Overview

Provides physical therapy interventions to designated patients under the supervision of a Physical Therapist (PT). Assists in monitoring patient progress through observations and follows up with supervising therapist. Works under PT supervision as required under NY State Education law.

• Follows established physical therapy plan of care, including physical therapy interventions and patient/caregiver education. Modifies specific treatment procedures in accordance with PT plan of care and Physical Therapy Assistant (“PTA”) scope of practice.
• Documents all care provided and maintain records per VNS Health policy and standards.
• Assists in monitoring patient progress through observation and follows up with Supervising Therapist in accordance with VNSNY policy.
• Complies with scope of practice for PTA, including a co-visit with Physical Therapist on all initial PTA visits and keeps PT informed. PT will re-assess patient every 6 visits or 30 days (whichever comes first).
• Manages assigned caseload; provides physical therapy services to patients as directed per PT plan of care.
• Participates in VNS Health meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews.
• Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient.
• Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment.
• Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care.
• Transports and utilizes required technology devices (tablet), infection control materials and supplies using VNS Health designated/supplied carrying case (backpack) weighing approximately 15 – 20 lbs to and from patient homes/care facilities, VNS Health offices and therapists’ home.

• Participates in special projects and performs other duties as assigned.
Qualifications

Licenses and Certifications:
New York State Certification as Physical Therapist Assistant required
Valid driver's license may be required, as determined by operational/regional needs.

Education:
Associate's Degree as Physical Therapist Assistant from a New York State approved program or whose educational credentials have been approved equivalent by New York State required

Work Experience:
Minimum of two years experience in clinical patient care (per NY State Education Law) required
Bilingual skills may be required, as determined by operational needs.
Basic PC skills preferred

Pay Range

USD $33.88 - USD $42.35 /Hr.

The Physical Therapist Assistant is part of a  multi-disciplinary team to deliver patient-centered physical rehabilitation services to patients, supports Physical Therapist in treatment procedures.  Physical Therapist Assistants (PTA) are required to have supervision from a Physical Therapist to deliver services. PTAs communicate thoroughly and regularly with Physical Therapists to ensure delivery of plan of care.  Completes all required documentation as per state/federal and facility guidelines.


Responsibilties:

A day in the life of a Physical Therapy Assistant at Hackensack Meridian Health includes:

 

1. Demonstrates team approach to problem solving through frequent communication and collaboration on complex issues to provide appropriate care. 
2. Ensures operation of equipment by using equipment consistent with equipment specifications; completing preventive maintenance requirements; following manufacturer¿s instructions; troubleshooting malfunctions; and calling for repairs. 
3. Participates and represents PT staff on committees, as requested by supervisors. 
4. Demonstrates appropriate knowledge in assessing the need for durable medical equipment for patients.  Follows through with appropriate professionals to ensure proper fitting and educates patient on proper usage. 
5. Maintains professional competence and keeps current with Physical Therapy trends through participation in continuing education, in-service programs, and professional staff sessions.
6. Complies with federal, state, and local legal and professional requirements.
7. Fosters good relationships and communications with all team members, physicians, patients, family/significant others and visitors.
8. Assists with new Team Member orientation and provide key experiences to prepare Team Member independent performance of essential job functions.  Problems are reported to designated Supervisor promptly. 
9. Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
10. Instructs, encourages and assists patients to carry out plan of care, specific to patient¿s needs and goals established by Physical Therapist(s).
11. Models and maintains safe environment to deliver services for patient, self, and healthcare team.  Employs correct body mechanics during all activities.
12. Demonstrates and implements evidence-based treatment techniques and skills with patients.  Maintains professional competence and keeps current with Physical Therapy trends.
13. Continuously monitors patient¿s response to treatment and adjusts treatment accordingly in consultation with Physical Therapist.
14. Identifies and reports any real or potential problems regarding the development or admin of patient care. 
15. Assists with discharge planning by collaborating with physicians, nurses, social workers, supervising physical therapist(s), and other healthcare providers.
16. Contributes to patient care conferences.
17. Completes all documentation accurately and in a timely manner, consistent with all state regulations.
18. Other duties and/or projects as assigned.
19. Adheres to HMH Organizational competencies and standards of behavior.


Qualifications:

As a Physical Therapy Assistant (PTA), you will:

- Updates the care plan in coordination with the therapist and the physician, and plan with the patient/family toward adjustment.
- Plans monthly patient re-evaluations with the therapist, coordinating with the therapist more frequently if needed, and plan the weekly patient schedule.
- Documents patient care data accurately, thoroughly, and in a timely matter, reflecting evidence of improvement/progress/decline in patient's condition.
- Submits the daily/weekly patient visit schedule to the Scheduler.
- Communicates as necessary with intercompany and intracompany contacts.
- Discusses treatment progress with the therapist, the patient/family, and physician as necessary.
- Coordinates with Patient Care Coordinator in making referrals to appropriate disciplines.
- Coordinates monthly supervisory visits from the therapist.
- Attends and participates in patient care conferences, meetings, and mandatory in-services as scheduled. Read the minutes of patient care conferences not attended.

Job Requirements:

- Requires completion of an accredited physical therapy assistant program and current State licensure.
- Minimum of one year’s therapeutic experience in a non-home health care environment; prefer two years of experience.
