We are seeking a licensed Physical Therapist with a strong interest or experience in pelvic health therapy to join our specialized team! This position is based in Cary at a dedicated pelvic health facility, where you will work alongside a collaborative team of seven pelvic health therapists onsite.

You will also be part of a larger, well-established pelvic health program that includes 14 additional Pelvic Health PT/OTs across six locations.

Summary: This is professional level work involving the evidence- based evaluation of patient disabilities, planning, and administration of physical therapy treatments.

Incumbents complete documentation instruct patients, families, and students and participate in clinical program development and management.

Clinician is expected to have involvement in the areas of continuing education, departmental and community activities, outreach education and role of preceptor to students.

Responsibilities: 1.

Diagnostic Testing
- Administers basic & complex diagnostic tests and evaluations in the area of Physical Therapy for the treatment of mobility disorders and related impairments 2.

Education
- Teach & supervise graduate Physical Therapy students and/or clinical fellows/residents.

Develops or contributes to innovative programs to enhance patient care.

Serves as resource consultant to other clinicians in respective specialty area.

Evaluation of Care
- Analyzes results of diagnostic and or therapeutic testing to establish patient goals and or modify subsequent treatment programs.

Identifies expected patient outcomes.

Communicates relevant information to promote continuity of care.

Implementation Selects and applies current and advanced techniques to provide quality and efficient rehabilitative services.

Provides habilitation/rehabilitation and maintenance of persons with mobility related impairments.

Organizes and prioritizes care according to patient/family needs.

Outreach/Community Involvement-Serves as resource to North Carolina government departments and statewide health professionals in specialty areas.

Presents at local (university/hospital) or state level, participates in community health screening and events.

Participates in the development of new clinical programs to meet emerging needs in conjunction with the clinical specialist and/or department director.

Participation
- Contributes to quality of clinical services through attendance and participation in departmental meetings and in-services.

Quality-Contributes to quality of clinical services through maintenance of valid & reliable records of patient procedures & progress.

Participates in quality improvement initiatives with the clinical specialist and other healthcare professionals.

Participates in continuous quality improvement efforts in conjunction with the department director.

Job Summary:

The Physical Therapist is responsible for providing and supervising primary care to approximately 25-35 patients in their home setting. This includes age appropriate interventions for the client that meet the physical, emotional, developmental, psychosocial and educational needs of the patients whom requires rehabilitative care. The Physical Therapist is responsible for the coordination of all rehabilitative disciplines, throughout the episode of care. The Physical Therapist communicates with all team members including RNs, LPNs, Therapists, Social Works, Home Health Aides, the Physician, patient, family, and other internal and external individuals to ensure that the patient's plan of care is appropriately developed, implemented and that goals are met. The Physical Therapist supervises the PTAs and Home Health Aides, throughout the patient plan of care. The Physical Therapist participates with leadership to develop and implement processes to continue improvement with patient and agency outcomes.

Core Responsibilities and Essential Functions:

DIRECT PATIENT CAREAssesses, coordinates with other disciplines and manages patients total needs, following physicians orders, using the physical therapy process Develops patients plan of care coordinating with other disciplines Manages patient from admission through recerts to discharge and appropriately scheduling visits per physicians order: adjust frequency as needed, based on patient clinical status and MD order Performs routine and complex therapy procedures to meet the need of the patient COORDINATION OF SERVICES FOR EPISODE OF CARERevises the plan of care based on patient need within physician orders, coordinating with other disciplines to ensure goals are met to achieve positive outcomes prior to discharge Supervises PTAs and Home Health Aides throughout the continuum of care. The Clinical Nurse Coordinator (CNC) ensures and delivers high quality, patient-centered care and coordination of all functions in the unit/department during the designated shift.
