Overview:
The Pharmacy Student works under the supervision and guidance of Mercy Pharmacists. The Student assists with the day-to-day inpatient and clinical pharmacy operations. The duties include IV preparation, compounding medications, order verification, picking medications, loading Omnicell machines, wasping medications, packaging medications, buying, assisting with clinical pharmacy functions, pulling out-of-date medications, answering nursing and medical staff questions, documenting clinical interventions in Pharmacy OneSource and other duties as assigned. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Qualifications:
- MINIMUM REQUIRED
- Education: Student enrolled in a pharmacy school recognized by the state of practice Board of Pharmacy or successful completion of the foreign Pharmacist Graduate Equivalency exam.
- Licensure: Intern licensed by the state of practice Board of Pharmacy (or licensure eligible).
- Other skills & knowledge: Basic computer skill. Effectively communicates using the English language.
- PREFERRED
- Experience: Two years of work experience in a pharmacy is highly desirable.

JOB DUTIES/RESPONSIBILITIES Duty 1: Enters and fills prescriptions timely and accurately.
Files prescriptions according to legal requirements.
Prepares billing statements/receipts for pharmacy purchases.
Answers pharmacy window and phones; triages phone calls for pharmacist.
Duty 2: Performs pharmacy billing functions, department and patient specific.
Fills and delivers floor stock items/narcotics timely and as appropriate.
Replenishes crash cart items Duty 3: Fills physician orders both manually and utilizing the robot.
Understands the basic dispensing properties of the robot.
Delivers medications to the nursing units and picks up new physician orders.
Retrieves orders received via the tube or fax.
Double checks the technical aspects of medication order entry.
Assists pharmacists in maintaining consistent work flow.
Prepacks medications as needed.
Duty 4: Is competent is all aspects of IV preparation.
Maintains a clean work environment and ensures sterility at all times.
Is competent on the safety requirements of admixing cytotoxic agents.
Understands and is able to use the TPN compounder.
Performs required Quality Assurance checks.
Duty 5: Maintains inventory control.
Is able to use the computer software for ordering.
Checks in the order accurately and notifies the pharmacy buyer or pharmacy operations manager of any discrepancies.
Prevents running out of any medication or supply.
Is conscious of overstock and understands the financial rationale behind a tight inventory.
Duty 6: Maintains the integrity and security of medications throughout the pharmacy and the hospital, by routinely checking for outdated products and for appropriateness of floor stock.
Performs unit inspections every other month.
Duty 7: Maintains pharmacy certification by completing the necessary CEU's required.
Is willing to take on extra projects.
Is involved in multidisciplinary committees within the hospital.
Duty 8: Supports fellow staff members.
Maintains a positive attitude regardless of work flow.
Follows through on problem resolution to the satisfaction of all involved.
Promotes good relations between all pharmacy staff members.
Maintains a professional attitude when dealing with other healthcare professionals, patients, etc.
Always provides upward support of leadership within the organization.
REQUIRED QUALIFICATIONS High school graduate with demonstrated Math/Science skills Completion of a pharmacy technician training program and registration with the Ohio Board of Pharmacy within 6 months of hire Ability to enter outpatient pharmacy prescriptions into the current computer system Ability to mix chemotherapy medications and IV products Ability to correctly bill for medications Typing and computer skills required Ability to perform short jobs and repetitive activities with accuracy Ability to lift up to 35 pounds, push, pull, climb, stoop, kneel, stand and walk a moderate amount.
Some exposure to heat, cold, bodily injury and toxic substances.
Visual acuity required Positive service-oriented interpersonal and communication skills required.
PREFERRED QUALIFICATIONS 2 years of experience as a pharmacy technician preferred PHYSICAL DEMANDS This position requires a full range of body motion with intermittent walking, lifting, bending, climbing, squatting, kneeling, twisting, sitting and standing.
The associate must be able to stand for five hours, walk for two hours and sit for one hour per day.
The individual must be able to lift 35 pounds.
The associate must have corrected vision and hearing in the normal range.
The individual must have excellent eye-hand coordination and reach work above their shoulders.
Associate must have excellent verbal communication skills.
Also requires exposure to chemotherapy and heat, cold, bodily injury and other toxic substances.
This position is classified “at risk” for possible occupational exposure to blood borne pathogens (HBV, HIV, etc.)

Summary:
The Specialty Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt medication therapy. Specialty Clinical Pharmacists provide proficient direct patient-centered care in a decentralized practice setting. In this clinical role, Specialty Clinical Pharmacists participate in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas. Pharmaceutical care services include but are not limited to assessing patient needs, incorporating age and disease-specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Specialty Clinical Pharmacists serve as departmental resources and liaisons to other departments, or external groups. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Specialty Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for Doctor of Pharmacy students and pharmacy residents. The Specialty Clinical Pharmacists will participate in the quality management program with the goal of improving services by monitoring processes, analyzing data, implementing interventions to improve, and evaluating the effectiveness of those interventions.
The Specialty Clinical Pharmacists will be responsible for the clinical management of patients on a variety of specialty therapies for medical conditions including but not limited to Hepatitis C, inflammatory conditions (gastroenterology, dermatology, asthma, and allergy), pulmonary conditions (idiopathic pulmonary fibrosis, cystic fibrosis), neurology (multiple sclerosis, migraine headache), dyslipidemia, osteoporosis, and oncology.
Responsibilities:

Facilitates pharmaceutical care services, direct patient care programs, and medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards.
Actively identifies practice-related issues that require evaluation and facilitate clinical research projects, quality improvement initiatives, or healthcare provider education as needed to advance practice.
Assures pharmacy is an integral part of the healthcare delivery system and facilitates enhancement and expansion of pharmacy services/programs.
Delivers direct patient care
Maintains proficiency in the clinical pharmacy program.
Works as an active member of the multidisciplinary team and collaborates with healthcare providers to provide patient-centered care.
Identifies high-risk patients and implements interventions to improve quality and safety.
Makes appropriate evidence-based, patient-centered medication recommendations.
Oversees the liaisons’ workflow to ensure timely review and processing of orders.
Oversees the prior authorization and appeals process.
Offers clinical assistance to the technician staff as needed.
Performs and documents initial and follow-up assessments and consultations in line with program policies and procedures and URAC and ACHC accreditation requirements.
Participates in a rotating after-hours on-call process.
Provides pharmaceutical services throughout the health system
Maintains proficiency in hospital computer systems and medication ordering systems.
Provides accurate, safe, timely, and appropriate medication therapy based on patient age and needs.
Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, sub-therapeutic dosage, overdosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures.
Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, etc.)
Actively participates in stewardship activities and restricted medication programs.
Participates in pharmacy operations and medication dispensing as assigned
Maintains proficiency in and actively engages in operational programs and specialty pharmacy areas as appropriate for job assignment.
Facilitates experiential education and practice advancement
Maintains proficiency in preceptor roles and actively emulates education/mentorship skills.
Develops student, resident, and staff training experiences/competencies and creates new relationships for teaching and training opportunities.
Identifies, designs, and implements improvements in the medication-use system to advance patient safety, maximize therapeutic outcomes, or control costs.
Job Requirements:
Education/Skills
Doctor of Pharmacy degree from an ACPE accredited school of Pharmacy is required.
Must accrue American Society of Health-System Pharmacists (ASHP) Residency Preceptor status within 18 months of hire.
Experience
PGY1 residency or 5 years of clinical practice (community, ambulatory care, PBM, or hospital) required.
PGY2 residency preferred.
Specialty pharmacy experience preferred.
EPIC suite products experience preferred (EMR, Compass Rose, Willow Ambulatory)
Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine in areas appropriate for clinical assignments required.
Licenses, Registrations, or Certifications
Licensed pharmacist in the state of practice.
Pharmacy preceptor license within 90 days of start date.
Must obtain Certified Specialty Pharmacist (CSP) certification or other applicable board (e.g. BCOP, BCACP, BCPS, BCMTMS, BCGP) certification within 18 months of hire.

Be the Pharmacy Director You've Always Strived to Be
- Convey the mission of the hospital and organization to pharmacy staff, holding them accountable for performance, while motivating them to enhance and develop their capabilities.
- Oversee the pharmacy department's operations, including scheduling, continuous assessment, and the implementation of process improvements to optimize department performance.
- Provide direct patient and pharmaceutical care.
- Demonstrate and maintain competence in clinical health system pharmacy practices, including antimicrobial stewardship, non-sterile and sterile drug preparation, controlled substances, hospital pharmacy accreditation, anticoagulation drug therapy management, and medication safety.
- Educate staff on pharmaceutical care and medication management, ensuring the pharmacy consistently complies with local, state, and national regulations.
- Stay current with new medications, indications, and evidence-based medicine. Collaborate with the Controller to prepare and manage departmental budgets for staffing, drugs, and supplies.
- Actively serve and lead various hospital committees, including Quality Assurance, Formulary, Pharmacy and Therapeutics, MEC, and Safety, as needed.
- Celebrate the achievements and victories of our dedicated employees along the way.
Qualifications
- Licensed in good standing by the State Pharmacy Board for the state where the hospital is located.
- Successful completion of a degree from an accredited pharmacy program or one recognized as substantially equivalent to a U.S. accredited program by an appropriate certifying agency.
- A minimum of 2 years of supervisory experience.
- May be required to work weekdays and/or weekends, evenings and or night shifts.
- May be required to work on religious and/or legal holidays on scheduled days/shifts.
