POSITION SUMMARY/RESPONSIBILITIES
Care Coordinator will be instrumental in assisting the department and clinicians in the Ambulatory setting by gathering information, coordinating utilization efforts, and reviewing HCC quality indicators, and RAF scores to eligible Medicare Advantage beneficiary. Will monitor opportunities within the Medicare managed group to enhance financial outcomes. Will coordinate the transition of care and the interdisciplinary treatment for Medicare managed patients across the healthcare continuum. Facilitates the delivery of services, evaluates effectiveness, tracks outcomes and functions as the patient advocate to identify and communicate health care needs. Works collaboratively with clinical staff, clinic leadership, and outside agencies in an effort to improve patient outcomes, compliance, and decrease complications.
EDUCATION/EXPERIENCE
Graduation from an accredited school of nursing with current RN licensure in the State of Texas, BSN preferred. Three years recent, full time hospital experience preferred. Work experience in case management, utilization review, or hospital quality assurance experience is preferred.
LICENSURE/CERTIFICATION
Current license from the Board of Nurse Examiners of the State of Texas to practice as a registered nurse is required. National certification in related field is desirable. Case Manager Certification (CCM, CPHQ, or ANCC) or Certified Diabetes Nurse Educator certification is highly desirable. Must have a current AHA BLS Healthcare Provider or AHA BLS Instructor Provider card.

Job Summary

Responsibilities

• Administers medications in accordance with hospital policies; Demonstrates knowledge of medications administered to include normal dosages, side effects and expected out comes; Documents according to policy using electronic medication administration and bar-coding; Utilizes tools and resources to supplement knowledge.
• Adheres to Standards and recommended practices for specific nursing service area(s).
• Delivers care in accordance with hospital policies and procedures; Implements care practices such as Infection control procedures, care planning, safety measures, etc.
• Understands that attendance affects team morale and confidence. Demonstrates reliability and punctuality in regard to attendance. Consistently is present and ready to begin work activities at start of shift, stays within established break periods, arrives on time for meetings and leaves work area promptly at the conclusion of the shift to reduce potential distractions. Schedules personal appointments or requests adjustment to schedule with least amount of disruption to service delivery. Provides prompt notice for unexpected absences.

Job Duties Provides safe and efficient operation of equipment used for the movement of packages/documents/heavyweight, dangerous goods and ULDs.

Evaluation and treatment of patients primarily in the Rehab hospital and LTAC hospital determined by business need; daily documentation and charging in the EMR; education of patients, families and staff as appropriate; communication of patient progress in clinical conference; communication with other team members daily; preparing equipment orders; contacting vendors for equipment; communication with outside physicians and other professionals.
