QuadMed is looking for a Full time Physical Therapist to join our team at our new health center in Beaver Dam, WI and Mayville, WI. This role will work 32 hours a week with the ideal schedule being Monday-Thursday.

JOB REQUIREMENTS:

Education:

• Minimum Bachelor's Degree in Physical Therapy from an accredited university or college; Doctorate in Physical Therapy preferred

Experience:

• Minimum one year of experience; minimum 3 years in an orthopedic clinic setting preferred

Certificates, Licenses, Registrations:

• Current State licensure

Knowledge, Skills & Abilities:

• Intermediate computer skills
• Excellent communication and interpersonal skills
• Evidence of organizational skills, appropriate decision-making, and problem-solving skills

Job Requirements:

• M.D. or D.O. degree from an accredited School of Medicine and completion of appropriate residency
• Active board certification in Family Medicine
• Ideally, 3+ years of experience in primary care
• May be required to obtain and maintain a valid DOT Medical Examiner Certification, depending on job responsibilities or location.
