population management activities including an awareness of the populations served and engages the patient in self-management support, care/health goal setting, pre-visit planning, and the capturing of information that leads to health care improvement activities.
- Performs selected patient/ laboratory testing, including Point of Care Testing, tuberculosis skin testing, electrocardiography, pulmonary function tests, vision and hearing testing. Removes sutures/staples. Documents activities in the patient’s electronic medical record (EMR). Performs required patient follow-up: to include pap smears, Pb levels, HIV, mammograms, and STD’s. Collects and processes specimens, completes requisitions, and documents.
- Provides escort and assists with patient discharge activities. Provides chaperoning. Fulfills infection prevention and environmental responsibilities as assigned. Updates appropriate records of scheduled equipment maintenance and manages drug and medical supply inventories. Provides telephone screening services to gather information from patients, determine appropriate disposition and document per established telephone screening protocols.
- May insert intravenous catheter for physician initiated IV fluid therapy or administration of intravenous medications. May insert, remove, and monitor urinary catheter for procedures and prepare and administer oral drugs and injections limited to intradermal, subcutaneous, and intramuscular. May perform phlebotomy. Utilizes the EMR to propose patient medication refills for provider approval. Completes requisitions for tests. Contacts insurance companies for Pre-authorizations for patient medications and specialty referrals including insurance authorization.
Qualifications:
- High School Diploma/ GED.
- Certified Medical Assistant (CMA) by the American Association of Medical Assistants within 1 year OR Registered Medical Assistant (RMA) through American Medical Technologists within 1 Year.
- Basic Life Support (BLS).
- Experience working in an ambulatory care medical office or equivalent.
- Externship in a medical or ambulatory health setting strongly preferred.
- Completion of an accredited Medical Assistant program preferred.
- Completed externship program at MedStar Health may be substituted for experience in ambulatory care medical office.

The Patient Flow Center is intended to improve the quality and safety of care, reduce the need for out of area placements, and optimize the use of resources. The Patient Flow Center RN Coordinator assists with placement of patients in the acute care setting in accordance with established guidelines to the appropriate facility and service line. MINIMUM QUALIFICATIONS
Education: Graduate of an accredited School of Professional Nursing; Minimum Associates Degree of Nursing required; Bachelors of Nursing preferred
Licenses/Certifications:
Current State of Texas license or temporary/compact license to practice professional nursing required
Additional certifications may be required based on discipline and/or nursing unit requirements
Experience / Knowledge / Skills:
Three (3) years of acute care hospital RN experience within the last 5 years (i.e. emergency department, critical care, intermediate, or medical/surgical units)
Preferred experience in an admission/bed placement center.
Previous OA/house supervisor experience desirable
Ability to type on computer
Familiarity with computer software, electronic health records
Demonstrated excellent critical thinking with strong clinical nursing assessment skills.
Effective communication (oral/written) skills.

PRINCIPAL ACCOUNTABILITIES
Uses critical thinking and clinical judgement skills in directing patients to the most appropriate level of care and unit assignment while ensuring accurate and complete communication with all areas across the system.
Organizes placement process to meet patient load influx and minimizes bed assignment delays.
Acts as a clinical resource to non clinical team members within the Patient Flow Center.
Utilizes clinical knowledge of current census information and potential inpatient bed needs to project accurate and appropriate placement of incoming patients.
Conducts admission reviews related to level of care changes, to assure patient is placed in the appropriate bed based on physician order and level of care required.
Applies professional nursing skills in applying the four aspects of the nursing process (assessment, planning, implementation and evaluation) when indicated to evaluate needs of incoming patient based on level of care, availability of beds, physician order, and unit capability.
Uses critical thinking skills and knowledge of disease processes to intervene and/or guide non clinical personnel when there is a known or unknown confounding factor which would negatively impact the patient and/or patient flow.
Manages all patient placement and progression functions in a responsive manner responding to all callers with the aid of the electronic health record, approved clinical protocols, and established patient care protocols and healthcare information.
Works with all Memorial Hermann hospital teams and key stakeholders on facilitating efficient, timely placement of patients across the system.
Facilitate the coordination of patient placement with physicians, staff RNs, and other departments while meeting state and federal regulations (COBRA/EMTALA); plan of care to optimize patient outcomes and decrease length of stay in an unbiased, standardized manner
Uses available tracking systems to maintain ongoing surveillance of available staffed beds. Monitors progress with bed assignments and arrivals of patients to assigned units; monitors for timeliness of transfers and discharges.
Incorporates knowledge of hospital and unit admission criteria, unit specialties, limitations, and patient need when determining proper patient placement.
Conducts or participates in daily bed meetings where appropriate.
Alerts department leadership of delays with patient flow for individual patients and departments experiencing delays.
Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

Job Summary
The Manager of Case Management is responsible and accountable to assist the Director of Case Management in the implementation of the case management program at the local level. The components/roles of the inpatient case management program consist of the following: Care Facilitation, Utilization Management, Case Management and Discharge Planning.The Manager is responsible for coordinating the use systems and processes for care/utilization management at the hospital level. In addition, the Manager is responsible for to assist the Director in managing the department’s activities related to discharge planning and clinical quality improvement. The Manager coordinates day to day departmental operations and the use of hospital resources appropriately and effectively. The Manager participates in the collection, analysis and reporting of financial and quality data related to utilization management, quality improvement and performance improvement.Job Description
Minimum Qualifications
Education: Bachelors of Science in Nursing OR Social Work (BSW). Master’s degree preferred*
*Note: effective March 1, 2019 and going forward, these are the minimum qualifications for this role; incumbents hired before March 1, 2019 may have commensurate experience in lieu of BSN.
Licenses/Certifications:
Current and valid license to practice as a Registered Nurse in the state of Texas or
Licensed Master Social Worker (LMSW) required, LCSW preferred
Case Manager Certification required
Experience/ Knowledge/ Skills:
Minimum five (5) years experience in utilization management, case management, discharge planning or other cost/quality management program
Three (3) years of experience in hospital-based nursing or social work
Three (3) years of demonstrated leadership experience
Knowledge of leading practice in clinical care and payor requirements
Self-motivated, proven communication skills, assertive
Background in business planning, and targeted outcomes
Working knowledge of managed care, inpatient, outpatient, and the home health continuum, as well as utilization management and case management
Working knowledge of the concepts associated with Performance Improvement
Demonstrated effective working relationship with physicians
Ability to work collaboratively with health care professionals at all levels to achieve established goals and improve quality outcomes
Effective oral and written communication skills
Principal Accountabilities
Assists in supervising and managing all aspects of the local level program.
Supports growth and development of the case management program consistent with enterprise wide philosophy and in response to the dynamic nature of the health care environment through benchmarking for best practices, networking, quality management, and other activities as needed.
Responsible for coordinating day to day operations of the program at the unit level.
Assists in identifying and achieving optimal targeted financial outcomes via the inpatient case management process.
Participates in departmental personnel functions (hiring, firing, etc.) in conjunction with the Director of Case Management.
Provides input to annual and interim performance appraisal reviews for the professional and non-professional staff in department.
Acts as liaison to facilitate communication and collaboration between all care partners (physicians, hospitalists, community care managers, nurses, community resources, etc.)
Responsible for leading a high performance team of “system thinkers” who incorporate leadership principles and vision in performing the functions of case management.
Uses data to drive decisions, plan, and implement performance improvement strategies for case management.
